The patient tells me she is "doing terrible." She has been under a tremendous amount of stress. She spent approximately 10 to 15 minutes describing to me all of the different types of stressors that she is dealing with. She says that yesterday she spent 10 hours in an airport, which was very stressful. She closed on a condo in May, but unfortunately needs to sell it because it is "falling apart" and she listed the items that are going wrong. She is also having issues with her car and the title. She has not been sleeping as well. She has not been able to concentrate, particularly in the afternoon. She says that her other twin daughter, one graduated about a week ago, the other is graduating this week. She is worried about getting her hair cut and colored, her shoes and getting her nails done. She feels very stressed about this. She was diagnosed with Addison's in the past. This was surgically induced after an adrenalectomy in the year 2000. She says because of that, she "can't do stress." She had been followed by an endocrinologist, Dr. Chan in Bedford, New York, who apparently she tried to call repeatedly regarding these issues and he only called her back once and has not tried to call her again. She says that she thinks she is going to "fire him". She is looking into local endocrinologists, but needs to find one that is willing to work with her, as she has had several "quit on me" in the past. She is wanting to find somebody that might be able to work with her. She is interested in my a thoughts on local endocrinologist. She feels that it is time to have some laboratory studies updated. On 8/3/15 worker spoke with Ms. Lisa Welch, SW at the VA, for the purpose of gathering information on Mr. James Kimbrell's current situation. Ms. Welch stated the VA had been involved with Mr. Welch for some time. During their involvement, Ms. Stacy Grey, primary caregiver and rents a trailer from Mr. Kimbrell. She refuses to pay rent but transports him to doctor appointments and grocery shopping. However, she charges him money for transportation. and when he buys groceries, she puts personal belongings into the buggy. Mr. Kimbrell is blind and is not aware of what he purchases until he gets home and she starts taking groceries just purchased out of the home.
On the day of October 21, 2015 the accused, Johnny Joshua Uy was nearing the end of his trial. Mr. Uy's case was presided over by Madam Justice Power and was tried on the Supreme Court Criminal level; Mr. Uy also elected to be tried by judge and jury. Mr. Uy was charged with three serious indictable offences. On 09/15/16, there is a Screening Report stating that Ms Rikki Miller (AKA: Bennett) called the GPPD asking to take her child, Keaton to the hospital because he was having bad behavior and she also asked to have her child removed from her home. DHS took that child and placed in the child’s home.
Psych: The patient states that she is depressed due to “falling apart” and anxious about dying. Denies suicidal thoughts, memory loss and confusion.
A 76 years old woman who was described as a healthy and active for her age Helena Lambert, from Ceston, BC was killed by an adverse interaction between two prescription drugs which was overlooked by health professionals she trusted, according to CBC. Helena’s doctor prescribed allopurinol to treat her gout however, Helena was also on mecartopurine, an immunosuppressant for colitis. After six weeks starting her new medication, she developed a blister on her foot. Her son took her to Creston Valley Hospital, where doctors found out the interaction between the two drugs causing Helena immune system to shut down. Her son said his mom suffered before dying from the infection and respiratory failure.

Walter McMillian filed a petition after spending six years in prison. He had been convicted and facing a death row at Alabama State. On 1986, Ronda Morrison was murdered in Monroe County. Walter McMillian and Ralph Myers were convicted for this murder. Myers pleaded guilty to a lesser offence presented at court and also testified against Walter McMillian. Walter was convicted for capital murder and was sentenced to death. The murder conviction was reversed, after the petition had been filed, on the grounds that the state had excluded exculpatory evidence.
As a wife I can understand wanting to take care of your spouse but in some way I feel ,the patient either didn’t understand that her lack of treatment could led to her death . Maybe she didn’t care and made the sacrifice to care for her husband. The care giver role that Mr. Williams is currently in is very common in the elderly especially with spouses. Addressing his anxiety will facilitate a speedier recovery for Mr., and Mrs. Williams.
Recently, I completed a psychosocial assessment on a patient in the PHRM/ISS program. She was 21 years old, pregnant with her fifth child. When she initially walked in she seemed pleasant. Then, before we could get started she received a phone call. She politely asked could she step out and of course I said sure. Immediately, after she walked back in she seemed anxious and annoyed as evidence by her frowning and checking her phone every minute. Then, she would continue to say yes and not pay attention to the questions that I was asking. After a while, this began to bother me. This is because I would ask her a question and she would not answer or ask me to repeat myself. Then, she made a comment stating that “it does not matter and she was ready to go because she was upset.” At that moment, I knew that I needed to take an assertive stance.
I feel that Dr. P has a case in this situation. I feel he has the ability to sue for compensatory damages. I would argue that Dr. P is a public figure. As you said he is the foremost academic on exposure to violence and the human psyche. Since he is a public figure and his work is important to public interest. Because of this we have to prove malice. In this case we also have to determine how defamation of DR. P is present. We have to answer how his reputation is being harmed, how his standing in the community is being harmed.

31 y/o AA male patient seen today for psychiatric-mental health assessment. He is awake, alert and oriented x4. He is calm, cooperative and follows commands during assessment. The patient reports he is depressed, difficulty sleeping and nightmares at night. The patient explained his depression is as a result of deep thinking from a news he received two days ago from his elder brother that his mother is ill. Stressors identified by the patient include losing his job a week ago before the news about his mother; his wife is 6-months pregnant with their first child, who currently works part-time at her present job; patient relates difficulty paying monthly bills and inability to provide adequately for his family as a man. The patient denies mood swings, suicidal/homicidal thoughts and ideation. Patient reports his spouse is at work at the moment and he does not want to put stress on his wife due to her current condition. Patient denies been hospitalized for depression or psychiatric illness; and denies family history of mental illness. Patient reports he is seeking help because he does not like feeling this way using terms of “helpless and loss of worth from his spouse”. Patient reports he needs help with his depression and nightmares before his current condition get out of hands and ruined his marriage.
Feels tired all the time, running after her very active toddler and also 20 weeks pregnant. She has been diagnosed with anemia and fibroids.
