Hello Dr. Ullom, majority of the long term care facilities are under staffed. There is usually one registered nurse in charge to manage a 240 bed facility, with LPN's and nursing assistance. I feel that these patients would benefit from having one RN to every six -eight patients with a nursing assistance. Not only would this benefit the patient but the nurse as well. Patient are placed in long term care facility with a certain problem, but ends up with additional condition such as UTI, MRSA, pressure ulcers, and etc. These issues are related to poor care they receive because of unstaffing. I'm not placing blame on the LPN or nursing assistance, but with a RN and low nurse to patient ratio, they will receive better care.
The past several decades, has shown and increased inquired interest on nursing home facilities, and making them more than just a facility, and more like a home for the elderly population. There have been a significant number of facilities that have actually strived to maintain an individual residing in nursing home facilities sense of autonomy; dignity and right over their lives in order have control over their own well-being. The Social Security Act, Sections 1819 and 1919 require facilities to be in compliance with federal requirements of quality of care and quality of life. CMS emphasizes on Person-Centered Care, Quantity of Staff, Individualized Approaches to care and Engagement of Resident and /or Representative in decision-making. These regulations are based upon values of respect and freedom for older persons in various ways to include an individual 's right to take risks or to make their own choices.

Congress. Codified as section 1819 and 1919 of the Social Security Act, the landmark legislation declared that every nursing home must protect and promote the rights of each resident, including, “The right to be free from … any physical or chemical restraints imposed for purposes of discipline or convenience and not required to treat the resident’s medical symptoms.” When the Nursing Home Reform Act was adopted it was widely accepted to use restraints as an acceptable and widely-used option to manage the behavior of residents who wandered; were agitated; or who in the view of treatment staff, simply needed to be restrained. However, the standard use of restraints was not supported by a body of research that found that physical restraints had

There were reports of rampant neglect as well as abuse of the people in the nursing homes across the USA, something that prompted the Congress to enact legislation to deal with this in 1987. The legislation was known as the Nursing Homes Reform Act which stated that each nursing home "must provide services and activities to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident in accordance with a written plan of care."(Motley Rice, 2013).

The following are issues that need to be addressed when advising Amy about Sleepy Hollow Nursing Home’s rights and liabilities.
We all know that nursing home is extremely expensive and not everyone can afford it. I personally think that there is not enough Medicaid to help people who need it the most. Many people want to go a nursing home, but they do not have Medicaid to pay for it. Medicaid becomes important for some families when their parents cannot no longer take care of themselves in their own home. Some people do find a way to get Medicaid to cover them but it is not going to do a lot. Nursing home facilities are doing business and they also need to pay their employees I guess that is why Medicaid is really expensive. I do not know anyone who uses Medicaid spend down, but when my parents get older I will observe them to see if they will use Medicaid spend
MRSA carriage and the associated increased morbidity has long been identified as a risk for nursing home (NH) residents, who represent a unique, vulnerable, and growing population (Gibson et al., 2014, p. 1458). NH residents typically exhibit multiple comorbidities and greater functional dependence, requiring more hands-on contact care from healthcare workers, and they are more exposed to health care workers interaction than hospitalized patients due to the fact they stay longer in NHs compared to patients in hospitals (Gibson et al., 2014, p. 1458). The goal of this study was to identify determinants of new MRSA acquisitions (persistent or transient carriage) among high-risk NH residents (Gibson et al., 2014, p. 1459).
The history of what is known as “nursing homes” in America is actually a relatively short one. Facilities set aside solely for the purpose of providing care for the elderly and infirm were nonexistent. Most elderly were cared for by their family, and if they had none, their only other choice was the almshouse. Almshouses were harsh institutions where the elderly lived alongside the insane, the inebriated, and the homeless. At the beginning of the nineteenth century, women’s and church groups began establishing homes specifically for the elderly. However, these homes often required considerable entrance fees and certificates of good character, making it difficult for the average elder to find placement in one (Nursing Home History, n.d.). Even
Pneumonia is a significant factor that leads to morbidity as well as mortality in nursing home residents. It has a one-month mortality rate that range from 15-35%. Nursing home residents are frequently referred to hospitals for diagnostic tests. They may as well be provided with acute health care services. In our qualitative study, we shall examine residents and family members’ perspective on pneumonia prevention. The study design that we shall use is qualitative descriptive in nature. The subjects will be residents and relatives who have received pneumonia treatment. We shall derive them from a randomized controlled trial. In total, we shall conduct 15 in-depth interviews. We shall then analyze the collected data editing style. The style was described by Crabtree to establish important themes. Both residents and relatives prefer pneumonia treatment to take place in nursing homes where applicable. Caring and attention crucial during provision of care and they can only be attained in a nursing home context. Interventions to control hospitalizations of nursing home residents suffering from pneumonia are congruent to residents and relative preferences.
The place where this case study occurred was in a nearby senior care facility. I was to do the worship service with communion for the residents. The church I pastor is responsible for two senior care facilities and they alternate each week. Because of my schedule I do not do the services on any regular basis. I was off for reading week from school so I could volunteer. There are a few women who go to the facility. One of the women used to come to our church. She told my senior pastor that she did not like my being a pastor so she stopped coming. She also attends the pastors Bible study occasionally.
Just the mere mention of “nursing home” causes most people to shudder and think, “I would never put my relative in such a ghastly place.” There are a great deal of horror stories that stem from nursing homes, most along the lines of , “ My mother was neglected after I put her in a nursing home. I would go to visit her and she would be lying in urine and have a new bruise, different than the one that was there last time.” There seems to be this popular perception that nursing homes are places where aging people go to wait out their final days and die; this causes many individuals to shun them. People are too caught up on these negative stories they
