Lexis Rhoads is a 13 year old followed by Pediatric Neurology Outpatient and Dr Suliman. She has newly diagnosed generalized epilepsy with convulsions and absence. She was diagnosed few month ago during the summer. She was seen for the symptoms of vivid visions but is also struggling with the adjustment to the diagnosis. We would like to refer her for the CBT individual therapy at the UPC with Jasmine Marini who work with children with chronic illness at the UPC .. Jasmine can you please help and schedule this pt with you.We also have a CBT group therapy for children with chronic illness but we are not recruiting at this time we could put her on the waiting list. The pt Poteat, Edward L is supposed to tx today at unit 1087. The unit 1087 is requesting the referral source to send a shortened and missed treatment report in order for the pt to be accepted. This report is not one of the requirements that is needed in order for a pt to transfer to a FKC unit. Also the referral source stated the she sent the report several times and received several conformations and clinic 1087 is stating that they never received it. The referral source and the unit 1087 have been in contact and I am just now finding out about this. Please let me know if the pt is accepted and to see what we can do regarding this patients
At 0824 this clinician met with the patient who reports her weekend was not unpleasant. She reports she is continuing to have problems locating housing. She reports her family members continue to refuse to allow her to live with them specifically her father and her uncle. She reports she has one friend who is considering allotting her the capability to reside with her under the condition that she speak with her Child Protective Services (CPS) caseworker first. She reports she does not want to reside in a homeless shelter with her son as she has done this before and the experience was not up to her standards.

Ms. Orlando reported the clinical director will review youth’s documents and will determine admission date. Ms. Orlando will continue to keep CM updated regarding youth’s status. Greek mythology has always been my favorite of all myths. Ever since I was a teenager I always had an interest in learning these stories. For this paper, I selected one of my favorite Greek heroes, Heracles. I grew up watching the stories of Heracles’s adventures on TV, and it has always fascinated me. Heracles, also known as Hercules, was a demigod; son of the mighty god Zeus and a mortal woman. He possesses an incredible strength that no other mortal on Earth have ever had. However, the same way as most of the Greek gods and demigods of the Olympus, Heracles is anthropomorphic. He speaks and acts the same way as us, humans, do. Some of the heroic qualities that Heracles possesses are bravery, strength, skillfulness, and humbleness. He does not act or do things for others with the purpose of obtaining fame. This research paper is an analysis based on the life of Heracles, its personality, labors, and death. In the process, I will be evaluating his acts and comparing them to those of human beings and gods.
CM received an email from Monica Guzman (Hunterdon Group Home- Assistant Executive Director) with carbon copy, Meghan Kelley (therapist at EMYL) regarding a scheduled transition date. CM was informed that Hunterdon Youth Services is ready to admit Jason (youth) on Thursday, 3/2/17 at 3:00pm. CM confirmed and will transport youth to Group Home. Ms. Guzman reported that the following documents are outstanding; school transfer card, psychiatric evaluation (within 1 year), updated physical form, and IEP/school records. CM stated that she will fax the IEP/School records to Ms. Guzman. CM will follow-up with Kerry Lopez (Child Study Team) regarding school transfer card.
On 12/12/16 L. Freeman met D. Williams, R. Morrison, and the FAPT team members at RCDSS to have a FAPT meeting for Zachary. During this meeting Zachary's needs, and strengths were addressed. They spoke about the therapy he would receive and how he was doing with it currently. Corrections and updates were made regarding Zachary's plan.

Is there a day next week I could interview Kayla Williamson? This would take place during your Achievement class. If next week does not work, that is ok, her IEP is not until the middle of January. However, I would like to get the interview complete as soon as possible, therefore I could possible hold the IEP before Christmas break.

I just wanted to touch base with you about Landon's referral for occupational therapy. We are in the process of switching doctors and unfortunately, he has not seen the new doctor yet. He has an appointment on September 29th; due to the flooding we were not able to make his prior appointment. I filled out the paper for the referral with new doctors information but, I did not want to send it without an explanation to why the doctor's office would not recognize his name.

Everyone is influenced by their surrounds, and the people they choose to be around, whether they are being influenced in a positive or negative way, in Macbeth’s case, he was highly negatively impacted. Macbeth starts out as a good person, and good soldier, true to his king, that is, until he meets the weird sisters who predict he himself will be king. With the influence of his wife, he becomes treacherous, and no longer has a know between right and wrong. By the end of the story, Macbeth was so caught up in his ambitious power hungry circle that he ended up getting killed by it, after having just about everyone turn on him. The negative influences that affect Macbeth for the worst throughout the story happen to be his wife, his conscience, and three weird sisters he meets one day in a field; all of these influences change Macbeth for the worst, causing turmoil all throughout the story.
