On 3/29/2017, this writer met with Mary Eckola alongside with Adriana upon request in the event that the patient assigned counselor is not available. During the discussion, it was based on addressing the overpayment of the patient and her significant other. Also, HCRC continues to send the patients medical record to the insurance company, but no payment. Adriana discussed about payment options, referring to an 8 week payment plan for Mary and her significant other as Mary appeared to the receptive the notion of the payment plan. The writer met with 5 y/o AAM brought to Sinai Grace by EMS due to suicide attempt drinking and took overdose of pills. The consumer also admits to prior suicide attempt three weeks ago. The consumer live with family and has income. The consumer present with flat affect, depressed, agaited and has limited insight into the need for treatment. The writer ask the consumer about what was going on in his life to make him attempt suicide. The consumer states it was life stressor and report that he was in a car accident a few weeks ago. The consumer admits depression, anxiety, limited insight into the need for treatment and impaired judgment and he has some desire to improve on his current situation. Since the patient had left the program, he admits to relapsing and using heroin by smoking, " It was a lace by a cigarette." Client comes to treatment because she has been mandated by the court to receive services for her drug and alcohol usage. Client self-reports an extensive history of drug and alcohol usage, as well as, issues with controlling and maintaining her anger. Client was invited to introduce herself to the other members of the group, and she did so with genuine enthusiasm. She had just been released the day prior from BCP, after completing her mandatory 5-day sentence and adamantly declared that she will not be doing anything that would jeopardize her freedom again. Also, she explained how her mother, father, and daughter are supportive of her recovery, including that her mom would make her leave if she “slipped.” She was fast and thorough with the assignment in which she elaborated on her three top relapse signs which included working too much, thinking about the past, and avoiding talking about her problems in recovery for which she had solutions for: “slow myself down”, “call somebody to talk to” (in recovery), and talk to people at AA meetings respectively.
