Interview of Dr Kenneth Smith, 901 Patients First, Washington, MO 63090 (636) 239-7500 On September 8, 2015 I interviewed Dr Smith at his place of business in Washington, MO. Dr Smith gave the following information: Arlene Skornia and her husband Wilbur had been patients under his care for 10-20 years. He had overseen Arlene on several hospitalizations, and surgical recoveries, finding her and her husband to be intelligent, articulate people. He had met her sons but had little direct contact with them. Arlene had a number of complex medical issues and at the time of her admittance to Mercy Hospital in Washington, MO in August 2013 was being seen by several specialists to include her Oncologist, Dr Heuser. Dr Smith says it was Dr Heuser that actually had her admitted to the hospital on August 13, 2013. He learned about it when the hospital contacted his office to tell him one of his patients had been admitted thru the emergency room. …show more content…
They felt she wasn’t resting and he was concerned because she was “pretty out of it” when she did take the medication. He wanted to “limit” the medications, but one of the sons asked if she could take Benadryl to help her sleep. Dr Smith didn’t want to give Benadryl as a sleeping aid so there were a number of discussions regarding her medication levels and the effects it was having. This particular nurse practitioner (NP), Diana Hayes, seems like somebody who has done this job for decades; in reality she obtained her acute care NP (ACNP) degree in 2014, less than 2 years ago and got her advanced practice license in Tennessee the same year. For this paper I have chosen to interview my younger cousin who currently works for the VA Hospital in Las Vegas, Nevada as a Registered Nurse in the Medical Surgical Unit. I will describe the targeted facility and unit she works for, the type of work done within her unit, any interesting, informative, or specific information related to the chosen unit which makes it unique, and a comprehensive description of my cousin, and her responsibilities.
For my interview I contacted a general practitioner at Kaiser Permanente in hopes of not only learning about the traditional treatments and information, but also for data and prevention facts on Alzheimer's to further understand Kaiser’s methods and the impact it tends to have. I wanted to use this interview as a method to gain insight on my developed questions, as well as a basis for my further development.
On Tuesday February 10, 2015 I interviewed Cheryl (Robinson) Henson in regards to her complaint about Dr. Kondro. Mrs. Henson stated she had recently remarried and had changed her last name to “Henson” explaining the change of name since she filed the complaint.
On April 8, 2015 I interviewed Dr Hawk at his current place of business. After introducing myself, telling Hawk the reason for the appointment I gave Hawk the Subpoena for patient records. Hawk said he would be unable to provide the records as he no longer had access to WCMH files. Hawk said if BHA could provide the records he would be happy to answer any questions. I told Hawk I would inquire with my supervisor as to what needed to be done under the current circumstances.
Ben Smith contacted Mr. Laiter on 06/07/17 at approximately 1000. Mr. Laiter expressed concern regarding Dr. Mizuki's accusations that Mr. Lassiter has received pain medication from eight different providers. Mr. Lassiter stated that he had recently received a kidney transplant, and during that process he was seen be several specialist, who each at one time prescribed him medications. Furthermore, Mr. Lassiter did not appreciate being considered a "drug seeker". In addition, Mr. Lassiter expressed his frustration regarding his Primary Care Provider (PCP) that he requested a change from last December. The undersigned explained to Mr. Lassiter that after contacting Dr. Mizuki, Dr. Mizuki agreed suggested the new consults be submitted on Mr.
She had her baby a week ago. Her baby was doing fine until day 5 when she was shaking and not eating. She received a feeding tube, morphine and clonidine. The writer spoke to her about the process of her child taking morphine and clonidine. She stated they will send clonidine home with her child to take.
When the patient first came in I think I did quite well as I welcomed
The pains were particularly bad today and she had none of the pills left. However not to worry as the doctor would call in a prescription for her and the pharmacy will deliver a little later. Even so she could do with taking some of these pills now and getting the persistent aching to go away and get a little relief. Arthritis had crippled her body making it painful to move and old age had muddled her mind and almost destroyed her hearing.
