1. Drug- or disease- related problem to presentation and current drug therapy for the problem are:
- Stopped taking hormone replacement therapy medications, hot flashes, high blood pressure, high cholesterol level, family history of breast cancer and heart disease. Current drug therapy for hot flashes according to the table 18.1 is Premphase, which is a conjugated estrogen medication. Ask the patient to get back to taking hormone replacement therapy medication if it was found there were no other alternatives. For treatment of high blood pressure and heart disease, there is hydrochlorothiazide and metoprolol tartrate. Lipitor is use to treat high cholesterol level.

Other alternatives are diuretics, beta-blocker, and calcium channel inhibitors. Eating healthy diets, go on aerobic exercises and herbs such as garlic, hawthorn, and French lavender. Garlic can increase the risk of bleeding. Hawthorn should not be use in high dose, which lead to toxicity.
-High cholesterol level is currently being treated by Lipitor. Other alternatives are to go on exercises, use artichoke leaf extract, red yeast rice and guggulipid (gum resin of mukul myrrh tree). Artichoke should not be taken if you have bile condition. Red yeast rice should not be use if you have liver damage. Guggulipid should not be use if you experience allergic reaction and not exceeding over 6000mg/ a day.
-Heart disease is currently being treated by metoprolol tartrate. Other alternatives are to stop smoking if you are a smoker, change to a healthier life style with no added sugar in your diet, to do surgeries, or to take herbs like coenzyme Q10, omega-3-fatty acid, and green tea. Coenzyme q10 should not be taken if you have respiratory problem. Omega-3-fatty acid should not be taken if you have bleeding problem. 8. Monitors that can be use on this patient around the pharmacy is the heart rate, and high blood pressure monitor machine. Pharmacist should recommend the blood pressure monitor, continuous glucose monitor to the patient to help check the signs regularly.
