CCIB received an emergency response referral on 8/5/15, referral number 1705-7411-4187-5075755 by reporting party Florence Keitel. RP reported during a supervised visit with Tahnya’s biological parents (names not provided) at the family visitation center on 7/31/15, Tahnya disclosed to her biological parents that “Mina hit me”. RP states that Tahnya mother asked “who’s Mina?” and Tahnya said “Ms. Annette hit me on my booty”. RP reported that Tahnya’s mother stated that she would talk to Ms. Annette about the incident. RP states that Social Worker Keitel was also present when Tahnya made this statement and states that with the assistance of Tahnya’s mother, Keitel did a body check on Tahnya and did not see any marks or bruises on her body. RP
CM met response time regarding the safey and well-being of VC Ariahna Hampton. Ariahna reported that she was being physically abused by Bmo Tiffany Hutchinson. The intial interview was conducted at school. Ariaha reported where Bmo is employed at, and where they reside. VC Ariahna reported that her mom hits her with her fist one time upside the head on a daily basis. VC Ariahna said sometimes it hurts. VC Ariahna reported that she has never had any bleeding, marks or bruises after her mom hits her. VC Ariahna reported that she is not scared of her mom and likes living with her sometimes. CM met with mom, who denied all allegations in the report. Bmo reported that she rarely spanks VC. Bmo confirmed that they do reside with MGM on Merrymount
As Avery's civil case of $ 36 million against Manitowoc County progressed, a photographer, Teresa Halbach, was nowhere to be seen and was reported missing. The last place she was known to at the Avery’s auto salvage property where she had an appointment with Avery to take a picture of a car there. Avery in the eyes of Manitowoc County he was the prime suspect. Manitowoc County “found” Halbach's burned bones in a fire pit, her car in the yard, blood stains on the car's interior, and her car keys in Avery's bedroom. Avery was then arrested again, and then was interrogated for many hours to see if he would confess to the murder of Theresa Halbach, and once again was not allowed an attorney. Why? CCIB Intake received a call from Melissa Sadikoff, MSW, LCSW from Orange Coast Memorial Medical Center who informed that a report was made by the reporting party regarding 1 year old Melanie Larmarre DOB: 5/17/11. The mother of Melanie informed the reporting party she had bathed her daughter approximately 5PM. During the bath the child indicated to her mother "it stings." The mother questioned the child asking "what hurts?" The mother had noticed blood on the tissue used by the child after using the restroom. The child indicated to her mother her vagina was hurting. The mother asked the child did something happen and the child disclosed that two other children Jackson and Penelope stuck a drumstick toy in her. The child was pointed to her vagina
CCIB Intake received referral number 0560-8525-1774-8086170 from reporting party Amy Rodriguez mother of Penelope Rodriguez on 4/2/16. The reporting party believes the black eye to have occurred from physical abuse do to "personal experience." She believes the black resulted from being hit or by being hit with a toy. The reporting party brought Penelope to a doctor (date unknown) due to the child's hair loss. The doctor suspects the child has Alopecia, however the doctor is not certain about the diagnosis. It was presumed the child's hair was falling out due stress. The doctor advised the child be seen by a specialist to confirm the diagnosis.

This case involves Ms. Brianna Mona Haggins a fourteen year old Minnie Howard School student. Briana left home January 6, 2016 and was returned on the same day by Officer Hudson. On January 11, 2016, a call of service for a domestic assault was reported for 6300 Stevenson Ave Apt# 611. In this incident (16-102714) Brianna and her mother (Sherry Haggins) were determined mutual combatants.
IRTC reviewed documentation entered in Connections by ECS IRTC, Heather Lewis, which stated, CPSSII/586 stated a visit was made to the hospital, child was discharged to parent with a follow up for tomorrow. As per medical staff child had no visible injuries.” ECS CPS, Shantae Dukes, documented on 6/10/17 that that home visit was made and, “No physical injuries were observed.”
Valerie called that office several times to inform me of the situation. Ms. Valerie stated that Catina took Bryce to her home on Monday, July 17, 2017, around 4 p.m and did not return until about 10:00 p.m. Ms. Valerie stated that Catina told her that they were going to group at HCAP office for Anger Management. I, Shalina Holmes stated that we do not have a group session on Monday’s at the HCAP office. Ms. Valerie stated that the assigned advocate Catine Hampton took Bryce to the forest and was unable to determine when the advocate took Bryce. R/s July 3rd-8th Danielle (4) and Josiah (5) were staying in Georgia with Judy and Felicia Wilson. R/s Felicia beat Mariah with an electrical cord and Mariah has scratches on her back and bruised buttock. R/s Danielle (mom) took the child to ER and trying to press charges against Felicia.
Jabori (Wytisha’s boyfriend) physically abuses Wytisha in the presence of Karter. Jabori physically abused Wytisha 11/23/17; the most recent incident was 11/30/17, which resulted in Wytisha having a black eye. Louise contacted law enforcement 11/0/17 to conduct a wellness check, but there was no response. Wytisha usually contacts law enforcement only so that Jabori can run. Law enforcement previously went out to the home, but after arriving Jabori ran and Wytisha would not provide Jabori’s name.

CCIB received a complaint via phone call from RP stating for the last 6 months Galen a non-verbal and Autism student hygiene has been declining. RP stated that occasionally Galen has been in class with a very strong body odor, very greasy hair, hands filthy and increasing bad acne on face, RP thinks that he is not being properly cared for. RP stated she also recently observed that Galen has been at school with cloths that does not fit him. RP gave an example of Galen’s shirts are too small (not covering his stomach), he has holes in his shoes, and his pants are too tight. RP stated that she has contacted Galen’s mom Bonnie (650) 520-9972 to get information and to tell her about what is going on with Galen. RP stated that Bonnie said that she
CCIB received a telephone call with an allegation against the facility. RP called to report that there is drug use and domestic violence happening in home due to the licensee runs a daycare. RP reported that on 10/9/2017 Veronica and Juan went to pick up children from school and Veronica asked staff at the school to call the police. RP reported that staff told her to contact the police to report what is happening. The RP reported that the police came to the school and arrested Juan in the parking lot. RP stated that it was stated on the police report that Veronica was hit in the face and kicked (unknown when incident happened). RP reported that the police report stated Veronica had a cut lip and red marks on her legs. RP also mentioned on 10/10/2017
Lisa Sherman worked for Verizon for many years. While there she never revealed her sexual orientation .Then one fated day, there was a diversification workshop with her fellow managers. The outcome of this was that her coworkers were bigoted towards homosexuality. Lisa Sherman had a tough decision to make, whether to stay or to leave. The reporting party (RP) stated resident Arev Dabbagh DOB: 12/27/00 disclosed on 4/15/16 she inappropriately touched by a male peer. According to the Arev on 4/14/16 a peer Ibraheem Mamoori DOB: 9/2/98) placed his hand on her shoulder and continued downwards towards her breast. The peer touched her breast over he clothing. The resident was distraught while she recounted the incident. Arev alluded to their being other incidents with the peer and other unknown males. Arev didn't disclose details or clarification. Arev was placed voluntarily in the facility by her mother and will continue her placement for a few more weeks. The peer is currently placed on a 5150 hold and will not have access to Arev in the future.
