This scenario can be applied to all types of services regardless of their size. As the scenario states, many services chose to handle when they are reported often with knee jerk type reactions. It is for this reason that many employees are scared to report mistakes because of the threat of termination for the individual involved. These acts then create the other victims in addition to the employee being punished. Now the first issue that I would address in my new position as the EMS risk manager would be to improve the communication skills of all employees from the highest position down to the newest employee. This would include the importance of reporting mistakes, near misses and having open communications with employees
The qualitative risk analysis performed in a previous report identified eight notable risks associated with setting up a call-center presence in Québec, Canada. An incident reporting was perceived as having a positive effect on safety, not only by leading to changes in care processes, but also by changing staff attitudes and knowledge and working environment. In response, our organization developed many forum categories for specific incident type; however SRS suffer an unknown degree of underreporting, given that reporting is voluntary and spontaneous, and a systematic surveillance system is not feasible. Yet some staff those using the reporting system for accountability; wrongly interpret SRS data as valid rates of errors or as a valid measure of patient safety.
At work, there are policies and procedures in place, if any incident occurs and there are also self-evaluation forms and reflective supervisions with the manager every three months in order to be able to reflect on ourselves in practice.
