The reporting party (RP) stated resident Arev Dabbagh DOB: 12/27/00 disclosed on 4/15/16 she inappropriately touched by a male peer. According to the Arev on 4/14/16 a peer Ibraheem Mamoori DOB: 9/2/98) placed his hand on her shoulder and continued downwards towards her breast. The peer touched her breast over he clothing. The resident was distraught while she recounted the incident. Arev alluded to their being other incidents with the peer and other unknown males. Arev didn't disclose details or clarification. Arev was placed voluntarily in the facility by her mother and will continue her placement for a few more weeks. The peer is currently placed on a 5150 hold and will not have access to Arev in the future.
I presented Esteban’s case during case consultation because I noticed that I had countertransference when doing his intake. My expectation when presenting this case was to receive feedback that would help me to provide effective treatment to Esteban without my countertransference affecting his treatment. Between the years 2008 and 2010, the VHA hospitals and outpatient clinics that participated in the MOVE! program, screened for obesity and had 66% to 95% eligible patients within that time span. The number of patients attending the first session for the program has doubled. Approximately two-thirds of the 114,541 patients that attended the MOVE! treatment in 2010 said it was their first time attending. The other third has been participating in the program for years. The number of participants joining the MOVE! program has increased within the past few years.

Over the past 38 years, the National Coalition Against Domestic Violence has been a leader in providing services and resources to victims as well as survivors of domestic violence. Originally, the NCADV began at the United States Commission on Civil Rights working to help battered women, but it quickly expanded. The group grew from only one hundred members to thousands of members and they worked together in order to help others with their experiences not only with domestic violence, but also homophobia, sexism, racism, and ageism. The NCADV officially became a nonprofit organization on September 22, 1978, in which they placed their focus on victims and survivors of domestic violence as well as abuse.
Most nurses decide to take this journey to transition because acute care settings are switching towards an overall RN health care provider, and this makes it harder for the LVN to find a position in the hospital. In my case, transitioning to a role of an RN means becoming more knowledgeable and confident as a nurse, as well as being able to do more for my patients. The expectations are higher of the RN’s even if they have some of the same duties as the LVN’s. In my understanding, the RN’s have to analyze issues even further and use critical thinking to determine what certain conditions are and how they relate to the patient. Richard is doing his best to provide great services to Veterans, however; following case reviews Richard was advised not to serve Veterans with missing SBE’s, spouses of Veterans if they don’t meet required criteria or TSM still on active duty. Justifications for activity codes should be provided, as well as justification on why the Vet is added on the case note. There were some issued codes present and the follow ups should be in timely manner.

Mr. Smith is a 56-year-old Caucasian man who is being evaluated in the emergency room with progressively worsening chest pain that began 2 hours ago. The patient describes the pain as pressure in the center of his chest. He rates his pain 7/10. He reports the pain is now radiating down his left arm and through to his back, he is diaphoretic, and complaining of shortness of breath. He denies nausea or vomiting.
