Evaluation Evaluation is the final and often the most critical step in evidence based research and practice. Evaluation of evidence based practice follows a pathway beginning with the selection of the area for improvement, synthesizing the research into a process improvement activity and evaluating both the implementation of the process improvement as well and the outcomes of the intervention (Titler, 2008). To measure the results of process change in the management of heart failure patients a retrospective analysis will be conducted comparing the readmission rates of a pilot and control population over a 6-month period. The pilot population will be evaluated with the LACE index readmission risk assessment upon admission and subsequently receive the recommended interventions based on the risk stratification. In comparison, the control group will receive the current process of telephonic contact only. The pilot group will include patients over the age of 18 residing in zip-codes 45402 and 45403, …show more content…
The Utilization Management and Care Coordination teams will be trained in small group sessions using oral presentations supplemented with handouts. The training will include demonstration of how to complete the LACE index and apply the risk stratification through the use of a patient case file review. The case file demonstration assists in establishing inter-rater reliability needs for consistent application of medical management decision making (McQuillan, 2001). Upon the completion of training the teams will be asked to complete a short training survey to assist in the measurement of change in behaviors associated with the training (APPENDIX G). The survey will ask the team to rate the effectiveness of the training based on a five point Likert scale rating from strongly agree to strongly disagree. The questions will
History: Martha Wilmington, a 74-year-old woman with a history of rheumatic fever while in her twenties, presented to her physician with complaints of increasing shortness of breath ("dyspnea") upon exertion. She also noted that the typical swelling she's had in her ankles for years has started to get worse over the past two months, making it especially difficult to get her shoes on toward the end of the day. In the past week, she's had a decreased appetite, some nausea and vomiting, and tenderness in the right upper quadrant of the abdomen.
ECG: sinus tachycardia with waveform abnormalities consistent with LVH, Pronounced Q waves consistent with pulmonary edema.
There is no cure or recovery from congestive heart failure, but there are ways to improve your hears condition.
Abnormal heart rhythms, infections in the valves of the heart, and congenital heart disease are other heart problems that can lead to congestive heart failure.
Heart failure HF is a condition involving the heart muscle that is unable to pump enough blood through the body to meet its oxygen needs. HF is associated with conditions such coronary artery disease, myocardial infarction, anemia, infection, thyrotoxicosis, hypothyroidism, dysrhythmias, bacterial endocarditis, pulmonary diseases, Paget’s disease, nutritional deficiencies and hypervolemia. HF main characteristic is ventricular dysfunction, usually left-sided heart failure develops first causing intolerance to exercise, reduction of quality of life and short life expectancy (Lewis, 2014).
Heart failure is a long-term (chronic) condition in which the heart cannot pump enough blood through the body. When this happens, parts of the body do not get the blood and oxygen needed for them to function properly.
According to NYHA, the classification places the individual into different categories based on limitations during physical activity (Classes of Heat Failure, 2015). Due to Ms. Griffin's physical activity description, I would classify her functional capacity assessment as a class four. I chose this class because she has symptoms even at rest. Ms. Griffin would also classify as a class D objective assessment. Also, due to her limitations and symptoms at rest.
In order to obtain relevant information to support this topic, databases including PubMed, Medline, UpToDate, and Google Scholar were used. Terms used in each search included and combined the following: mesenchymal stem cell, cardiac stem cells, HF, heart repair, ejection fraction, cardiac output, cardiac index, mortality.
A 57-year old male came to my clinic with the following symptoms, gradual onset of dyspnea, frequent dyspepsia with nausea, epigastric pain and breathing difficulty, especially while lying on his back. His vital signs are B/P 180/110, pulse 88, temp. 98.0 and his respiration 20.
Intervention plans can impact health care in many ways, but successful plans may provide a benefit to either patients, facilities, or both. Evaluation of an intervention plan is necessary to determine its effectiveness and the value of the intervention’s impact in healthcare. In congestive heart failure (CHF) patients engaged through a telehealth program, the effectiveness of the plan revolves around the intervention components. Identifying outcomes and tools to measure the data obtained through the intervention will assist advanced practice nurses (APN) in identifying which aspects of the intervention provide the most benefit. The purpose of this paper is to describe methods to evaluate the effectiveness, identify short-term, intermediate,
Strategies to educate in reference to the self-care needs of the congestive heart failure patient seems to be common theme in multiple literature reviews. The educational materials may be presented to the patient by the bedside nurse, physician, pharmacy technician, and the nurse or nurse practitioner who is involved with the patient post discharge. It is suggested by one article that the effective impatient measures should include medication review, one hour of heart failure education, adequate discharge planning and communication between the patient, and health care team (McClintock et al., 2014). Once discharge the article suggests that the effective measures include early follow-up, continued education and health management by clinics,
Research by the American Heart Association (2014) states that heart failure effects an estimated 5.1 million Americans and it is predicted to increase 25% by 2030. Heart failure is a pathophysiological condition that indicates the heart is unable to promote enough cardiac output causing insufficient blood supply to the body. Pharmacological treatment for cardiac failure is dependent upon the ability to decrease rate of blood flow and blood pressure. Survival after heart failure diagnosis has improved with medication but the death rate remains high with over half of the people diagnosed with heart failure will die within 5 years. (Go et al., 2014)

As population ages, so are their increase in number of people requiring long term care due to chronic health care conditions. Telehealth as one of the healthcare intervention used commonly in today’s healthcare organization has been seen as effective and efficient method of delivering home health in patient with chronic health condition such as; congestive heart failure.
Pedone et al. (2015) conducted a randomized trial study that evaluated the effect of telemonitoring and hospital admissions of elderly patients with heart failure. The study involved a final sample size of 90 participants: 43 from
