The experimental question of this study was how metabolic alkalosis contributes to hypercapnic respiratory failure in adults with acute exacerbation of cystic fibrosis (CF) and what the relevant reasons of it were even though both the patients with CF and COPD became hypercapnic. Hypothesis: The hypothesis of this research article was that in comparison with patients of COPD, the patients with CF were more prone to develop metabolic alkalosis and there were several relevant factors associated with it, even though both group of patients have similar PCO2 levels and became hypercapnic. Colorimetric analysis are used to evaluate the status of electrolytes such as sodium, potassium and chloride. Albumin, creatinine and urea levels are also included in the study. They distinguish respiratory acidosis, metabolic alkalosis and mixed respiratory acidosis and metabolic alkalosis groups based on their ABG values and acid-base results on Stinebaugh-Austin diagram. According to the study, there are several factors associated with the acid-base imbalance, such as chloride exhaustion, malnutrition and hypoalbuminemia, extracellular volume contraction or hyponatremia. Pathophysiologically all these elements or factors leads to acid-base imbalance in patients with CF. therefore, the treatment should be focused on to restore acid-base balance and to correct hypercapnia by ventilation.
