For this example i will use the name of Phillip Smith. Phillip prefers to be called Phil and never uses his GIVEN NAME .He fills in the forms as Phil Smith, this name is then given to the awarding body .Mr Smith passes his exams and his certificates arrive with the name PHIL SMITH on which he is very happy with . Twelve months later Mr Smith applies for an SIA licences and is rejected as his name does not match his GIVEN NAME .He then complains to the training complains to the training company that not only has he lost his SIA application fee £240.00 he has now got to pay £35.00 per certificate (£70 in total) to have his name changed he has also lost out on a job £500.00 per week. The company advises Mr Smith that the paper work he had signed had stated his GIVEN NAME and that the company had used the name he had put. In this case this was the candidates error but there has been incidents of inputting error which then this would have been very costly to the company not only financially but also to business .
