DOI: 4/18/2002. Patient is a 56-year-old male welder who sustained cumulative trauma to multiple body parts from lifting/carrying. Per progress report dated 10/31/16, the patient will stay on Vesicare 10 mg daily for his bladder problems and Cialis 20 mg as needed for his erectile dysfunction. The provider notes that he will add Myrbetriq 25 mg daily in view of the increase in his lower urinary tract irritative symptoms. Per the prescription script dated 12/12/16, IW was diagnosed with Cialis 5 mg 1 tablet daily with 2 refills and Cialis 20 mg one tablet prior to intended sexual activity as needed, as booster in addition to daily Cialis 5 mg with 2 refills. Based on the medical report dated 12/19/16, the patient has required spinal surgery through an anterior and posterior approach in 2013. He has developed urological problems resulting from his work related injuries for which he is seen periodically in this office. He comes back for a urological follow-up visit. He states that his back pain is getting worse. He has not been able to refill Vesicare and as a result, he has been experiencing increasing frequency and urgency. He has been experiencing increasing frequency and urgency. Cialis 20 mg as needed is of benefit for his erectile dysfunction. Since Vesicare is not being approved, recommendation was made to start on a trial course of Cialis 5 mg daily, as daily low dose Cialis has proven to be beneficial in patients with both voiding and erectile dysfunction. He was given samples of Cialis 5 mg to start the therapy trial. He will continue using Cialis 20 mg as needed as a booster dose. The patient is encouraged to be active physically and to exercise regularly. He will be reevaluated in 6-8
