The reporting party (RP) stated on 3/7/16 she arrived to find a 6 month old infant named Josephin (last name unknown) crying. The RP stated the infant continued to cry for approximately 10 minutes before the RP went on a 10 minutes break. According to the RP the infant was still crying upon her return. The RP stated she could no longer endure the infant's crying so she picked her up to comfort her. When the RP picked up the infant and determined by the smell the infant required a diaper change. Immediately the infant was removed from the RP care by a teacher named Kim who changed the infant's diaper. Another teacher named Margaret removed the child from Kim, who tightly swaddled and placed her in a crib. The RP asked Kim why the infants are
RP stated law enforcement received a call from an anonymous caller stated an employee, Shemaiah Messengale grabbed the child Marley's arm which is fractured. Law enforcement contacted the child's mother to verify injuries. It was disclosed the child had suffered 4 fractures to her elbow and wrist. The RP stated law enforcement spoke with owner/licensee of the facility and the employee accused of causing the injury. They reported Marley sustained the injuries accidentally when she fell from slide. The RP stated the child was treated at Children's Hospital Los Angeles (CHLA) by Dr. Lindsey Andras who determined the injuries are consistent with a fall. The RP stated on a visit she observed that her son's right eyebrow was swollen and escorted her son outside of the home. The RP stated her son is non-verbal but understands and is able to shake his head yes and no. The RP asked her son if someone had hit him and he nodded yes. The RP stated she named off the names of the caregivers and when she came to a caregiver named Chips, her son identified the caregiver by nodding no. Reporting party (RP) stated that mother Guen Mencias (818) 693-0534 contacted their office today to report that child Xyza Evangelista (DOB: 12/30/13) had a hand print on her bottom. When mother asked licensee about the injury, licensee stated that she was unaware of any injury because she was on vacation and there were 2 staff members left with the children in her absence. RP also stated that on 3 separate occasions, the licensee failed to pick up her son from school (RP did not have the name of the male child). The licensee told RP that she thought her daughter was going to pick up the child up from school (please not that it is not clear if the licensee's daughter or the mother's daughter failed to pick the child up from school). R/s Quankeya’s (8) whole right leg is burnt. R/s Quankeya’s leg is wrapped from her upper thigh to the top of her knee and from the bottom of her knee straight down to her ankle is also wrapped. R/s according the child’s guardian, Keyanna, Quankeya and a smaller cousin put water in the microwave and was burn. R/s it was reported that Keyanna was in back of house when the children were putting the water in the microwave. R/s the smaller child was burned on the left side. R/s Keyanna stated that she was told to take Quankeya to the burn center, but she didn’t take the child instead she will call the Burn Center when she gets a chance. R/s Quankeya is not coherent and Keyanna said the child is on Tylenol with Codeine. R/s the fluid is seeping through the band aid. R/s five other children are in the home.
On 09/10/15 Ms. McClain voluntarily admitted her son to Parkwood for treatment; the child has not been given a discharge date. During the assessment with the child and his mother, Jameraqua disclosed to the reporter that he and his mother have had multiple physical altercations in the past and a several months ago his mother whipped him with an extension cord. The child did not have any visible marks or bruises and indicated to the reporter he doesn’t have any. Jameraqua feels his mother fights him because she dislikes him; Ms. Mitchell is unaware of the reasons. The reporter witnessed a volatile relationship between the child and mother and they had to be separated during his assessment.
