A) MSTT will examine with caregiver how to increase supervision and monitoring of the youth during school
B) Caregiver will contact the school and determine what they can do to help increase the supervision and monitoring at school
A, B) MSTT met with the caregiver to examine how she plans to increase the supervision and monitoring of the youth while out in public. Caregiver expressed while the youth is at home she is going to speak with her mother and his aunt to help while she's at work. MSTT encouraged caregiver to reach out to the school and ask them if they would be able to supervise and monitor him. Caregiver contacted the school and is waiting to hear a response. MSTT will continue to work with the caregiver in increasing the supervision
The guidance counselor stated that the other three teacher could not attend the meeting however they provided a report about Shonquasia’s grades and behavior in each of their class. Ms. Witkers stated, that the other teaches reported that Shonquasia is not doing well in their class, and that she is often caught sleeping and that she has an aggressive attitude toward the teacher and her peers. MS. Witkers stated, that the other teacher reported that Shonquasia is not complete assignment and homework. RORC teacher reported that Shonquasia is fail his class because she refuse to wear her uniform. Ms. Witkers, recommended that Shonquasia should be encourage to participate in her class, and to be more respectful to other. Ms. Witkers suggested that consequences for behavior and accountability should be reinforced in the classroom and at home. Per mom Shonquasia does not like to be told what to do and that Shonquasia is capable of getting better grades, however she do not want to put in the work required to get better grades. Per mom Shonquasia likes to do her own thing and it is very difficult for her to punish Shonquasia for bad behavior. Per mom, since last CFT meeting Shonquasia s behavior at home and in school has not
Intervention: CSP, MHS and the youth discussed weekly improvement in the home and community. CSP and Miasia review the youth’s feelings and thoughts for the past week in the home and school. CSP, MHS and Miasia discussed ways to improve mornings and keeping on task. CSP and MHS praised Miasia for her progress on this goal. The youth had zero incidents of disrespecting others belongings. CSP provided reinforcement interventions for respecting boundaries in diverse settings. CSP discussed medication management concerns with MHS.
Some examples of the occasions the child missing at school when they are allowed to leave the classroom or fails to return beacause they are not being supervized such as:
MSTT met with Sabrina to inform her of the report from the school. MSTT and Sabrina examined Esmrel behaviors and brainstormed what could be causing Esmrel to engage in these negative behaviors. Sabrina believes Drew should start to take a more active role in Esmrel school behavior and academics. She stated Drew need to step it up because she will soon be moving out and Esmrel will be staying with Drew and he needs to be on top of these behaviors. Drew have previous stated he does stay in contact with the school. Which is true what he does not know is the school is not telling him everything because of negative interactions from the pass. MSTT and Sabrina examined Drew statement of being on top of everything in the pass and was told he was to involved and needed to give the children a little more freedom. Now everyone is telling him he need to be more involved. Sabrina would just like Drew to be more involved with Esmrel academic and school behaviors.
On 04/22/16, Ken'Dell came to school with a red scratch mark on his face. The reporter stated that the child isnt complaining of pain or needs any medical treatment. According to the reporter, Ken'Dell told her the scratch on his face came from running into a pole. According to the reporter, the child told he ran into a pole while chasing his baby brother, that chased a cat. The reporter stated that the children were not properly supervised and was outside the home alone. Ms. Pendleton stated there are not any prior concerns and the child's basic needs are being met. Ms. Pendelton stated her primary concern is the proper supervision.
Caregiver stated she does not like the way the school communicate with youth. Youth failed three classes and wants to seek educational alternative programs. CM provided the family with a list of educational programs, the family agreed to follow-up with the programs. The family will keep CM updated in regards to education alternative programs.
A,B,) MSTT met with Jose and Juminia an address Jose after school routine. Which will they were willing to change. MSTT also examined with them the cost & benefits with Jose changing schools. Jose was able to further explain to his mother why he feels switching schools is better for him.
A, B, C) While assessing youth triangulation MSTT discovered it was being influenced by the parents. MSTT met with Rebecca to assess why she continues to make statements which are having a negative impact on how the children interact with their dad. Ex. (You do feel more comfortable with me right, You do not talk while you are at your dad house because you like it here more right, You know it's better here because you don't have to hear your dad disrespect
CM spoke to Ms. Quinones (caregiver) in regards to a follow-up on services for Taina (youth). Caregiver informed CM that youth met with IIC for the initial session. Caregiver reports she likes the IIC. CM was informed that caregiver attended the youth’s school meeting on 2/23/17. Caregiver explained to CM that youth teacher reports youth is failing her classes and she continues to misbehave in school. Caregiver also, stated that since 2/23/17 she has not had any phone calls from youth’s school due to her misbehaving in school. CM will continue to monitor youth’s behavior and grades in the school and therapeutic sessions with IIC.
MSTT stated working with the family identifying if they see value in Miguel attending school and if so what are the issues Sherrlly is facing getting Miguel to attend school. Sherrlly stated she clearly sees the value in a good education because of the mistake she made quitting school when she was 16 and is know struggling to maintain a job because of her run-ins with the law which lead to her getting a felony on her record. Miguel clearly stated he does not see the value of attending school because he feels as if he does not need to attend school. MSTT assess why Miguel feels the way he does and address with Sherrlly what strategies she have used to try and get Miguel to attend school.
The reporter received a text message from another student at the school; the child alleged that Darren had threatned to commit suicide. The victim has no known plan or history of acting on his threats. Ms. Cleveland was called to the school and came to pick the child up with plans to take him to St. Francis Hospital for evaluation. When contacted later, Ms. Cleveland stated she'd changed her mind and would not be taking the child in. When asked about his suicidial ideation, Darren would not disclose his reeasoning. The vicimt's basic needs appear to be met but Darren has prior history of violent behaviors. Darren has never acted out or had a violent temper while at school. Mr. Flowers is unsure if a decision will be made in regards to the
MSTT met with Laura and Analyse to examine what she have been doing to increase the home school link. Due to the storm and the heavy snow fall Laura was unable to reach out to the school. MSTT, Laura and Analyse worked on developing a list of different things they would like to discuss with the school. Analyse would like to be placed back in her previous class as discuss before. Laura would like the school to explain to her why the school placed Analyse in a different classroom setting. Laura would alos like the school to provide her with a little more information in regard to Analyse being placed in advance college prep classes next year. The information she has is vague and all she knows is Analyse will have to take a test. MSTT worked examining
The student lives in a single family home with her father, 16-year-old sister and 7-year-old brother. The mother is not in the picture anymore and has not been in the home for about three years. There were major concerns, possible emotional abuse, with the mother when she was in the home. The student is from
Parent Mentor Supervisor conducted the assessment while standing in the livingroom. PM Supervisor discussed the purpose of the visit and described the Parent Mentoring program to Ms. Johnson. PM Supervisor then asked Ms. Johnson for the tasks that she is to complete as per the Divisions recommendations. Ms. Johnson says she needs to get an Identification
Upon arrival I contacted the school social worker, Monica Cervantes, who advised me she was concerned for the safety of 12 y/o Carter Lopez, who had disclosed to her that he was afraid of his father, Manuel Lopez. Cervantes advised me the teacher had called Manuel to inform him Carter had failed an exam. Manuel then requested Carter be sent to the office so he could pick him up. Carter told Cervantes he was afraid because he believed Manuel would be mad at him and hit him or call him names as he’s done in the past. Cervantes further added Carter had reported Marijuana and alcohol use by Manuel. Cervantes provided me with Carter’s information sheet, which was attached to this report. Cervantes also informed me his current and previous teacher
