Describe 4 features of ACT and Intensive Case Management: four features of the Assertive Community Treatment are:
1)ACT is a Mental health service that is provided to an individual with a mental health disability, this service is provided within a community location such as, in the person’s home, coffee shop, parks or in a shopping centre. (ACT Model. (2017)).
2) The ACT teams usually consist of psychiatrist, nurses, social workers, rehabilitation practitioners. These multi-discipline teams also provide a small staff to individual ratio which cuts down on the need for outside referrals. (ACT Model. (2017)).

There are no time limits for this service, and care is offered on a 24-hour basis. This service is offered and clients are seen in a non-clinical setting. The care team is usually made of up of social workers and nurses working one on one with the individual.
1) Intensive Case Management provides a comprehensive, individualized assessment of the individual, so that the program can learn the person’s immediate and on-going needs. This system of ICM includes encouraging the individual to discover their own strengths and helping them to establish their own personal goals. The treatment plans are developed so that they incorporate the individual’s goals, values, and skills. The treatment plan, also identifies the resources, that will be needed and the type of services that will be required. in The ICM model, a Case Manager is responsible for the individual caseload, while the ACT approach has a multidisciplinary clinical team who share the responsibilities of the caseload. The other difference, is the ICM Case Manager helps the individual by connecting and coordinating service for them with other direct service providers. For example, the ICM offers the mental health counselling but may need to link the person to an outside agency for employment supports and another agency for housing supports. The ACT team generally puts the majority of their time providing clinical services to the individual in the community with very little assistance from other service providers. Both systems are person’s centred and our strength base. The individual, in both the ACT and ICM program is involved in goal setting and in the creation of their service
