On Thursday 11/12/2015 at 1905 hours, during shift briefing, Officers Jason Peterman and William Miller pass down information concerning what was believed to have been an elopement from CDU by a male Baker Act patient. He was reported missing by Registered Nurse Ophie to Doctor Gomez at approximately 1530 hours. Doctor Gomez who was concerned for the patient’s safety and wellbeing then contacted Primerose Vernet from the Florida Hospital Risk Management Department assigned to our facility and she in turn contacted the Orange County Sheriff's Office and reported the issue.
FACTS: Douglas E. Metzger lived in an apartment in Lincoln, Nebraska. About 7:45 a.m on April 30, 1981 a residence of the apartment complex was parking his vehicle directly in front of Metzger’s apartment window. Metzger was standing naked with his arms at his side for five seconds. At 8:00 a.m two officers had arrived to the apartment, they observed Metzger standing naked in front of the window eating cereal. The officers said that Metzger was nude from mid-thigh on up and he was standing a foot of the window. Douglas Metzger had violated the Lincoln Municipal Code which stated, that it was unlawful for a person in the city of Lincoln to commit any indecent exposure in the presence of another person.
Reporting party resident Michale Dean Lopez (DOB: 11/3/58) stated that approximately 2 months ago, that he was lying down in his bed and felt something behind him. Resident stated that he jumped up and turned around and resident Chris (last name unknown) was standing at his bed side with his boxers down and his "thing" sticking out. Resident noticed that his boxer were down half way. Resident does not know for sure if Chris penetrated him but stated that he felt funny and that he takes Ambien along with Trazodone. On June 21, 2016, members of the Metropolitan Police Department (MPD) Sixth District were dispatched to a call for service for an “Assault in Progress”. Officers were met by the complainant who reported that the defendant invited her over to his residence and when she arrived the defendant already had another female companion at his home. The defendant began to insult the complainant at which point she tried to leave his home. The defendant grabbed the complainant by her hair and punched her in the face and body. The complainant refused medical treatment.
The reporting party (RP) stated referral #1143-3655-1102-7078895 was generated from the following referral #0534-9062-5752-307279 dated 1/28/15 regarding resident Nicole Morris age 14 DOB: 6/1/00. Per the referral Nicole is an alleged victim of sexual abuse, physical abuse and general neglect by an unknown perpetrator. According to the RP on 1/28/15 a staff member named Rebecca took Nicole to Long Beach Memorial Hospital clinic to be examined after she disclosed having suicidal ideation. Nicole revealed to hospital staff that she planned to hang herself. While waiting at the hospital Nicole disclosed being "fingered" by two different boys (names unknown) at the home. On Friday, 06/08/18, at approximately 0933 hours, I was on duty, dressed in full police uniform and driving a marked patrol vehicle, in Daly City. I was dispatched, along with Officer Chittenden, to Stanford Medical Center in Palo Alto, on a report of a rape that occurred at the Mylo Hotel in Daly City between the dates of 06/03/18 and 06/06/18. The reporting party was identified as CV-1.
Officer Lajoie’s prior law enforcement experience has enabled him show he has a good grasp on the fundamentals of being a solo patrol officer. This has allowed him to focus more time on investigating in-depth cases such as a sexual assault on a child case (Case #17007910) and a sex registrant in possession of multiple firearms (#17008132). During both cases Officer Lajoie investigated them from start to finish, working with detectives to create solid cases.
On 12-09-16 at about hours 0748 hours, Officer Petrella and I responded to the above location, regarding a PC 273.5(a)-Domestic Battery investigation. The victim and Suspect Goldbaum are currently living together and have been in a dating relationship for the past two years with a common son. The victim stated, on 12-09-16 at approximately 0740 hours, she was involved in a verbal argument with Goldbaum. While inside of the their bedroom, Goldbaum forcefully pushed the victim onto their bed. Goldbaum then used his right fist and punched the victim twice in her stomach. Goldbaum then placed a cover and pillow over the the victim's face for approximately two minutes. The victim had a hard time breathing with the cover and pillow over her face.
Upon arrival, I made contact with Ms. Sara Adame (Complainant), who stated she believes she was drugged and raped earlier tonight (May 16, 2017). She advised at approximately 1500 hours, she arrived at Uno Chicago Grill in reference a job interview. At approximately 1600 hours, she met the bartender and had two double shot margaritas. At approximately 1630 hours, she left the restaurant with the bartender. She does not remember anything after leaving Uno Chicago Grill. Due to an app, Snapchat, she believes she was at the Pineda Inn Bar at approximately 1850 hours. At approximately 2247 hours, she woke up at the bartender’s (she believes it was his residence) trailer. The both of them were nude and felt as she just finished having sexual intercourse.

I knocked on the door and there was no answer. I opened the door and went inside of the apartment. While clearing the apartment I saw a male laying down in the floor of the room. There was blood on the bed, floor, and wall near the male. I cleared the rest of the apartment and called Lt. Roof #1525 and advised him of the situation.
At 1601 hours I was dispatched to 12625 Wetmore, SATX, Suite 436, Room 1163 to meet with Mr. Jose Robledo concerning the positioning of 4 mannequins in an apartment room in a sexual manner. I arrived on scene at 1604 hours along with several other officers. On October 25, 2017 at approximately 7:52 p.m., Desoto Police Officer Eric Howard located a vehicle parked behind the Gallery Night Club (16310 N. U.S. Highway 51, Desoto, Illinois). Officer Howard noticed there were people in the back seat of the vehicle and therefore checked on the status of the people. It should be noted that Gallery Night Club has been closed and vacant for several years. As Officer Howard approached the vehicle he noted the windows were fogged. Officer Howard made contact with two individuals and noted they were undressed in the back seat of the vehicle. A white male, later identified as Carl J. Campanella (M/W, DOB: 07/09/1952), got out of the vehicle and told Officer Howard they were parked there to have sex. Officer Howard identified the other passenger as Darius A.
