The reporting party (RP) disclosed that Taylor Trusheim DOB: 4/3/99 was brought into Sutter/Yuba Behavioral Health for a Triage appointment to assess for possible services. Prior to the appointment Taylor's mother La Vonne Bettencourt received a call from a family member who brought up an incident which occurred when she was 4 years old. The family member mentioned an incident where the child was molested by the daycare provider's husband Bret Baker. Subsequently the mother discussed the information with Taylor who confirmed the molestation. The confirmation alarmed the mother who recalled the last day the child was at the daycare. The reporting party is (RP) Ophelia Larose the grandmother of the foster child Aniyah Blake. The RP stated she and the biological mother had a visit with Aniyah on 6/19/15. The RP the foster father (Mark Cannon) did not bring food or diapers for the visit. When foster father was asked about the missing items the foster father replied "the FFA did not provide me with them when they dropped Aniyah off." Reporter stated he was getting ready to go to work Friday morning around 4:30 a.m. and Marquise was standing outside his home crying. Marquise has been sleeping at the home since Friday. Reporter hasn't met any of Marquise's family. Reporter's child is friends with Marquise. He said Marquise's mother is in a nursing home in Noxubee county. Reporter stated he and his wife are trying to help Marquise. Reporter stated Marquise hasn't been in school. He said he was suspended for 10 days but he barely goes to school when he is not suspended. He is suppose to be back at school now. Yesterday was his last day of suspension. The RP stated medical office has received 4 alleged abuse complaints from 4 different children. Each child stated Ms. Lisa was the perpetrator. The RP stated a 3 year old Hispanic girl was initially potty trained. The RP stated the child was eager and happy to attend the preschool however; the child began to soil herself and refused to back to the preschool. According to the RP the child stated that Ms. Lisa put her finger in her vagina.

This worker spoke with the office at East Elementary about picking up Tamyia Crenshaw for the integrated assessment on the 5th of January. The school informed this worker without the guardian's consent this worker could not pick up Tamyia. This worker needed Jeremy to call the school and have this worker put on the emergency contact list for this worker to be able to pick Tamyia up from school. The woman put this worker on hold and spoke with the assistant principal. There was nothing that could be done without the guardian's consent.

REPORTER: The reporter/cousin (Sherry) called with concerns for the victim, Tanning. According to Sherry, Tanning is not being physically abused or neglected. The reporter said she is concerned and worried about the child. The reporter said Tina rides around with the child in the “front seat” (unknown when) and he’s not properly restrained (seat belt over his stomach but not his arms). Also, the reporter said she has pictures of Tina driving down the road with the child in the front seat, not properly strapped in. The reporting party (RP) stated that Llanet Zaligan and is the girlfriend of her husband and according to her 7 year old daughter (name not provided) is operating a day care. The RP stated she received a texted message from her soon to be ex-husband who informed her he quit his job and was assisting his girlfriend in operating a day care. According to the RP there are 17 children in care 5 of the children belong to the girlfriend, 3 of the children belong to the RP and the other 9 children are day care children from different families. The ages of the children ranges from the age of 2 to the age of 12. Subsequently one of the 2 year old children is running around with a "Pink eye" infection. The reporting party (RP) stated the biological mother of foster child John Wilson age 2, reported on 1/23/ she observed John to have red buttocks and penis. The mother asked if someone hurt him and the child replied "yes." Present during the mother and child visit was FFA Social Worker Jennifer Jackson who disclosed a co-worker (name not provided) overheard the mother repeatedly ask John if he was ok of if some had hurt him. Apparently John had refused to pull down his pants and pull-up in order to use the toilet. Once the mother had convinced the child to use the toilet, she observed the red buttock and penis. Reporting party (RP) stated that her daughter, Capri Taniguchi (DOB: 12/18/15) attended this daycare for only 2 weeks. RP stated that she specifically told licensee not to give the child any medication without notifying her or RP's husband first. RP stated that she noticed that the bottle of Tylenol was used and the cap was sticky. RP stated that the licensee stated that the child was teething. Licensee told RP that she would speak to her assistants to find out who administered the medication without permission. RP also mentioned that the she found teething gel in her daughter's diaper bag that did not belong to her. RP is concerned that the facility might have used the teething gel on the child when the child's pediatrician recommended that

The reporter stated on 05/09/16, she witnessed bruises on the child's arm that did not need any medical treatment. The reporter stated she was told by Shermecia that her father believed she may be talking to boys and duct taped her to a chair and whipped her with a belt. The reporter stated that the child doesnt have any behavioral issues and is a straight A student. The reporter added on 05/02/16, she and other faculty at Florence High School witnessed the child's father talk to her in an aggressive way and called her a bitch. The reporter also stated he threatened the child's life and told her he would kill her. Per the reporter school administration were present but nothing was done to intervene on the child's behalf.

On 11/1/16 while walking down steps the child turned away from the RP 19 month child and loss contact causing the child to fall down the steps.
