On May 28th, 2016, I, FAM Travis Ringer, and FAM Joseph Drossos were assigned on mission status to DL 2330, from Detroit Metropolitan Airport (DTW) to San Francisco International Airport (SFO). At approximately 1615 EST, while flight DL 2330 was taxiing from gate A66, flight attendants called for medical assistance. The Captain announced the aircraft (767-300) was returning to the gate to meet medical personal. EMTs and Detroit Metropolitan Airport Police boarded and moved toward the rear of the aircraft, where PAX John ANTON was seated. Shortly thereafter FAM Drossos and I heard loud screaming and calls for police assistance. We exited our seats and began moving toward the area of the EMTs. On 1/22/2015 SO EMT Perez was dispatched to FC-117 via phone call from a resident regaurding a fall . SO EMT Perez kocked and announced his presence at the apartment door. SO EMT Perez was the verbally greeted into the apartment. SO EMT Perez the unsecured the apartment door and proceeded to search the apartment. SO EMT Perez eventually found the resident, a Mr.David Furman on his bedroom floor by his bedside. Mr. David Furman slid out of his bed and was unable to get up on his own strength. SO EMT Perez asked Mr. Furman if he was in any pain, which he denied. Mr. Furman also did not present with any pain while SO EMT Perez preformed the pelvic exam. SO EMT Perez then helped Mr. Furman to his feet and witnessed him standing on his own strength
On March 39, 2017, at 1224hrs, VA police received a Duress Alarm for 1E127. Investigation revealed that the Veteran was upset over not being able to receive her Ulfima Five Tens Unit. The MSA Clerk informed her that she would have to attend a class on how to properly us the Tens Unit. The Veteran demanded she give her the Tens Unit, because she had use it in the past. When the MSA Clerk refused and told her she would schedule her an appointment, the Veteran got angry and called the Clerk a witch and walked off. VA Police made contact with the Veteran at Primary Care Desk B lobby. Where the veteran gave an omission and stated she had an appointment and because the clerk was not at her desk she missed it. On Thursday 12/24/2015 at approximately 2307 hours. Security Officer Omar Alonso (420) was contacted by E.D. Charge Nurse Sharey Selover about an uncooperative intoxicated male patient, Jose D. Gonzalez (DOB: 03/30/1977; FIN# 85006354), come in through the EMS Offload area. Officers Alonso and Ayuso reported to the call and observed an intoxicated male being wheeled into the Special Care Unit (SCU) E.D. room # 39. According to his assigned Nurse Sara Lopez, the patient had been involved in a physical altercation and had been kicked hard in the groin area. Patient did not behave badly or disruptive once he saw that Security were present and his Nurse was able to get his vitals, blood work, and urine without having any issues. Security staff
On 10/21/2015 SO EMT Perez was dispatched to HS-318 regaurding a general assistance call. SO EMT Perez knocked and announced his presnece at the door and was verbally greeted in by the resident, a Mrs. Netha Hopson. Mrs. Netha Hopson stated that she needed help retrieving her purse because her motorized chair was caught on it and she could not get it untangled on her own. SO EMT Perez managed to untangle the purse and retrieve it for Mrs. Netha Hopson. After insuring that Mrs. Netha Hopson needed nothing else SO EMT Perez cleared the scene without further incident.
On May 15, 2016 at approximately 2335 hours Security Officers Steven Evans and Omar Alonso were in the E.D. Security Office when we heard someone yell for help inside of the E.D. entrance doors in regards of a “Gunshot Wound” patient. Out of the Office, we observed a Hispanic male hoping towards the PFS counter with blood on his left abdominal area. Security Supvr. Evans grabbed a wheelchair and wheeled him to one of the E.D. triage room, while his friend, Joseph Park (witness) register him with PFS. Security team in coordination with E.D. Charge Nurse Robbie Phillips placed the Emergency Department in a lockdown, Security Officers responded and kept watch over both E.D. entrances. The patient, Daniel Lopez (DOB: 10/16/1997, FIN #85640879) was transferred to E.D. #35, he had 4 “GSW”, 2 on the right and left buttock, 1 on the right
On Thursday, 05/18/2017, at about 9:27 p.m., partner Officer Griffin (#3135) and I were on uniformed patrol in a marked patrol vehicle (Unit #21) as 3A21. We were dispatched to an injury traffic collision at Lake Ave. and Maple St. Upon our arrival, we saw three vehicles that appeared to be involved in a traffic collision. While we were assisting with the scene, we saw Victim Captain Konrad struggling with the suspect, later identified as Kevin Hendrix. Officers Griffin, Riehl (#7454), Spatz (#8058) and I grabbed Suspect Hendrix and took him to the ground. I placed properly adjusted, double locked handcuffs on Hendrix. Officer Griffin and I placed Hendrix in our patrol vehicle.
On 5/29/17, at approximately 1804 hours, I was calling the last inmate for medication distribution in D-4, A pod. Myself and Nurse Bob were stationed at the entrance of A pod. Inmates began to crowd around the entrance of the bathroom. An unidentified inmate started yelling, "There is a guy on the floor in the bathroom having a seizure." I ordered the inmates to move out of the doorway of the bathroom. I had a partial view of the bathroom from the pod doorway and was able to partially see an inmate lying on the bathroom floor. I directed all inmates to lockdown in A pod and made a radio call to Security Control indicating there was a man down in D-4, A pod.
