On 6/25/17 at approximately 5:06 AM, Transportation Specialist Eric Warrior notified Weekend Supervisor (W/S) Henry Blume about accident that occurred in the truck yard. Immediately, W/S Blume made his way to the location of the incident , Slip location F25, to take pictures and to document the necessary information. Upon arriving at slip location F025, W/S Blume made contact with RJ Davis yard jokey Patellis Santorinios. At the time of the incident, the Yard jockey was driving tractor 211. The Yard Jockey went on to say that, while attempting to back trailer 838682 into a slip location, he misjudge the distance and hit trailer 887105. Some damage can be seen on the driver’s side top corner of the trailer.
Robert Stone has served a supervisor for Elliott Construction for approximately 27-28 years. Scott Elliott is his direct boss. Prior to working at Elliott Construction, he worked for CG and Company for about four years as a backhoe operator. He has a 9th grade education and never received his high-school diploma. He got arrested a couple years back for a DWI. Mr. Stone took an eight-hour TEEX safety course on excavating and shoring. He cannot recall when he took the course, but he guesses it was more than five years, but less than 10 years before the accident.
On December 22, 2015, at about 6:20 pm amazon associate Girard Pedley notified James Weisgable that his car was damaged by another car. The case was then passed down to security officer Tatiana Kania to review some footage. At end of shift Girard Pedley approached his car and had noticed that a blue pickup truck’s bed was parked on top of his car. He noticed it and backed his car up to check if it was damaged. He saw that the tow hitch of the truck had made a small scratch and peeled off some paint. After reviewing camera footage; using camera C244 port 1 and camera C074, Tatiana was able to see that the blue pickup truck had parked at 4:03 pm. The owner of the blue truck had come in to the building at 4:05 pm. At 4:05:06 the truck had rolled
I viewed video surveillance from LRHS camera #7.24 at approximately 3:04 p.m. and observed a Red pick-up while backing up strike the vehicle in question left rear back fender near the back tire. At approximately 3:05 p.m I observed the suspect getting out of the truck and walking up to the vehicle in question and kneeling down near the left rear area as if he was assessing the damage. Margaret Harris (W) contacted me and told me she had witnessed the collision. Margaret stated she was stopped behind Cheryl's vehicle on the eastbound Hwy 18 off ramp to West Valley Hwy S, and Krystyna's vehicle was stopped next to, to the south of, Cheryl's vehicle in the right turn only lane. Margaret said the signal turned green, Cheryl's Impreza began to make a right turn onto West Valley Hwy S, but Krystyna's Odyssey went straight ahead through the intersection instead of turning right, and the front of Krystyna's vehicle struck the passenger side of Cheryl's vehicle.
On October 4th 2016 Deputy Akins was dispatched to 8821 Centennial Road for an accident involving a red truck that left the roadway and struck a parked vehicle. Upon arrival Deputy Akins saw a 2013 Red Dodge 1500 in the ditch with the truck’s front end smashed into the rear end of a 1001 Honda Civic. In the driver seat of the dodge truck was a male Caucasian. Deputy Akins recognized the male as Evan Case. Deputy Akins asked Mr. Case if he was injured. Mr. Case replied with I can just pay the deductible and leave right. Deputy Akins asked Mr. Case again if he was ok or if he needed medical. Mr. Case replied no.
The driver of vehicle 1 pulled out of the Kroger parking in front of vehicle 2 and had a collision with vehicle 2. The driver of vehicle 2 was traveling north on N. State Rd. in the inside lane at the time of the collision. Vehicle 1 had damage to the front driver’s side wheel and fender area. The damage to vehicle 2 was to the front end area. Vehicle 1 was towed by Montague towing to Patriot Collison. Vehicle 2 was able to be driven.
I spoke to the drivers who were all out of their vehicle and appeared that they were going to transfer information. I asked “Who was injured?” They replied that everyone was OK but one driver, later identified as Erin Feeney had injured her knee/leg. I requested Goodwill Ambulance to check on Feeney. I then asked “What happened?” Becky Boone who was operating a 2017 Silver Ford Focus stated that she believed a white vehicle struck her from behind causing her to collide with the vehicle in front of her.
Mr. Alvarado approached my client suddenly after running a red light. The negligence of your insured caused Ms. Tellez to collide into Mr. Alvardo’s left side of the door after Mr. Alvarado ran a red light. Ms. Tellez bags deployed, causing great damage to her car. See Photographs, attached as Exhibit A. The two-car collision occurred suddenly and without warning.
On October 21, 2015 at approximately 1319 hours, Investigator Nyesha Bradley received call from Port Rail Supervisor Derek Midkiff via cell phone, reporting an incident on Ryan and Sallier Street in Lake Charles, Louisiana. Midkiff advised that as the Port Rail locomotive was crossing the intersection at Ryan and Sallier St., the locomotives hand rail struck the mirror of a minivan that was stopped in close proximity to the railroad tracks. Midkiff reported that no one appeared to be injured or reported any injuries; and, damages sustained appeared to have been on the mirror of the van only. The driver of the van sped off; hence, no contact was made with the driver to report any further information.

Zamudio, Human Resources Administrator and acting custodian of personnel records of the Domino Realty Management Company who allowed access, and copies in support of any relevant information pertaining to any injuries, had located a “Work/School Status Report” under the name of the “Talbert Medical Group.” The document had placed the claimant off from work from 2-5-01 through 2-5-01 for pain to the claimants left knee, and yet, according to Ms. Zamudio, the document did not state that a work related injury occurred as there were no other documentation in support of an injury. Furthermore, the witnesses had not cited any job related incidents where the claimants left knee from 2001 had been injured were the alleged 2001 left knee had been irritated or exacerbated in any
When I arrived on scene I spoke with three individuals who were involved in the collision. The operator of the White Ford F-250 was identified as Albert John PIKITUS and his passenger was identified as Nathan GULDEN. Both stated that PIKITUS was driving south on Swatara Road when they observed a vehicle travelling north on Swatara road. They said the vehicle began to travel in their direction and they thought she was going to strike them so PIKITUS stopped his truck in the road. They said they could see the eyes of the operator and she never looked at them and was only looking at the turn where she later turned right onto Florida Avenue. The operator turned into their vehicle making a wide right turn. As they sat stationary the female struck the driver's side of the truck.
