This article was a study that was to look at the effect of advanced practitioners (APs), that include physician assistants and nurse practitioners, in a trauma intensive care unit (ICU) on care processes and patient outcomes. Due to the tremendous shortage of the resident physician (RP) workforce, many resident programs are having a hard time maintaining patient safety and good quality of care. For this reason, APs have been recognized as a possible solution to remedy this problem. The study performed was to evaluate outcomes from using APs on patients admitted to ICU, looking at mortality rates for inpatient, length of stay in the ICU, and mechanical ventilation days. The study looked at each care process provided, and the clinical outcomes
The role of Advanced Practice Nursing (APN) has changed dramatically in recent years. Currently, the Unite States (U.S.) health care is focusing on delivering a cost -effective health care to all patients. In the last decades, there were many efforts to control health care over spending in the U.S. One of such efforts is to focus on applying proven principles of evidence-based practice and cost-effectiveness to find the least expensive way to produce a specific clinical service of acceptable quality (Bauer, 2010). The vast changes in health care system, such as cost, need for high productivity, limitation on reimbursement, and the inadequacy on access have made APNs to think in a way where they most fit to provide independent care for
With the passing of the Affordable Care Act in 2010 approximately 32 million more people will be insured throughout the United States. The need for healthcare workers and providers will be in drastic demand to provide care to these insured Americans. The 2010 IOM report details out how the advanced practice nurse can be a valuable asset in primary, chronic and transitional care and their skill set should be used to promote better healthcare across the nation (IOM, 2010). This impact of this report should help progress advanced practice nurse’s ability to practice without individual state regulation and be governed under one body to server in and outside of the hospital setting
while ICD-10-CM has many differences from ICD-9-CM; the new classification system does retain the traditional format and many of the same characteristics and conventions (AHIMA's 75th Anniversary National Convention and Exhibit Proceedings, 2003). Creating a data integration plan for Green Tree Healthcare starts with should be a unique code for all substantially different procedures.
There have been many studies documented in the literature regarding the reduction of CLABSIs. The majority of the studies have reported statistically significant decreases in CLABSI rates post-implementation of a quality improvement initiative (O‟Grady et al., 2011). Some studies used approaches in which multiple strategies have been implemented together to improve compliance with the use of evidence-based guidelines. A seminal study conducted by Pronovost et al. (2006), known as the Keystone ICU project, included a collaborative cohort of 108 ICUs within the state of Michigan. The strategies in this study included the use of five evidence-based bloodstream infection prevention practices for CVC insertions, use of a checklist to ensure adherence
Some groups, including the American Medical Association, have stated that advanced practice nurses should not have full authority because their training is shorter than physicians (Hain & Fleck, 2014). The concern raised is that an APRN is not trained well enough to safely diagnose and treat a patient without supervision. Many studies have been completed to test this argument. One such study by the Cochran review concluded that in primary care settings APRNs had high patient satisfaction scores and overall improved patient outcomes (Hain & Fleck,
The installment of the ICD-10 coding into the United States healthcare system causes institutions to be faced with some difficult challenges. To conquer these challenges, the healthcare management team can establish an adequate planning strategy. The first strategy is to develop a management team. The management team can organize effective strategies in the work place to avoid costly mistakes. Education and communication are a few effective strategies an organization can achieve during the transition period. An article from the Healthcare Financial Management Association discusses that by not having adequate training for employees can lead to coding errors and reimbursement delays (Noel). By training employees on ICD-10 coding ahead of time
As the physician assistant (PA) profession becomes more proliferative in the United States the role of the PA in various healthcare settings changes. The baby boomer generation is aging which is increasing the demand on practitioners. PAs are becoming vital to closing the patients to provider gap in the United State. There is now an increased need for PAs in all medical specialties. Because PAs are trained as generalists, it poses the need for postgraduate residencies in specialties like orthopedics, obstetrics and gynecology, surgery, and emergency medicine.
ACNPs have been propelled to the forefront as health care providers for the acute & critically ill as a response to a mandated eighty-hour weekly work limit for physician residents. The Accreditation Council
Hello Megan as you mentioned, Advanced Practice Nurses (APN) are prepared typically in master’s degree programs and especially the Nurse Practitioners (NP) are often team trained alongside medical students sharing the same course work, patient rounds and other onsite clinical experiences. The APNs have either a masters or doctoral degree or a certificate requiring up to two years of additional clinical training. After reviewing the studies conducted in 1986, the Congressional Office of Technology Assessment (OTA) concluded that the quality of care provided by the NPs and physicians was equivalent for comparable services, based on the weight of evidence for both process measures and actual outcomes. These studies also found that quality of care
While uncertainty about the role of an Adult-Gerontology Acute Care Nurse Practitioner (AG-ACNP) persists, what research has found about the role is that AG-ACNP’s provide advanced nursing care to those who are acutely, critically or chronically ill in both traditional and nontraditional healthcare settings (Kleinpell et al., 2012). Standard of scope differs between all types of scopes and nurse practitioners alike. The scope of practice (SOP) for an Acute Care Nurse Practitioner (ACNP) is not based on practice setting, but rather what type of care the patient will need, for example, someone who needs ventilator management in either the home or hospital environment (Kleinpell et al., 2012).
NPs are not only moving into increased roles within the acute care hospital setting, but also into roles in the community as Primary Care Providers (PCPs). The anticipated shortage of physicians to handle an increased number of aging patients already affects my position as a staff nurse at a hospital. A career in nursing offers countless paths to further your education, whether it is to become a specialist in a field of practice, foray into research or to pursue a position as a nurse practitioner. I was interested to learn more information about how expanding the role of Advanced Care Nurse Practitioners (ACNP) may relieve shortages of physicians and lessen the burden on our current healthcare system. Increasing the roles of ACNP would only be feasible if patients are still receiving quality care while decreasing the cost of healthcare. As a nurse returning to school to further my education, I would like to know if pursuing a career as an ACNP would be a viable option for me to continue to interact with patients in an acute care setting in a more specialized role.

Palumbo, Marth, and Rambur (2011) stipulate that over the years, advanced practice nurses have continued to avail care that is of top quality. Subsequently, they have been able to create a recognized place within the healthcare system. By 2012, it was reported that there was an approximate number of 240, 000 advanced practice nurses (Newhouse et al., 2012). The report also added to the assertion by Palumbo, Marth, and Rambur (2011) through the indication that advanced practice nurses offer their services in settings that need a heightened range of professional independence while providing healthcare. What concerns their function, LaBrocca and Angosta (2016) indicate that as a health expert, an advanced practice nurse foresees issues in healthcare
The health care industry is experiencing a surge in the number of baby-boomers needing health care and increased demands on the physicians providing the care. This trend has made the role of Advanced practice providers more important (cite). It is imperative to understand the differences and similarities in the advanced practice provider roles as it pertains to healthcare. This paper will identify specialty nursing roles including advanced practice registered nurses (APRN). It will further compare and contrast the advanced practice nurse (APN) and physician assistant (PA) roles in practice.
CD-10-CM has a greater number of new features that allow a greater level of specificity and clinical detail such as combination codes for conditions and symptoms or common manifestations, Codes of combination for poisoning and external causes, extensions were added for the episode of attention among others.
