On 04/29/2016 at 0832 hours, I Deputy R. Dean Hildebrand responded to Elkhorn Heath and Rehab, 474 Hwy 282 Clancy, Montana 59634 on a complaint of elderly neglect. Upon arrival I spoke to the complainant Guy Thomas Hayter he told me the following; He had been told by staff member Michelle Anderson believed that resident Beverly Elizabeth Brown had been abused by her son (John Robert Brown) and his wife (Karen Sue Brown). Hayter went on to explain the victim had been taken from the rehab to live with John and Karen three weeks prior. Michelle Anderson stated on 04/25/2016 at 1310 hours John and Karen had returned to the rehab to readmit Beverly Brown because they could no longer attended to Beverly Browns care and needs. Please see written report completed by Anderson. Anderson said while she was assessing Brown she complained about her hip. Anderson stated Brown received an x-ray of her hip and the results showed a fracture. I requested to speak to Beverly Brown about her stay at her son’s home. When Brown came into the room, I saw she was in a wheel chair. It appeared she had suffered a stroke at some time and she was unable to use her left arm. I was informed Brown had also lost 15lbs in three weeks in her son’s care. Anderson stated Brown was very guarded and had a deer in the head- light look when she was readmitted to the facility. Anderson stated that Brown has opened up and seemed to be doing better. I was informed by Hayter Browns medical records were being transferred from Nevada and a comparison could be made of the current injury and determine if the hip fracture could be related to the previous hip injury.
I made contact with the department DPHHS Adult Protective Services and was told they were informed about the incident and they were in the process of following up on the complaint. A copy of this report will be made available for Adult Protective Services and the Jefferson County Attorney’s Office. I took no further action in this
Jane was involved in a single-vehicle accident that resulted in multiple areas of injury from a head trauma, two broken ribs as well as bilateral fractures in her leg’s as well as right arm and wrist fracture. Jane was, transported a hospital that was an hour and half away from her home.
The reporting party (RP) stated referral #1143-3655-1102-7078895 was generated from the following referral #0534-9062-5752-307279 dated 1/28/15 regarding resident Nicole Morris age 14 DOB: 6/1/00. Per the referral Nicole is an alleged victim of sexual abuse, physical abuse and general neglect by an unknown perpetrator. According to the RP on 1/28/15 a staff member named Rebecca took Nicole to Long Beach Memorial Hospital clinic to be examined after she disclosed having suicidal ideation. Nicole revealed to hospital staff that she planned to hang herself. While waiting at the hospital Nicole disclosed being "fingered" by two different boys (names unknown) at the home. Nicole also disclosed that a girl (name not provided) beat her up. Referral 0290-1132-4581-1002176 dated 3/13/15 reported by Tatiana Garcia, Clinician with Bayfront Youth Group home.
Initially we had difficulty obtaining his discharge orders as the hospital care manager replied she had forwarded the documentation, but neither I nor Mrs. Bianco had received a copy. Mrs. Bianco made a visit to Mills Creek and met with Mr. Smith at that time she was made aware of an appointment that Mr. Smith attended. Mr. Smith attended a physician appointment on June 28, 2017 with Dr. Leonard, Orthopedic trauma. Mills Creek assisted with the transportation and a mobile x-ray unit was coordinated to obtain x-rays of the femur at the facility as Mr. Smith didn’t bring his slide board to the appointment and couldn’t get onto the radiology table.
This case involves Ms. Brianna Mona Haggins a fourteen year old Minnie Howard School student. Briana left home January 6, 2016 and was returned on the same day by Officer Hudson. On January 11, 2016, a call of service for a domestic assault was reported for 6300 Stevenson Ave Apt# 611. In this incident (16-102714) Brianna and her mother (Sherry Haggins) were determined mutual combatants.
