On 02/11/2017 I was Dispatched to Smith Springs Elementary School for an investigation. A student Cire Kistner, was reported as being visibly distraught upon her arrival at school. She was taken to Lance Forman's office the schools principal and was questioned. The student reportedly told Forman that she and her sisters walked into the bathroom and witnessed their father, Eric Kistner, with a needle in his mouth and blood on his forehead. He was passed out on the floor. There was a spoon with "black watery play dough" on the counter. Principal Forman contacted DCS and informed them of the situation as well as MNPD. The reporting party (RP) stated referral #1143-3655-1102-7078895 was generated from the following referral #0534-9062-5752-307279 dated 1/28/15 regarding resident Nicole Morris age 14 DOB: 6/1/00. Per the referral Nicole is an alleged victim of sexual abuse, physical abuse and general neglect by an unknown perpetrator. According to the RP on 1/28/15 a staff member named Rebecca took Nicole to Long Beach Memorial Hospital clinic to be examined after she disclosed having suicidal ideation. Nicole revealed to hospital staff that she planned to hang herself. While waiting at the hospital Nicole disclosed being "fingered" by two different boys (names unknown) at the home. Nicole also disclosed that a girl (name not provided) beat her up. Referral 0290-1132-4581-1002176 dated 3/13/15 reported by Tatiana Garcia, Clinician with Bayfront Youth Group home.
I then proceeded to enter the residence and noticed a female was visibly upset and was crying. I identified the female as Mrs. Gloria Lipscomb Plato (Victim) and her daughters Marissa Dawn Plato (Victim) and Kara Plato. While on the bus the week of 08/17/15, Mattie bumped her head while sitting in her sister’s lap. The child had a gash in her head that bled all over the bus. Mattie may have needed stitches but she was not taken for medical treatment. The bus driver stopped the bleeding and bandaged the child’s wounds; the driver also told Mr. Cleveland what had happened and suggested she be taken to see a doctor. It was rumored that Mr. Cleveland did not take Mattie for treatment and made statements that she would be okay. CCIB Intake received a call from Melissa Sadikoff, MSW, LCSW from Orange Coast Memorial Medical Center who informed that a report was made by the reporting party regarding 1 year old Melanie Larmarre DOB: 5/17/11. The mother of Melanie informed the reporting party she had bathed her daughter approximately 5PM. During the bath the child indicated to her mother "it stings." The mother questioned the child asking "what hurts?" The mother had noticed blood on the tissue used by the child after using the restroom. The child indicated to her mother her vagina was hurting. SUMMARY OF ALLEGATIONS A report was received on 7/9/2017, indicating that Tammy Albury (mother) and Armando Guerrero (father) were involved in a domestic dispute in the presence of Cember Albury (CV-15), Devon Albury (CV-16), and Damian Guerrero (CV-1). According to the report, Armando went to Tammy’s truck to retrieve his property.
I, Cpl. Lessane, along with Deputy Ayer, responded to Hampton Regional Medical Center, to assist Deputy Smalls, with a runaway mental patient. Upon arrival, Deputy Smalls stated a mental patient, Travis Johnson, ran away from the Emergency Room with a set of black handcuffs on one of his arms. Deputy Smalls stated he wasn’t sure of Travis exact location at the time. Deputies along with assistance from the Varnville Police Department and the Hampton Police Department patrolled the area in an attempt to make contact with the patient. No contact was made at that time.

IRTC spoke with the assigned CPS, Nancy Rios, who reported that she made a visit to the case address on 6/22/17 and spoke with the family regarding the alleged choking incident. CPS expressed that the subject child’s younger sister indicated that she observed the incident and the alleged subject did grab the subject child by the neck and choked him during the altercation. CPS conveyed that she spoke with the subject child who reported that he was not choked during the incident and he was maneuvering like the “Matrix” when the alleged subject tried to grab him. On Tuesday, November 08, 2016, I, Officer B. McMillon, #135, of the Mansfield ISD Police Department was on assignment at Legacy High School located at 1263 N. Main in Mansfield, Texas 76063. At about 1130 hrs police were dispatched to 2404 K ST SE for the report of a physical domestic in progress. Upon arrival, Leondra Joyner reported that she was assaulted by her 16 year old dauthger, Ayana Holloway. Leondra explained that they were in a dispute, which is a daily occurance between the two, when Ayana struck Leondra in the side of the face with her fist. I observed a red mark and a small cut on the right side of Leondra's face. Ayana admitted that she did in fact strike Leondra during the dispute, but stated that Leondra struck her as well.
