At 0824 this clinician met with the patient who reports her weekend was not unpleasant. She reports she is continuing to have problems locating housing. She reports her family members continue to refuse to allow her to live with them specifically her father and her uncle. She reports she has one friend who is considering allotting her the capability to reside with her under the condition that she speak with her Child Protective Services (CPS) caseworker first. On 7/11/2015, CM did a visual and had client come to the social service office. CM completed Bi-Weekly ILP Review. In the meeting client appears to be wear out, and tired. She was constantly throbbing her forehead, like if she was having headache. CM inquires what the problem is. Client replies “she doesn’t like the shelter food and sometimes she doesn’t eat” CM advised the client to eat and nourished her body. CM also observed that client is depressed but she continues to refuse medical referral to see a psychiatrist and medical doctor. Client continues to mention her son who is in foster care, and the physical altercation she sustained many months ago here at this shelter. On June 2, 2016, I was assigned this case to follow up. This case involves Mr. Mirna Zelaya-Portillo a 16 year old Minnie Howard School student. I contacted Marillu Portillo-Zelaya (mother) on June 6, 2016. I received permission to speak to Mirna at school. I contacted School Resource Officer Pierre Hill who checked attendance at the school and Mirna was not in attendance.

The case worker came out and told us the director wanted to speak with us, but we would have to wait a while. The following is a case study of a client by the name of Lydia Sanchez. Lydia has been truant 20 times within the past 3 months. Lydia teacher Ms. Ellis sent a referral to the main office because Lydia came in her classroom smelling like alcohol, and marijuana. Ms. Ellis also noted in the referral that Lydia pushed her when she tried to look inside of her backpack. The principal found cigarettes, and a white water bottle filled with alcohol inside Lydia’s backpack. Ms. Ellis noted in the referral that Lydia has not handed in any assignments this year yet. Ms. Ellis told me that Lydia was a straight a student last year, and that she does not know what has gotten into her. Lydia’s mother Stephanie also called into the school stating that her daughter Lydia needs help, and that she has been dating a 21 year old man for the past 6 months. Stephanie does not know what to do with Lydia anymore, and she is worried that she might drop out of high school.
On 3/28/16 A Case conference has been conducted with Client Alexandra Almonte #119, along with HS Supervisor Ms. Garland, RS Supervisor Mr. Mitchell and Sr Case Manager Ms. Arias. The reason of this conference was as per client request due to a previous incident that occur on 3/15/16 that a smell of marijuana was coming out from Ms. Almonte unit, which she denied , stating that she did not some her unit and she refused to open her door at moment because she was in the shower. Ms. Almonte received a writer warning from her Case Manager Ms. Arias and that upset her therefore she request a conference with Ms. Arias supervisor. The department’s intervention plan was to remove the child from the home and place her in a safer environment. My client’s response to this intervention at first was very angry and resentful towards the department.
