On 3/22/18, Linda Blaylock came to the administrators office and alleges someone (later identified as former employees, Greta O’quine, CNA and Stephone Else, Medication aide) told her that Robin Lyles, LVN, ADON instructed the nurse Phillip to “take them to china” when a resident has unwanted behaviors, which creates an adverse reaction and possibly death. Ms. Blaylock explains the phrase means to use antipsychotic drugs and Benadryl to sedate the residents. Ms. Blaylock alleges this was told to Gwen but did not do anything about it. However, According to former employee Stephone Else, medication aide, alleges she heard Ms. Lyles tell Phillip Yovonnie, LVN to “Take her to China” with Amara Anele, RN, and Lilian Foster, Medication aide present at 6:20-6:30am. Most of the staff interviewed never heard anyone say “take them to china” with the exception of the former employees, Greta O’Qine and Stephone Else. When interviewed, Ms. O’Quine or Ms. Graham alleged she never told Gwendolyn Rubbell, DON nor Administrator Kendra King about the phrase “take them to China” or anyone else before. Per investigation, there was no failure to report. Facility could not prove this neither allegations. Later, Stephone Graham, Med aide alleged when resident Virgil Green coded, Robin Lyles stated “lets crack his ribs”. In the investigation, there were no witnesses that indicated there was a phrase was said to that nature. Roommate (interviewable) stated he was in the room and no one made the statement “crack his ribs”. Both staff who were involved and residents denied allegation. On Thursday 12/24/2015 at approximately 2307 hours. Security Officer Omar Alonso (420) was contacted by E.D. Charge Nurse Sharey Selover about an uncooperative intoxicated male patient, Jose D. Gonzalez (DOB: 03/30/1977; FIN# 85006354), come in through the EMS Offload area. Officers Alonso and Ayuso reported to the call and observed an intoxicated male being wheeled into the Special Care Unit (SCU) E.D. room # 39. According to his assigned Nurse Sara Lopez, the patient had been involved in a physical altercation and had been kicked hard in the groin area. Patient did not behave badly or disruptive once he saw that Security were present and his Nurse was able to get his vitals, blood work, and urine without having any issues. Security staff
On 06/27/17, the nurse from VNS Shaneeza Khan called the agency to report that when she visited Mrs. Slaninka on 6/26/17 the patient told her that on 06/26/17 in the morning the aide spilled hot coffee on her leg which cause burn. The aide did not reprt thos incident. Patient had the doctor’s appointment later that morning, the doctor recommended to put bacitracin and light dressing on her leg. In addition, the patient mentioned that the aide is always late for her shift, takes multiple breaks for cigarettes and needs to be called multiple times if she needs her. During interview Ms. Stoker was alert, aware of surroundings, and answered all questions appropriately and independently. During the visit Ms. Stoker dry heaved into a garbage bucket, and visited the restroom once. The worker was at the residence for about 2 hours. Initially Ms. Stoker was shaking uncontrollable to the point of shaking the entire bed. At the end of the visit the shaking had stopped and Ms. Stoker was smiling and interaction with worker while talking about DELETEbeing a wife and mother. He was brought in by his friend Gavriel Martinez. At 2338 hours Orange County Sheriff's Deputies Kenneth Jorgensen (Badge# 8180) and Deputy Michael Fairley (Badge# 6210) arrived and interviewed the GSW patient, both deputies were already on site for an unrelated event. Security Manager Richard King was informed by text of the incident at 2340 hours as well as Nursing Supervisor Debra Reilly. At 2350 hours OCSO Deputy Kristine Helms (Badge# 5021) arrived and requested to speak to the patient’s friends who was waiting in the E.D. lobby. At 0010 hours Deputy Michael Fairley contacted SO Alonso and advised that it was OK to open the Emergency Department. The lockdown was lifted and Security Manager and Nurse Supervisor were informed of the incident. The patient, Lopez was airlifted to Orlando Regional Medical Center (ORMC) at 0030 hours at which time OCOS Deputy Kristine Helms informed me that the Gang Unit task force has been called for further investigation. On March 2, 2016 at approximately 2013 hours Security Officer Tom Mejia and Shift Supervisor Steven Evans responded to dispatched call for a 51D (Disorderly Patient in ED) to Emergency Room #42. It was reported that the patient was intoxicated