While I was on duty and assigned as the 4C4 Housing Deputy, Inmate Sarra, Melissa came to the 4C4 Officer’s Station and handed me a Form 62 addressed to the Shift Commander that stated, in part, “I do not feel safe in my room. My roommate threatened me after you let us off lock down.” Sarra told me that Inmate Deak, Nichole was upset with her because Sarra inadvertently woke her up, and said to her, “If you don’t fucking stop, you are going to be fucking sorry.” Sarra said that no physical altercation had occurred. I spoke with Deak at the 4C4 Officer’s Station. She also told me that no physical altercation had occurred, and said that she did not threaten Sarra. On December 11, 2016 at approximately 1330 hours, Detention Officer Harden reported duty and was assigned to work 1200 Baker Street Jail, 3rd Floor, A-Pod Control Center. At approximately 1420 hours, Detention Officer Harden advised she was not feeling well. At approximately 1430 Hours, Detention Officer Harden had her blood pressure checked in the 1200 Jail Clinic and was advised it was elevated. At approximately 1440 hours I called for Houston Fire Department. At approximately 1445 hours Houston Fire Department Medic 17 arrived at 1200 Baker Street Jail. On the afternoon of November 14, 2015 at approximately 5:07 P.M. Deputy Singleton responded to a domestic disturbance call at 4547 Old Salkahatchie Hwy in Early Branch SC. When I Deputy Singleton arrived on scene I interviewed Sheree Washington who stated that after Deputy Ayer had left the scene from the previous call that her boyfriend Christopher Williams who resides at the residence said "bitch I should kill" you then proceeded outside, and retreived a light with a piece of pipe attached to it, and then struck her in the chest. At this time Washington called dispatch, and asked for a Deputy to come back out to her residence. (S1) Inmate Dennis was placed against the chow cart that was next to us and informed him once again to stop resisting and that if he continued he would receive additional write-ups. On August 23, 2014 Sgt. Lavelle was assigned to the 83 Precinct. She was present for duty at 0800 hours as the domestic violence Sergeant and she conducted several home visits. At 1450 hours she was post changed to the 83rd Precinct desk and she assumed the duties of the 83 desk office. Sgt. Lavelle remained as the Desk Sergeant for the entire tour. Calhoun forwarded me his findings and informed me the incident occurred on Evening Watch, I viewed the incident on Avigilon video and determined a use of force incident report need to be completed in Blue Team. I then notified Sgt. Kizer-Hudson, who was the supervisor over 8NE, and inquired on if she was aware that the use of force incident occurred. She replied, “No.” I proceeded to show Sgt. Kizer-Hudson the video and she stated, “I remember the incident the officers called a non-compliant I responded, but they didn’t tell me about the use of force part.” I replied, what did they tell you?” Sgt. Kizer-Hudson stated, “Officer R. Dunans stated he give Inmate Mitchell verbal commands to return to his cell, inmate Mitchell refused and wanted to speak to a supervisor.” I advised Sgt. On 02/12/2017, while working at Durango Jail located at 3225 W. Gibson Ln. Phoenix AZ, 85009 I was assigned to housing unit 2 when I heard a radio call for a "Man Down," in Durango 3 A-Pod, that which I responded to.
At 2225 hours, I asked Inmate Ward who assaulted him and he explained that he was hit one time in the back of his head and fell to the ground so he could not identify the inmate. I also asked Inmate Ward where he was when he was assaulted and he explained that he was talking with another inmate while inside Cell17 facing the back of the cell when he was hit. I asked Inmate Ward if he could tell me who he was taking to and Inmate Ward declined to identify the inmate he was talking to. I asked Inmate ward if he could tell me anything else about the assault and he didn’t know who did it or why. I asked inmate ward if he could think of any reason why he would be assaulted and he again replied no, that he didn’t know why this happened. I asked Inmate ward if he would like to aid in prosecution and he said no.
