On 6/29/17 at approximately 9:30 AM, Security officer Lawrence Mayer (Employee number 31933) notified me, that he may have pulled a muscle while conducting an interior rover of the building. Mr. Mayer stated that while conducting a rover of the building, he dropped his pen in a bathroom, and when he bent over to pick up the pen. He felt a sharp pain on the left side of his back. Upon Mr. Mayer notifying me about what occur, I immediately asked him to have a seat. I went to on ask him, if he wanted to leave work early, he refused. He agree to take the his next scheduled shift off.
On 3/13/16 SO EMT Perez was notified that an engine had come on campus. SO EMT Cunningham responded along with SO EMT Perez and caught up with the engine by lakeside commons. The driver of the engine stated that he was responding for a lift assist to 3144/3146 apt 220 but that he wasn’t sure which one he was going too. SO EMT Cunningham and SO EMT Perez decided to respond to FR-220 first being that they were the engine came from gate 2 and they were closest to that building. SO EMT Perez and SO EMT Cunningham knocked and announce their presence at FR-220’s door and were both verbally invited in. The resident a Mrs. Helen Neff stated that she could not get out of her chair because it was stuck and needed help getting out of it. SO EMT Perez
On 10/21/2015 SO EMT Perez was dispatched to HS-318 regaurding a general assistance call. SO EMT Perez knocked and announced his presnece at the door and was verbally greeted in by the resident, a Mrs. Netha Hopson. Mrs. Netha Hopson stated that she needed help retrieving her purse because her motorized chair was caught on it and she could not get it untangled on her own. SO EMT Perez managed to untangle the purse and retrieve it for Mrs. Netha Hopson. After insuring that Mrs. Netha Hopson needed nothing else SO EMT Perez cleared the scene without further incident.
On 11/4/16, I, Account Manager Enmanuel Cabrera, was notified via email by Shift Supervisor Massiel Perez that Security Officer Bailey Forte no called no showed. S/O Forte is usually off on Fridays, but was scheduled in advance, 0200-0600, to cover the open post. S/S Perez made several attempts to call the officers from 1st shift to cover the open post, but was unsuccessful to get someone to come in. Also on 11/3, during the afternoon parking lot detail AVP1 was 1 flagger short, all vital positions where filled with the exception of the parking lot supervisor/rover. As well as on 11/4, morning parking lot detail was short 1 flagger. Daniel Canela no call no show for the AM parking lot detail. All vital positions in the parking lot were filled for the exception of the supervisor/rover position.
Upon my arrival, I spoke with the complainant, Jason Robertson. Jason stated earlier that morning, Travis Walker, a former employee who was terminated about two months prior, drove onto the complex in a white car. Travis then drove to the back of the compound, behind the Con Ex box. Travis told the employees he was working with a man and needed the concrete saw and rebar gun. The employees told Travis that he needed to leave and that he is not allowed on the property. Travis then left the property.
Zamudio, Human Resources Administrator and acting custodian of personnel records of the Domino Realty Management Company who allowed access, and copies in support of any relevant information pertaining to any injuries, had located a “Work/School Status Report” under the name of the “Talbert Medical Group.” The document had placed the claimant off from work from 2-5-01 through 2-5-01 for pain to the claimants left knee, and yet, according to Ms. Zamudio, the document did not state that a work related injury occurred as there were no other documentation in support of an injury. Furthermore, the witnesses had not cited any job related incidents where the claimants left knee from 2001 had been injured were the alleged 2001 left knee had been irritated or exacerbated in any
On 12/12/16 at 5:15 AM, Weekend shift supervisor Mencido Melo notified me that Flagger assistant Regina Tiboni no called no showed. In conclusion, AVP1 ran 2 hours open for the morning parking lot detail. All vital positions were filled for the exception of the parking lot rover/supervisor. On 12/13/16 at about 5:15 AM, Shift supervisor Massiel Perez notified me that Flagger assistant Cleiry Castro was going to be an hour late and Flagger assistant Francis Tapia no called no showed. Cleiry Castro was able to come in at 6:00 AM. In conclusion, AVP1 ran 3 hours open for the morning parking lot detail. As you are aware, the decision by the Court of the Barton v. Rona case (2012 ONSC 3809) recognized that although Mr. Barton’s misconduct was serious, his specific acts of misconduct were not severe enough to warrant his dismissal. The Court felt that in this situation, a stern warning to him never again to permit a safety violation by an employee would have been sufficient (p.13, para. 55). This was particularly the case, based on your investigation you have performed on April 24, 2009, given that nothing in his excellent work record and no prior infractions suggested that he would not be amenable to such discipline (p.10, para. 40) or that he would repeat such misconduct in the future (p.9, para. 38). Given that the Court could not find that his misconduct met the threshold of striking at the heart of the employment relationship (p.14, para. 55), it concluded that Rona had breached Mr. Barton’s employment contract, and Mr. Barton was entitled to damages as a result (p.14, para. 56).
On Monday, 11-16-2015 she recalled the claimant had punched in early for work that morning and did not say anything to her until shortly later around mid-morning when the claimant came into her office. On this date worker made an unannounced visit to the residence of Mr. Lawson Lovett, for the purpose of linking Mr. Bobby Lovett with VA nursing home application and information. When worker arrived, Mr. Bobby Lovett was cutting grass. Before worker could exit vehicle Mr. Bobby Lovett came to worker's car. Worker exited the vehicle and explained the reason for her visit. When worker gave Mr. Bobby Lovett the nursing home application he gave it back to worker and asked worker if she could help him fill out application because he did not have much education. Worker explained to Mr. Bobby Lovett she would assist with the application. However, he would need to gather documentation and sign application for Mr. Lawson Lovett since he was POA. Mr. Lovett gave worker his number and told her to call, they would set up a time to fill out
On August 5, 2016 at 6:20 am I held Aaron Mills after the morning check in to discuss a situation that arose at Annandale High School. Present at the meeting was Craig Carney, Liaison and Steve Passino, HVAC Supervisor. The previous day Aaron was responding to a service call for the Athletic Directors office area Rooftop B1. Upon arrival at the jobsite I twice questioned Aaron if he had thoroughly checked out the Rooftop unit. Aaron responded “YES”. On 2/17/16 at approximately 8:30 P.M, Shift Supervisor (S/S) Enmanuel Cabrera was notified by Safety Specialist Matthew Marsicano that IH services associate Elizabeth Andrisoqie was asked to be removed by IH services Project Manager (P/M) Courtney Berger via cell phone. Upon arrival, S/S Cabrera spoke with IH services associate Elizabeth Andrisoqie and advised her that IHS P/M Berger asked for her removal. At this time, IH services associate Elizabeth Andrisoqie refused to leave. After, S/S Cabrera advised IH services associate Elizabeth Andrisoqie, that her refusal to leave the premises forces security to contact the State Police for trespassing. On 3/17/16 at 12:10 A.M, Safety Specialist Kevin Sabulsky notified Shift Supervisor (S/S) Enmanuel Cabrera that he saw the officer who was positioned in the exit shack not check the back of a trailer. S/S Cabrera advised Safety Specialist Kevin Sabulsky that he will look into the matter. Upon further review of the video surveillance system, S/S Cabrera saw that Security Officer Dinh Nguyen did not check the back of one trailer at 11:57 P.M on 3/16/16. S/S Cabrera immediately made his way to the exit shack to speak with S/O Nguyen.
