On 04/22/2018, at approximately 1900 hours, while in Durango Jail located at 3225 W Gibson Ln Phoenix, AZ 85009, I was assisting medical with medication distribution in Durango 5 A-pod, I noticed Inmate Staggs, Joshua T450226 come out of D5 A-pod and told me he needed to go to medical for his jaw. I asked him what happened and Inmate Staggs told me that he fell in the shower. I then escorted Inmate Staggs to the Durango Medical Clinic. While on the way to medical I asked Inmate Staggs what time he fell in shower? He told me around the time the evening meal was served. I asked Inmate Staggs if the reason he is going to medical was due to a crime committed against him while in MCSO custody? Inmate Staggs said "No." After that I dropped him off at medical. In the video recording at approximately 1612 hours, Inmate Staggs walks into the bathroom with an unidentified inmate, then they go out of camera view. Inmate Staggs stumbles back into camera view holding his face. Both inmates then walk out of the bathroom together and shake hands. On 06/06/17, offender Madrid informed CCM Thimmesch that he wanted to be released from segregation. Offender Madrid stated on 05/20/17 he requested protective custody because he was having problems with another offender. Offender Madrid reported the offender he was having issues with was locked up. Offender Madrid was asked if he knew the name of the offender he was having issues with, which he reported that he only knew the offender nickname “Texas”. 1) Police across Ontario, Canada are looking for a David Maracle, a serial offender who escaped from a correctional facility in Kingston, Ontario on January 22, 2017. David sexually assaulted a 14-year-old girl at gunpoint in 1997, served time and was released in 2015 to 24 hour supervised correctional center. He was only allowed out if staff received a detailed itinerary of a planned trip. On that Sunday, around 8 pm, staff realized David was missing and alerted the authorities (Edminston, 2017).
I placed my firearm, OC, and knife in the lock box at the jail. I gathered the paperwork I needed from the vehicle then Sgt. Garwood and I escorted Mckenna into the jail. Support Coordinator went to Vibra Hospital to meet with Mr. Sampson. Upon entering his room, he was asleep but woke up shortly after. He was able to communicate once he suctioned his throat. When he spoke it was low tone and slow but his mother reported on Monday he wasn't able to speak at all. He seemed to be in good spirits as he joked with the SC. He said today I was his first visitor. I explained it was still early as it was morning. During the visit after many disappearing pens, he signed a release. Daniel Burch is his Case Manager with the hospital #678-7718. I was not able to give the release to anyone as there was a meeting in which most of the staff was in attendance.
