The driver, Cecilia Blair, of vehicle 1 was traveling north through the intersection of N. State St. and Flint St. when she had a collision with vehicle 2. The driver, Jacqueline Muir, of vehicle 2 was heading west on Flint St. when she was struck by vehicle 1. Witness state that vehicle 1 ran the red light heading north on N. State St. and the white Jeep had a green heading west on W. Flint St. One witness over head the driver of vehicle 1 state the light was yellow and turning red when she went through the intersection.
Unit 2 was driving eastbound in the left lane of 167th St. between Orchard Ridge Ave. and Anthony Ave. in Hazel Crest, IL. Unit 2's approximate speed was 35 miles per hour. A large puddle of water had formed from a very recent rain. It covered the right lane and part of the left lane. The driver of Unit 2 noticed the puddle and proceeded to slow down to a speed of approximately 20 miles per hour. The mother exited the house and was asked if she drove the vehicle. She related the car is hers but no one has driven the car for a few days. I advised the marks are identical to the other vehicle that was parked and struck. The female Jacqueline SOLIMINI stated the car wasn't moved and the damage has been on her vehicle for some time. I informed her that I have her mirror for her car in my possession. That the mirror was knocked off her car and remained on the scene. SOLIMINI denied the vehicle being driven and stated if it's just going to be some tickets to send them to me.

At approximately 1:28 a.m., I arrived at the scene and met with Trooper Otten, who advised what had transpired prior to my arrival. Trooper Otten stated a blue 2014 Ford F-150, driven by Eric A. Wenzel, was traveling northbound on Illinois Route 4 south of Norton Lane. While traveling northbound, the blue Ford veered to the left and entered the southbound lane of the roadway. Once in the southbound lane, the front passenger’s side of the blue Ford struck the front passenger’s side of a southbound silver 2003 Ford F-150, driven by Alan L. Schilling. Trooper Otten stated both Mr. Wenzel and Mr. Schilling sustained incapacitating injuries in the collision Trooper Otten advised the silver Ford’s front seat passenger, Sarah L. Wagner-Schilling, sustained life-threatening injuries in the collision and was not expected to survive. Trooper Otten said Mr. Wenzel admitted to having been drinking prior to the collision and was suspected of driving under the influence. All three occupants had been transported from the scene

Main St. The vehicle traveled through a red traffic light as it continued south on MO 11. As it approached the intersection of MO 11 and Park Circle Dr., the vehicle turned left onto Park Circle Dr. The vehicle continued on Park Circle Dr. toward the South City Park. The vehicle still failed to yield as it traveled onto Inman Drive. Inman Drive turned from asphalt to gravel. As the vehicle traveled southbound on Inman, I slowed down allowing a greater distance between the vehicle and my patrol car. While on patrol, I was heading northbound on North Three Notch Street. While traveling, I observed a burgundy Chevy Tahoe traveling southbound in the northbound side of traffic, approaching my patrol vehicle. To keep from being involed in a head on collision, I pulled my patrol vehicle into the Sunoco on North Three Notch Street. I then engaged my lights and siren and attempt to catch up to the violator vehicle. The Tahoe then pulled into the Piggly Wiggly parking lot and that was the final. I then identified the driver as William Anthony Thomas. I advised Thomas that he alost hit me head on and he stated, "I'm sorry, I am too fucked up." Thomas had an strong odor of an alcoholic beverage coming from his expelled breath. Thomas also had red glossy eyes. I then advised Thomas multiple times to step out of the vehicle to perform FST's. I noticed that Thomas had unsteady balance as he was waling to the back of the vehicle.
During the collision, a gold 2000 Saturn LS1, driven by Cheri E. Post, was reportedly being pursued by a black 2014 Ford Mustang GT, driven by Jordan M. Kinne, after a drug deal gone bad. The Saturn and Ford entered the ramp from Interstate 55 southbound to Tudor Avenue traveling the wrong way. While traveling from Tudor Avenue to Interstate 55, the Saturn and Ford entered the path of a silver 2005 Chevrolet Equinox, driven by Pierre D. Manley. Once in the path of the Chevrolet, the front passenger side of the Saturn struck the front passenger side of the Chevrolet.

On 05-26-2016 at approximately 2157 hours, Vehicle 1 was northbound on Hulsey Road approaching the intersection of Heatherfield Drive at a speed witnesses estimated to be approximately 50 mph. Witness statements also said Vehicle 1 had been performing a wheelie prior to the crash, but was down on both wheels about 4 seconds prior to the crash. Bicyclist 1 was riding from Heatherfield Drive and crossing Hulsey Road from east to west, violating the right of way of Vehicle 1. Bicyclist 1 was crossing the roadway in an area where there were no streetlights, traffic signals or crosswalks.
SF and ALL both agree that ALL driver passed by the intersection and that SF began to proceed forward. Both SF and ALL agree that ALL drivers began to back up. ALL contends SF did not use last clear chance. However both ALL and SF have a statement from the SF driving stating he tried to swerve. In SF Statement “after I saw him Reversing I tried to swerve to move uh to miss him”. In ALL statement “after I got into the intersection, I know that he—had the other car reversed and backed into my side. He goes onto say “I just tried to swerved out of the way to the left”. The point of impact pictures show that the ALL had backed up and the scraping down the side shows SF was in a forward moving motion.
Enroute, Dispatch advised me the vehicles may be blocking the eastbound lanes of travel. I arrived and found a sedan had run into the back of a truck & trailer. I exited my squad and learned there were no injuries. I then learned the sedan driver was Ryan Burnos & the truck & trailer owner was Keith Lapinoja. It should be known the Keith was not in the truck at the time of impact as he was snow plowing on Floral Dr. nearby and parked the truck & trailer on Co Rd 18.
