On 6/2/16 at approximately 2:00 PM, Inbound bound Area Manager (A/M) Caitlin Summers (caitl) notified Security Officer (S/O) Zachery Smith that a yard jockey caused damage to a dock lock. Then, S/O Smith spoke with RJ Davis driver Mark Billus, who was driving tractor 279 at the time of incident. Mr. Billus stated to security that while backing UPS trailer 878098 into Dock Door 58, he notice that the dock lock was not properly working. After, Mr. Billus pulled out the trailer from Dock Door 58 he saw that the dock lock was damage and also was the UPS trailer. After speaking to the Yard Jockey, S/O Smith took pictures of the damage on the trailer and the dock lock.

On 12/8/16 at approximately 11:30 AM, An Outbound Desk Clerk notified Security Account manager Enmanuel Cabrera that a yard jokey found a trailer with damage. Upon being notified, A/M Cabrera instructed Security Officer Larry Mayer to take pictures of damage and to gather the information needed. Upon arriving at the location where the incident occurred, S/O Mayer was met by RJ davis Yard Jockey Mark Billus JR. Yard Jockey Mark Billus JR stated that, while pulling XPO trailer W17150 out of dock door 6, he notice it was too close to dock door 7 for the TDR crew to properly seal the trailer. On 6/2/16 at about 7:30 PM, RAD 1 associate Christopher Veizaga (veizaga) asked security if he would be able to leave out of the B building bypass with a large box. The box contained 17 TC-55 also known as dolphins. Shift Supervisor (S/S) Enmanuel Cabrera then asked Mr. Veizaga, why exactly did he have to remove the box out of the building? Mr. Veizage stated that it was a last minute shipment to another site and did not think that he needed to put an asset removal form because it is not product. S/S Cabrera quickly spoke with Operation Manager (O/M) Thomas Gerlach about the situation at hand. On 9/28/16 at 8:30 AM, Area Manager Daniel Ahner notified Shift Supervisor Ashley Lorah that a RJ Davis Yard Jockey caused damage to a trailer while backing up into dock door 4. RJ Davis Yard Jockey Danny Gorski, driving tractor 241 pulling trailer P5076637 out of slip location F-957, accidently damage the trailer’s door while backing up into a dock door. Mr. Gorski stated that while he was backing the trailer into the dock door, he notice that trailer’s door unlocked and hit against the building. No damage was found to the dock door but, the hinge on the trailer’s door was damaged.
The Work Breakdown Structure (WBS) helps develop all the work structure and responsibility structures of a project. There were different issues and inputs during the first meeting between the members. They were considering hiring some qualified referees in the playground to minimize the uneven decisions. They were also discussing about contacting local colleges to find an appropriate field and the entry fees of the players. There was confusion about rewards of the players, they have decided to contact with local corporate sponsors and collect fund. They have decided hiring an artist who will design the unique T-Shirts & Sweatshirts for the players. The WBS will be helpful for Nicolette about how to responsibly distribute, cost estimation, scheduling and resource allocation on the project. This information may also be used in future tournaments. Overall, the WBS will provide a detailed picture on the project and will help create a structure for the activities that needs to be planned and managed.
