Case Study One presents Michael Anderson, a 48-year-old male with a range of comorbidities including a 9-year diagnostic history of Insulin Resistant Diabetes, Morbid Obesity – BMI of 57.09m2, hypertension, depression, sleep apnoea and gastro oesophageal reflux disease (GORD). This essay will identify and discuss two Complex care priorities using the Levett-Jones Clinical Reasoning Framework (Levett-Jones, 2013), this framework will be used as a structure to establish goals, interventions, means of evaluation and will provide alternative interventions. The Two Care Priorities that will be discussed are Management of Diabetes & preventing further complications; and Depression and social isolation. As the community nurse, the main goals to address this care priority are to manage diabetes effectively to prevent further complications from occurring such as Acute or Chronic Kidney Disease, metabolic syndrome, cardiovascular disease and Stroke (Bullock & Hales, 2013). The RACGP & Diabetes Australia (2016) guidebook states that individuals with a BMI > 40 kg/m2 should initially consider reducing body fat by 5-10%; to improve diabetic status and reduce the negative impacts and consequences that the Diabetes; teach healthy habits to assist recovery and continuation of life; reduce fatigue and increase quality of life. Interventions to achieve the goals are: Continuation of Low Energy High Protein (LEHP) diet advised by the dietician, in conjunction with regular physical aerobic exercise, at least 30 minutes per day, initially a few days a week increasing up to every day as per tolerance (RACGP & Diabetes Australia, 2016). Asking Michael to keep a diary of his blood glucose levels throughout each day, documenting the time and reading in combination with what he has eaten, will help to better manage and promote awareness for Michael over his diabetes. Patient education about his conditions, the interrelatedness and interconnectedness, ensuring that Michael knows how to correctly check his blood glucose levels and correctly administered his insulin; importance of complying with medication management regime.
