On 05/15/17 at approximately 2242 hours, at the 4th Ave. Jail located at 201 S.4th Ave. Phoenix AZ 85003; while conducting a security walk in 2C 100 pod, SP-2 Inmate Arroyo, Daniel T326894 advised me he needed to be moved to a different location. I then questioned the Inmate Arroyo and asked why did he need to be moved? While speaking to Arroyo, I noticed his physical conditions and blood on his face, he advised to me that he had been assaulted and wanted to be moved from his current pod. Sgt. Weber A5273 the lead on level two at that time, was advised of the assault and directed me to review the video of 2C pod 100. On March 39, 2017, at 1224hrs, VA police received a Duress Alarm for 1E127. Investigation revealed that the Veteran was upset over not being able to receive her Ulfima Five Tens Unit. The MSA Clerk informed her that she would have to attend a class on how to properly us the Tens Unit. The Veteran demanded she give her the Tens Unit, because she had use it in the past. When the MSA Clerk refused and told her she would schedule her an appointment, the Veteran got angry and called the Clerk a witch and walked off. VA Police made contact with the Veteran at Primary Care Desk B lobby. Where the veteran gave an omission and stated she had an appointment and because the clerk was not at her desk she missed it. On Friday, 17 July 2015, at approximately 1400 hours, I, Officer Allison Bigham, was contacted by a Parks Services Supervisor, Andrew Jones, in regards to an associate who was reported to have their cell phone on them. Jones stated that Miller had already been told to put the phone in her locker; however, she was being uncooperative. After updating Sergeants Frank Long and Charles Drakeford on the situation, I was instructed to pick up the associate, Victoria Miller, and escort her to the South Carolina Interview Room in order to complete a compliance verification audit. Taylor and Officer Villalpando provided an appropriate response to a child in danger. I was not able to find an attempt to locate for the child, CID was not notified in which could have assisted, and an Amber Alert was being initiated without approval. Several hours had transpired from the initial call to when the child was entered into NCIC/TCIC. I requested a memo from Sgt. Taylor to clarify the events that transpired during the incident. In the memo Sgt. Taylor acknowledged that he was contacted by Officer Villalpando at 1250 Hrs. and that he advised him to have the child entered into NCIC/TCIC. There was no mention on attempting to locate the child or verify information provided by parents.
On 07-24-2017 at approximately 1135 hours I, Officer H. Solis P#2512, was dispatched to 3146 Mayfair Dr. reference a report of a runaway juvenile. Upon arrival I was met by Marry Russell at the door. Marry explained to me that her daughter (Hannah Russell) did not come home today after school. She stated that Hannah is currently a member of Central High Schools’ band and sometimes had band practice after school but today was not a scheduled practice day. While I was on duty and assigned as the 4C4 Housing Deputy, Inmate Sarra, Melissa came to the 4C4 Officer’s Station and handed me a Form 62 addressed to the Shift Commander that stated, in part, “I do not feel safe in my room. My roommate threatened me after you let us off lock down.” Sarra told me that Inmate Deak, Nichole was upset with her because Sarra inadvertently woke her up, and said to her, “If you don’t fucking stop, you are going to be fucking sorry.” Sarra said that no physical altercation had occurred. On 02/04/2017 at 1244 hours, I COII Benally badge #11837, responded to an Incident Command System in Dorm 2 F-pod initiated by COII Brown badge #1305 due to a green leafy-like substance found on inmate Pierre ADC #182318 during a random pat search. At the time I arrived, I observed COII Brown and COII Zamarron badge #11855 giving verbal directives to inmate Pierre to hand over the substance and inmate Pierre refused directives while walking away from staff towards the restroom area in F-pod and continued to refuse directives given by staff to submit to mechanical restraints. Inmate Pierre then walked over to the toilet in the corner and attempt to flush the substance, COII Brown and COII Zamarron grabbed inmate Pierre by the shoulders in attempt
While conducting medication pass with Nurse Snipes on Bravo Wing, Inmate Hawkins Kerry exposed his penis and appeared to be masturbating. When I told Hawkins to move away from the cell bars and cease his actions, he began to call Nurse Snipes and me racial slurs. Hawkins called Nurse Snipes a “Cunt nigger bitch.” Hawkins also stated that” I am a nigger and he owns me.” I attempted to counsel Hawkins about his inappropriate behavior, but he spoke over me and disregarded my orders. Sgt. Valachovic, Cpl. Dudley and Cpl.Nomikos responded to Bravo Wing and proceeded to deescalate Hawkin’s behavior, but he was not receptive to their orders. Once at HQ, I made contact with Andrew. I advised him there's a warrant for his arrest and to turn around with your hands behind your back. Andrew was then handcuffed without incident. As I was escorting Andrew to the booking room, he was inquiring about why he was under arrest. I advised it was a Theft from the above listed location. Andrew in an excited utterance stated, "I did steal that generator it was Dante Cuticchia and he stays in a Brooklawn Hotel, room 106." I advised Andrew at that he was under no obligation to reveal who else was involved in the theft since he was not Mirandized. Andrew was then taken into booking where he was process and photographed. On January 1st, 2016 at 9:20 PM, Security officer Canela came up to Main Screen and reported that an associate had bypassed secondary screening. Using camera Reuben.seasonal.c60, Supervisor in training Steven Gibbs and I were able to determine that the person of interest was an Integrity associate by the name of Nicholas Upton. By viewing camera footage, an observation was made that Officer Canela had advised Mr. Upton to proceed to secondary screening after triggering the metal detector. By further inspection of the footage we determined that an associate in front of Upton had triggered the detector and by tailgating the person, Upton is required by default to visit secondary screening Mr. Upton had persisted in explaining to Officer Canela
On August 28, 2015 at approximately 0325 hours, Security Operations Center (SOC) radioed East Orlando Officers regarding a patient assist in the Emergency Department room 30. Upon, arrival I met with Nurse Jon Jose who stated that patient, Heather Locklear (FIN: 70596879) was discharged but refusing to leave. According to nursing staff patient wanted to stay in the room till 0600 hours. I went inside the room and speak to the patient. She stated that she was waiting for a bus pass. Upon receiving the bus pass from nursing staff, patient agreed to leave without incident. At approximately 0330 hours I escorted patient outside the building where Officer Lourdes and I observed patient left premises without incident. Security did not have to go
This officer advised Alcantar she was not under arrest, however she was being detained for investigation for OWI. This officer secured Alcantar in handcuffs and had Off. Malerich search her. Alcantar was placed in the back seat of my assigned unit.
P claims she was sitting on her walker outside of her apartment building talking to her friend when she saw two MOS in the building lobby. P claims she saw the MOS slam a neighbor’s boyfriend against the mailbox and said that she was going to call his girlfriend. P claims that when she made the call one of the MOS (Lt. Lynch) requested that P give him the phone, and when P refused the MOS slapped her in the face. P claims she sat back down on her walker and the MOS lifted her up and kicked the walker. On Saturday, April 23, 2016, at approximately 2018 hours, I, Officer Kenneth Snipes, was patrolling with Officer Heidi Cassidy by Mini Striker when I noticed associate Ronnie Truesdale place his cell phone into his jacket pocket. I approached and proceeded to conduct a pocket audit, and the cell phone was recovered. At this time I put the cell phone in an evidence bag and wrote the associates name and ID number on the front.
