This morning I am working 7a.m.-7p.m., on a really active Orthopedic Medical/Surgical unit in the hospital. I am assigned 7 patients today. I am of speaking on the phone to a physician about his client who has a history of Myocardial Infarction, Hypertension, and Hypercholesterolemia. The client’s vital signs had become unstable and he coded 10 minutes ago. I concluded with the physician that I called the Rapid Response Team, and that they are currently responding to his client. The physician has indicated that he would be in as soon as he can. I hung the phone up, and heard a loud distinct thump sound. Purposeful rounding has been shown to increase patient satisfaction with their hospital experience as a result of regular attention from nurses. There a three stages. The introductory round used during shift change, ask the patient about current needs and conduct environmental scan. The initial round are the same as the introductory expect that you explain to the client with more details and ensures that the client understands rounding and knows that it is purposeful. The hourly round is the same until 10 p.m., then it is modified for overnight hours ( 2300-0600). I would explain to the patient that I will come in to check that they are safe, however, I would not awaken them. During the purposeful rounding I ask the client, “What is the most important thing I can do for you, right now or today?” I check for pain, bathroom needs, pumps, machines, periphery, plans, reposition, their possessions needed ex. dentures, and I explain that I will be back again in an hour.