RP stated law enforcement received a call from an anonymous caller stated an employee, Shemaiah Messengale grabbed the child Marley's arm which is fractured. Law enforcement contacted the child's mother to verify injuries. It was disclosed the child had suffered 4 fractures to her elbow and wrist. The RP stated law enforcement spoke with owner/licensee of the facility and the employee accused of causing the injury. They reported Marley sustained the injuries accidentally when she fell from slide. The RP stated the child was treated at Children's Hospital Los Angeles (CHLA) by Dr. Lindsey Andras who determined the injuries are consistent with a fall. May 18th 2014 Mr. Beaird fell and broke his hip. This followed hip surgery performed by Dr. Baker and 21 day rehabilitation form Cordova NH. Before his 21 days was up Mr. Beaird tried to break out of NH by running his scooter though the front door at the NH. It seems as if medical neglect happens too often in pediatrics. Medical neglect is when a parent fails to adequately care for their child, when those cares are pertinent to treating an illness or physical injury. In the two years that I have worked in pediatrics, I have seen many of these cases. The majority of the time, it occurs with our chronic patients who are medically heavy, time consuming, hospitalized a lot, and the parents do not adequately take care of them. In this paper, I am going to discuss a case of medical neglect that I have encountered in a child who had received a heart transplant.
Investigator Stockwell stated in September, 2015 an allegation of sexual abuse was reported by the victim. Investigator Stockwell stated reports of sexual abuse were made against the child’s step father. The reporter stated a case was open by law enforcement and DHS at that time and the child was placed in her grandmother’s care; Mr. Adams was instructed that he should not have contact with the victim. On this date and time this caseworker met with Latonya Myles for a follow up visit. At this time this caseworker observed Jacoreyn to be clean. healthy and adequately dressed. The marking cause by the fall on Jacoreyn forehead had fully healed and his skin has return to its normal color. The caseworker observed Jacoreyn walking and playing with his tablet along with his brother Jaylin. Ms. Myles reported that Jacoreyn head has heal well and she has not had any problems. Ms. Myles reported no concerns. Ms. Myles gave the caseworker permission to speak with her son Jaylin.

On Oct 8, 2012, a counsellor from Terry Fox Secondary School brought forward concerns of Sally Brown being abused. Mr. Highcliffe reported that he noticed some bruises on Ms. Brown's upper arm and shoulder. When Mr. Highcliffe asked Ms. Brown about these bruises, she replied with a vague story of being involved in a game with her friends and collided with a wall. To investigate, I visited Ms. Brown at her residence and was informed that her parents were away for an overnight prayer vigil at their church. From initial appearances, Ms. Brown presented as depressed and insisted that I leave. I was able to establish a rapport with Ms. Brown and discussed my concerns about her noticeable bruising. Upon agreement, I took Ms. Brown to the emergency ward of the Prince George Hospital where Dr. Rose, examined her arm and shoulder. From Dr. Rose's medical opinion, the bruising did not appear to be consistent with her story of running into a flat surface such as a wall, rather appeared to have been caused by a blow by a heavy instrument. I confirmed with Dr. Rose that he would be willing to testify in court to his findings about the bruises. 7/20/15, there was a report made the Department of Social services and the allegations were; Reporter states grandmother is watching child and maternal grandmother seems paranoid. Reporter states the family is not using air conditioning and the house is hot with a child. The maternal grandmother covers vents in house and she feels someone is listening. Maternal grandmother feels there are cameras in the fans. Reporter states the maternal grandmother was heard saying she will whoop the child’s ass about 2 months, unknown why she stated that. As requested, I have reviewed the facts of the above-captioned file, along with the applicable law and summarized same in this memorandum. Mrs. Mary Smith suffered an injury to her right ankle in an automobile accident on 10/3/95. After surgery and months of rehabilitation, Mrs. Smith still suffers daily. I have researched the facts regarding a personal injury action against Paul Joseph, as well as a medical malpractice action against the medical providers.
Reporting party (RP) stated that her mother, Betty Joe Hippik (DOB: 06/04/29) has been residing at this facility for almost 2 years (July). RP stated that she visited the resident on 3/3/17 and it was discovered that the resident had 2 raised spots her back and that resident was itchy. RP stated that she saw the resident again the following day and the resident had more spots present. On 3/8/17, the administrator (Lori) contacted RP and informed her that the resident was seen by the facility doctor, Dr. Henry who suggested that RP purchase an over the counter cream for the resident.