On Saturday, September 12, 2015, I was assigned to the 6 Northwest as the security officer. At approximately 0500 hours while breakfast trays were being distributed in pod 100, an inmate, later identified as Kimmie Hawkins became insolent and advised me to get out of the pod. I immediately asked him whom he was talking to and simultaneously walked over to him to view his armband. As I reached for Inmate Hawkins’s left wrist, he snatched away from me, got in a boxing stance, and began bouncing rhythmically on his toes. Therefore, I retrieved my T.A.S.E.R. from its holster and gave Inmate Hawkins loud verbal commands to get on the floor. However, he refused to follow my orders; therefore, I pushed him toward the wall and continued to instruct
On March 23, 2017 at 1402hrs, Doctor from Mental Health notified Police Service of a Health Welfare. At 1410hrs, VA police notified the Austin Police Department (APD) and spoke with a dispatcher – 7974 and requested a Health and welfare at 2407 S. 4th street (Green Doors Halfway House) for a possible suicidal Veteran. APD Dispatcher stated he had officer’s en-route to the above address and provided me the incident #170821057.
A friend saw the post and alerted the Fort Hood Military Police, who called 49th BN Staff Duty. The HHD Chain of Command was informed at 0400 and made multiple calls to SGT Coble’s cell phone, which there was no response. The Chain of Command then proceeded to SGT Coble’s house at 6303 Tempora Loop, Killeen TX. His truck was parked in the street and the garage light on. Several more calls were made to SGT Coble’s cell phone, which was heard ringing inside the house, however he did not answer. The Chain of Command
On December 01, 2015, I, FAM Christopher Catone, was assigned a FAM mission on Delta Airlines flight number 0132 (DL #0132) operating from Detroit Metropolitan Airport to Amsterdam Airport Schiphol (DTW-AMS). Also assigned to the flight were FAM Wayne Solgot, FAM Damion Callahan-Howard and FAM Scott Howard. At approximately 20:32, while flight 0132 was taxiing from gate A28 of Detroit Wayne County Metropolitan Airport (DTW), I was approached by Fight Attendant (FA) Anita CLARKE (ID#0545538). During the boarding process, FA CLARKE indicated she observed a passenger, later identified as David SPIRIDON, W/M (DOB: 3/25/1983), attempting to conceal an object in his jacket and she also saw what appeared as wires protruding from SPIRIDON’s jacket. FA CLARKE contacted the Pilot in Command, Captain Burt D. THORP, and advised Captain THORP of the SPIRIDON’s actions. Captain THORP, through FA CLARKE, requested the FAM team speak to SPIRIDON concerning his actions. FAM Howard went to the passenger, identified himself as a law enforcement officer, and asked if SPIRIDON would agree to speak in the rear galley. SPIRIDON agreed and walked with FAM Howard to the rear galley. I then met SPIRIDON and FAM Howard in the rear galley. When asked, SPIRIDON indicated he was unaware his actions caused concern and was not trying to hide anything. I asked SPIRIDON if I could look in his property and he agreed to a search of his person and property. During the search SPIRIDON assisted by removing property from his pockets to show me and FAM Howard. FAM Howard conducted a search of SPIRIDON’s person with negative results. At approximately 1700, Michael Pentoney and I were stationed in vehicle 5 on top of Power Cove. As we look down to the water we notice a group of people running down from the picnic area down to the water. We then see a female running towards our vehicle. She's screams that her niece was missing in the water. I asked her, "if had seen her go under water and not come." She yells, "yes". I told her to get in the vehicle and we started driving down to water. As we are driving down to the water, I call on our Blue Frequency, "emergency traffic, emergency traffic only. SURCOM 22P992, we have a code X in power cove, we need rangers and lifeguards." Then I got on our Bronze Frequency, called for Andy Mendoza who was on vessel 94 to pick Kyle Christian
On Friday morning, July 11, 2015, at about 12:15 AM, I was driving a marked squad and driving on the Wiouwash Trail. The campus area of the trail has a posted closing hour of 11 PM. I saw 2 males near the boat landing southwest of the Student Recreation and Wellness Center (SRWC) and start to walk away when I shined the squad spot light on them. I then saw 4 other individuals walking away from my position. I radioed to dispatch that I would be out with the group of 6 individuals and exited my squad.
Upon arrival, I noticed an older gentleman sitting in Desk B lobby area. The man was later identified as Veteran Thomas Keller. I contact RN XXXXXXX, who stated that Keller was intoxicated and causing issues. I then contacted Keller who, stated he was here to get a cain. I noticed an odor of alcohol emitting from his person. I then asked OFC Massey to get the PBT so I could check his level of Intoxication. After Massey left, Keller attempted to leave, I informed him that he was been detained. He replied “I’m fucking leaving, you can’t hold me”. Then He asked “what are you arresting me for?” I replied that I could smell and odor of and alcoholic beverage coming from him. Keller then stated “Hell yes I’ve been drinking it’s still a free country”.
As I was checking credential in the smoke area on the SIDA, Wes Mahr (TSO) informed me that he was escorting a trainee as I noticed that the trainee was issued a sterile area badge and Wes had on a SIDA area badge with escort privileges.
