R/s Tenesha (mom) stated that Keshaun has asthma. R/s in Keshaun’s school record states he has asthma, but there is medication or in haler at school for him. R/s on 10/19/2016, there was a home visit for an attendance plan. R/s there were meetings scheduled for 11/15/16, 12/13/2016 & 01/12/2017 and Tenesha didn’t call or come in for meetings.
ALLEGATIONS:
R/s Keshaun missed 24 days of school, 23 days are unexcused and 1 medical absence. R/s reportedly Keshaun has asthma.
UCM: CPSW met with Mr. Mauer for supervision at Brookdale and the visit went well so far. Mom and dad played with their son. Ms. Mauer changed Kaison's diaper and feed him during the meeting. Kaison seemed little tired today. Mr. Stark joined us today and he mentioned that he was in jail for two days and his case was dropped because it was wrong allegation from a friend. Ms. Mauer, Mr. Stark will be meeting their county nurse today after supervsion visit is done. Kaison is doing well and he had no fever and no coughing during the visit. The foster provider stated that since he got the shot for croup to help him breath better he has been doing well. No fever and no coughing so far according the foster provider. R.J. is a 15-year-old boy with a history of asthma diagnosed at age 8. His asthma episodes are triggered by exposure to cats and various plant pollens. He has been using his albuterol inhaler 10 to 12 times per day over the last 3 days and is continuing to wheeze. He normally needs his inhaler only occasionally (2 or 3 times per week). He takes no other medications and has no other known medical conditions. Physical examination reveals moderate respiratory distress with a respiratory rate of 32, oximetry 90%, peak expiratory flow rate (PEFR) 60% of predicted, and expiratory wheezing.
R/s possible physical abuse in regards to Ky’leah (7), Janiah (4), and Hailey (3). R/s it was reported that Shaquanda’s (mom) girlfriend Sada had beaten Ky’leah with a thick belt leaving a welt. R/s Sada had threaten to break Ky’leah’s neck. R/s Shaquanda works a long hours as a correctional officer.
Mom is very open to the asthma education. However she had specific requests regarding her availability. She visits the Hospital for Special Care (see attached) everyday where her son is currently staying. In addition, she needs to be available to care for her other three children after summer camp, and she wanted the education completed before her son returns home from the hospital. Lastly, we do not need an environmental conducted as Chris has already completed one.
R/s every time the children visit their mother, Cassie they are sick. R/s the mother takes the children to Medical Group of the Carolinas Pediatrics Westside. R/s Leeanna needs glasses, but has not been seen. R/s Daryl threatened to remove children from Medicaid so mother can’t take them to the doctors. R/s at the beginning of the month no one was at home to get Leeanna off of the bus. R/s Leeanna was taken back to school and was for 2 weeks.
R/s Kim is sending Kadyn to school with medication and it is against school policy. R/s last school year Kim was advised not to send medication with the child. R/s last week Kim sent the EpiPen and this week she sent Benadryl with Kadyn. R/s she called Kim on yesterday regarding the medication policy and Kim response was “okay.” R/s Kim gave permission for the school to change Kadyn’s clothes at school. R/s the school has clothes for Kadyn due to the strong odor. R/s Kadyn’s bookbag has to sit outside of the classroom because of the urine smell. R/s last year, they had the same issue with Rodney.
The school called the reporter (mother) because of the children behaviors. The reporter states Ke'eric has ADHD, and he has been acting out. She could not get to the school, and she called Alvin to go to the school for her. Alvin removed the children from school, took them home and whipped (beat) them with an extension cord. Ke'eric has bruises on his back, under his butt area. Taylissa has bruises on the side of her hip, by her butt area, as well as one of her arms. The children are dark skinned and the bruises are seen. The reporter states she does not have a problem with Alvin whipping the kids, but it is how he did it and the object he used that bothers her. CW then told Iva that she was going to be very honest at this time and that needs Iva to be sure that she is telling the truth. CW then stated that it has communicated to CW that the children Mariah and Briahna have come to the home of the boyfriend to stay the night. Iva stated that this is not true. Per grandmother, the client’s pregnancy, and delivery were normal. The grandmother recalls that Keisha’s developmental milestones were reached appropriately, including basic motor skills such as crawling, walking and fine motor skills such as writing. According to the client, she experienced an accident when she was nine years old. The accident affected her brain causing her to experience a coma for more than a month. It took her several months of rehabilitation to be able to return back to school. Keisha reports fatigue and/or loss of energy very often. Even after sleeping for more than twelve hours per day she feels tired every morning. She has been feeling this way for more than three years. Furthermore, the client reports that she has poor appetite, eating one or two meals per day. Due to her decrease in appetite the client reports that she lost about ten pounds since last year. Keisha mentions that her appetite has decreased since she started high school. Per client, her father abused drugs when he was younger, which included heroin and crack. The media likes to make things more dramatic than they actually are, and they will make a big deal out of a small situation, for example, global warming. The media enjoys to portray global warming like the Earth is going to blow up in a few months. Global warming is not a major issue right now because it does not affect human lives, the average temperature has not been significantly changing, and predictions about the impact of global warming have been proven wrong by professionals.
The child I helped take care of was a 15-year-old adolescent male. He was brought into the hospital due to an asthma attack. According to his mom, he has had asthma for years and sometimes has more severe attacks causing him to come into the hospital. Throughout this paper, this child’s developmental stages and medical diagnosis will be discussed.
