The department’s intervention plan was to remove the child from the home and place her in a safer environment. My client’s response to this intervention at first was very angry and resentful towards the department. Father especially angry with the department and did not understand why he couldn’t keep his daughter since mother had the drug issue. This transition to electronic formatting has pros and cons associated with it. I will be describing the EHR mandate, including who initiated it, when it was initiated, the goals of the EHR, and how the Affordable Care Act and the Obama administration are tied into it. What the HIPAA law states. Health Insurance Portability and Accountability Act (HIPAA) is a law that was enacted in 1996 establishing safeguards and rules to protect patients demographics and medical records. These rules limit the circumstances of how health records are used or obtained without the patient's authorization. HIPAA has set national standards that require these safeguards to maintain the attainability of health records and keeping them classified. CPSW did a home visit to meet with Ms. Berner and to discuss about her safety plan since the children are moving with her on10/29/16 from the foster provider. Ms. Berner was late for her appointment and CPSW waited a 40 minutes for Ms. Berner. Ms. Berner apologized for being late. CPSW explained Ms. About safety plan. Ms. Berner understood and she signed them. Ms. Berner stated that she will be doing a house arrest for two weeks and the recommendation of her criminal court is to obey law and continue taking her medication on time and seeing her therapist. The HIPAA law dictates how our organization not only handles patient privacy and record keeping, but also how we handle patients’ request to view medical records for a variety of reasons. A patient in our organization requested to attain complete access to his records that include psychiatric records as well. The regulation I choose to discuss is the Health Insurance and Accountability Act also known as HIPAA. Throughout the course of 6/22/15- 6/27/ Igoe REACH has come into the home of Jessica to observe her behavior. The behaviors, which Jessica engages in, place her in harm of herself and her peers. Monday she engaged in elopement, which then led to a sit down strike in the middle of the road. The incident required two staff to physically escort her in the home while trying to deescalate the behavior, which included screaming and kicking staff, was punched in the face another staff was also forced in a ditch that was filled with mud. All these incidents happened while in the presence of her support coordinator for Prince William County. Collin another in home counselor came to observe Jessica. I made contact with him on his second time with Jessica. Collin was unaware of Jessica normal behavior so staff shared with him that he should, “observe” her getting of the bus. Immediately entering the house she started to engage in behaviors that led staff to verbal redirection. This took place for about 45 minutes until the AD for Community Concepts (Regina Goldston) advised him to back away and observe from a distance. It has been noted that Jessica will engage in more behavior with an increase presence of staff. Collin spoke with the home manager about the normal behaviors. Although sharing of patient information through EHR is highly encouraged, it is crucial that only the people involved in the care of the patient have access to the patient’s data. Moreover, patients should give consent to the providers before sharing their medical information. Stage 1 of meaningful use encourages the protection of the patient’s electronic information. This principle is aligned wih the concept of meaningful use.
During his initial check-in Mr. Williamson discussed his charges, substance use, education/employment history and relevant medical background with DOC case management staff. Mr. Williamson arrives to his weekly check-in appointments on time. He indicated that he has custody of three (3) of his five (5) children and would like to seek and obtain employment to be able to support them.
