If a health care service provider gets stagnant in the care they provide, and does not continue to research the needs of their community, and change their systems and processes of what kind of care that is available and provided, then ultimately the quality of health care given and patient satisfaction rates will decrease. Providing quality patient care for the patients is essential, but it is also imperative that the healthcare facility is consistently reviewing the community health assessment needs, and considering ways to provide those needs to the community. Identifying changes that need to occur within the community of Huron County required data collected by the use of secondary sources such as MercyHealth Willard website, the Community Health Needs Assessment Report, and the use of the Census of Population provided by the United Census Bureau. Two other big health risk factors for this community is substance abuse, and mental health disorders. Three of the biggest impacts on the healthcare community is obesity, substance abuse, and mental health disorders. These three risk factors affect the cost of care mainly due to the lack of services because patients that are affected by these health risk factors typically must travel outside to other local healthcare facilities which is at minimum fifteen miles away or more dependent on the which risk factor is being dealt with at the time. Another cost factor for these is the lack of education provided to the community, which causes increase health care cost due to the secondary health contributing factors the primary risk factor causes such as obesity can lead to diabetes, or substance abuse can lead to community-acquired diseases such as HIV, or cardiac issues and so
* Colorado Springs is very health driven community. There are hundreds of hiking trails and free places to get outside in the community.
Trinity Community Hospital conducted a focused community health needs assessment centering on cancer, orthopedic and cardiovascular services. The information was obtained through a multitude of sources including: epidemiological surveys, focus groups, interviews with healthcare professionals and healthcare facilities’ planning guidelines.
Healthcare is in a constant state of change with movements that impact rates, access and quality of care. Hospitals have become more competitive due to the rising cost of care delivery and the reduction in reimbursement from payers. This causes difficulty in delivering quality care to all patients, which is being measured by mandated patient perception surveys, Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS). HCAHPS scores are part of value
The determinants of health are economic and social conditions that affect people’s health status. These influence the living and working conditions that impacts people’s everyday living condition. Factors such as the place and the environment we live in, genetics, educational level or work status and income, as well as friends and family are some of the aspects that affects our health condition. On the other hand, the people that have an access to good health care and have financial stability are less susceptible and are often less affected. (NANB, 2009)
Strategic Planning is an essential first step in the development of a result-base accountability system,” (Schilder, 2013, p. 1). In strategic planning an organization must know their goals, missions, and how to reach them. Health care facilities are in huge demand for the elderly and for people who lives in low-income communities. A person who receives good quality health care services can prevent the spread of contagious diseases. This paper will describe the need of the community, population it is serving, and a brief description of the facility.
As a resident in Fresh Meadows for twenty years, conducting a community health assessment was an opportunity to learn more about the neighborhood. Located in the northeastern section of New York 's Queens County, this small town was first named “Flushing Meadows” before the American Revolution. This was due to large areas of wet meadows that were also partially located in Flushing. Nonetheless, Fresh Meadows has become home to many with its diverse community in cultures and ages, and activities for the local.
In the United States we are very fortunate. For the most part we do not live in fear of disease outbreaks from day to day. Our government along with the Centers for Disease Control and Public Health Departments have managed to have adults and children vaccinated and set up recommendations regarding those vaccinations. Despite all that is done our country allows freedoms. One of these freedoms is to not vaccinate your child from communicable diseases due to personal beliefs. With the worlds populations becoming more mobile, that sets your child up for dangerous encounters; because some countries are not as strict on laws regarding vaccinations and some
Measles was discovered in the 19th century by a Persian Doctor, but was not recognized until 1957 as an infectious agent in human blood by a Scottish physician. In 1912 measles became a notifiable disease and in the first decade that records were kept there was on average 6,000 deaths per year reported from the disease. (Center For Disease Control). There is no definite origination of measles but scientists believe that it dates back to the Roman Empire about the 11th and 12th century (NCBI, 2010). The first outbreak known in America was in 1657 in Boston,

This assignment will investigate the Community Health Profile for Oldham, a metropolitan borough within Greater Manchester. Oldham has a population of around 224,900 (Oldham Council, 2017). At present, there are several prominent health issues in Oldham, according to Public Health England’s Health Profile (2017), such as: levels of obesity in Year 6 children; under 18 conceptions and children in low income families. The intention of this assignment is to gather further evidence and talk in more detail about one specific health issue in Oldham, and how nurses can promote and improve the health of young individuals in this area.
A comprehensive family assessment provides a foundation to promote family health (Edelman & Mandle, 2011). 1987 Marjorie Gordon purposed 11 functional health patterns to use for guidance in order to facilitate nurses to have a frame work for the family assessment in. Gordon’s 11 functional health patterns help organizes basic family assessment information. This standardized format will guide nurses to complete the family assessment using system approaches, which will identify a patient’s
A family consists of a group of interacting individuals related by blood, marriage, cohabitation, or adoption who interdependently perform relevant functions by fulfilling expected roles. (Edelman, Kudzma, & Mandle, 2014, p. 150)
Using Gordon’s functional health patterns to assess a family will guide the nurse in developing a comprehensive nursing assessment that is holistic in nature. Gordon’s functional health patterns are founded on 11 principles that are incorporated within the nursing practice. These 11 principles serve as a framework for a thorough nursing assessment in which to build a holistic and individual family care plan (Grand Canyon University, 2011). The author has developed family-focused questions for each of the 11 principles and utilized these questions as a tool to assess her own family. This paper will summarize the family assessment that was performed by the author and discuss two wellness nursing diagnoses that
The elderly have multiple health problems and struggle to stay healthy and able to care for their own needs. The elderly in the rural areas tend to have a number of needs that may be compounded because of their choice of dwellings. The rural person may need to seek health care in different nearby cities, or may find some health care providers in their own local communities. Many elderly or older adults would benefit from a health promotion plan that would focus on the needs of this age group. The components that this plan would focus on includes physical activity, proper nutrition, and coping with mental stressors. While all older adults could potential benefit from this plan, this plan will be offered primarily in rural communities in order
