The transition from expert registered nurse to novice nurse practitioner includes inherent challenges in adjusting from one practice role to the next (Barnes, 2015). There are both internal and external factors that combine to create this difficulty in role transition. External factors that define the role of nurse practitioner contribute to this challenge by creating role stress. Internal conflicts regarding new roles create role strain (Dreher and Smith, 2017). These two sets of factors taken together have a profound effect on successful role transition. Data was then analyzed to establish themes in the experiences of these new NPs. Finally, this data was used to create a schema of the first year of NP practice. This includes four stages: laying the foundation, launching, meeting the challenge, and broadening the perspective (Brown and Olshansky, 1997). While their paper does not specifically discuss the concepts of role stress and role strain, the ideas are key to the launching stage. In the interviews reported in Brown and Olshansky’s work the participants report role strain. Evidence for this occurs in participant statements such as, “I have this profound insecurity...and I feel like I 'm faking it.” This indicates the internal angst of perceived incompetence. Role stress is also supported by participant statements, such as “I 'm the one who holds up the clinic,” that demonstrate the external stressors applied to new NPs. The authors’ conclusions also support the idea that external demands create role stress and that internal anxieties demonstrate role strain (1997). More support for the significance of role strain during the transition to nurse practitioner comes from an article in the Journal of Professional Nursing. This article cites anxiety, stress, and emotional turmoil as characteristic of the RN to NP role change (Poronsky, 2013).