and was attempting to leave. On arrival, E.D. Registered Nurse Camila Perez explained that the patient, Ms. Shayna Patkotak (FIN: #85305794) was indeed intoxicated and was wanting to leave but she was back in her room. Ms. Shayna was verbal about wanting to leave and smoke but the medical staff was able to get her to comply with them. Security stood by while the medical staff attended to her. CCIB received a SOC 341 via email from Maria Doreen Ainsleigh (medical SW) regarding Terrylee Young (DOB 2/18/1948). RP reported that Terrylee stated that some of the nurses treat her and other residents bad (did not state how). RP reported that Terrylee stated that the staff (names unknown) yells and scream at her and other residents (LPA contacted the SW and Terrylee neither parties could tell me what staff says). On Monday January 4, 2016 approximately 11:40pm Stephen came downstairs looking for EMS for his mother who was having an epileptic seizure. C. Horton went over the 900 side to see what was going on. On August 26, 2015 around 2335 hours, Security Supervisor Steven Evans, Security Officer Allan Topher, and I was dispatched to room 5104 in response to a non-Baker Act patient, Lillie M. Smith (FIN:84487321) who was arguing with medical staff. Upon arrival, we made contact with Nurse Julia, who stated that Mrs. Smith did not want to leave her room. She further stated that Mrs. Smith was going to be transported to room 510 bed #2. On Thursday 07/21/2016 at approximately 2223 hours, Security Supervisor Steven Evans was contacted by Assistant Nurse Manager Robbie Philips via landline and asked to conduct a (44V) Enforcement Escort Visitor Off Property for the discharged female patient in E.D. #48. The patient, Susan Harris (DOB: 03/22/1952), had been quarrelsome and refusing to leave. SOs Christopher Paz and Ariel Weiland responded to the scene. Upon arrival, we observed the patient laying down on her bed, we approached the discharged patient and spoke with her. Mrs. Harris agreed to leave without further incident. An experienced nurse Julie Thao was taking care of 16-yeas old Jasmine Gant who was about t give a birth. Thao is accused of making a mistake that had terrible and tragic result on the life of a pregnant teenage, unborn child, Gant’s family, health care, and Thao’s life. Thao mistakenly gave Gant an epidural anesthetic intravenously instead of an IV antibiotic for a strep infection. Within minutes of receiving the epidural IV, Gant suffered seizures and died. Her child, a boy, was delivered by emergency Caesarean section and survived. So what caused this tragedy to happen? The employees were accusing Aurora for being under the influence of Marijuana. Aurora denied the allegation and Kate stated "you're a F'n liar, and Shawna reiterated "you're a liar." Both staff stated "no wonder why you are in the system, because you don't listen." According to Aurora the staff threatened to "tear up" her room to search for drugs. Aurora was crying and told "shut up and stop being a baby." The RP stated Aurora had an encounter with a young boy who put a condom on a broom handle and it her on the "butt" and waved it in her face stating " you slut, you should use a condom. Aurora mentioned the incident to staff person Chris and was told "you shouldn't let that bother you." The RP stated Aurora recalled staff person Kate telling another resident to "knock the F off." She also recalled staff person Shawna saying to her "if I were to speak with my mother in such at tone my mother would kick my ass." Other comments that were heard are "get your ass inside", "if you don't stay within my eyesight we are going to send your asses back home.", "there is no reason to act F'n crazy, because it is not going to help you with anything."

As noted, on February 29, 2016, the patient was nonetheless admitted to the UCR hospitalist. This was a senior member of the UCR hospitalist team who knew or should have known all of the policies and procedures for admission, and should never have admitted the patient as an attending to the hospital. In so doing, he was directly and deliberately interfering with the doctor patient relationship.
Upon my arrival I, made contact with the complainant Linda Reyes, Mrs. Reyes stated that she was taking a bath when her friend later identified as Crystal Taylor knocked on the door and was yelling. Mrs. Reyes stated that when she opened the door she saw Mrs. Taylor take some pills Mrs. Reyes thought that Mrs. Taylor was over dosing her self and called 911. EMS was already on scene and had located Mrs. Taylor in the woods behind the motel.
