INVESTIGATION: On 09-01-2015 at 0750 hours, I was notified of a fatal traffic crash that occurred at the intersection of Kings Avenue and Lumsden Road. I responded to the crash scene and upon arrival at approximately 0800 hours, I was assigned as to forensically map the scene. I observed a silver Lexus GS 300 sedan, stopped within the outside lane of Lumsden Road, east of Kings Avenue. The Lexus had damage to the front bumper, left front quarter panel, hood, windshield and roof. The damage was caused when the Lexus struck the bicyclist. West of the silver Lexus and stopped in the median lane of westbound of Lumsden Road, was a tan Infiniti J30 sedan. There was a black backpack lying south of and next to the bicyclist. The green hat belonging to the bicyclist was lying on the roadway, just to the southwest of the bicyclist. A cell phone, and cell phone holder were found within the inside, eastbound lane of Lumsden Road, just east of the marked crosswalk. The mirror from the driver door mirror from the Lexus was found lying near the center of the eastbound lanes of Lumsden Road, also just east of the marked crosswalk. I forensically mapped the crash scene using a Sokkia Total Station. (Serial #: 147397) The data collected at the scene was downloaded into the Cad Zone 9.4.92 Drawing Program. A scale diagram of the scene was completed and a copy of the downloaded points was printed. There were evidence cones that had been set up on the crosswalk markings, on the west side of Kings Avenue, across the eastbound lanes of Lumsden Road. At the time the cones were mapped, it was unknown that the cones had been moved or hit and were no longer properly marking the crosswalk. When the diagram was completed, I used a satellite image of the intersection to place the crosswalk. I also used the satellite image to place the island median on the southwest corner of the intersection as well as the crosswalk on the south side of the intersection.
Prior to my arrival the involved parties moved their vehicles in the Peasley Canyon Park & Ride. I contacted Cheryl Southwick (UNIT 2). Cheryl said the collision actually happened at the intersection of West Valley Hwy S and the eastbound off ramp of Hwy 18. Cheryl stated she was stopped at a red light on the off ramp of Hwy 18 in the left, right turn/straight lane in her grey 2012 Subaru Impreza WA/AHH3445, waiting to turn right to head south of West Valley Hwy S.

On 11/08/2015, at approximately 0201 hours, your Affiant and assisting Officers were on State Route 924 near the Girard Park investigating a vehicle accident. Officers had marked police vehicles with the emergency lights activated which were stationary on both sides of the roadway.
In this paper, I will discuss the tragedy incident of Bruce Kevin Lee. First, I will state his background and his journey to the Sheriff’s Department. Next, I will present his expertise in collision diagram/accident reconstruction. Then, I will analyze his final call that lead to his end of watch. Lastly, I will diagnose the call, and point out what went right, and what went wrong. My purpose on this paper is to remember Bruce Kevin Lee and to honor not just him but all the officers who are willing to go out on the streets to protect our community.

On 04-01-2017 at 1252 hours, Det. Carter # 181968, notified me of a traffic crash fatality that occurred on County Road 39, approximately two-tenths of a mile south of Thompson Road. The crash occurred at approximately 1200 hours. I responded to the crash scene and upon my arrival at approximately 1408 hours, took digital photographs of the scene and also forensically mapped the scene.
On Friday, December 15th, 2017, at 2013 hours, I, Officer Williams was patrolling westbound on Highway 90 near Oak Street when I notice a green Jeep (AL Tag:2BY7614) swerve several times over the right line of the right hand lane. I engaged emergency lights to initiate a traffic stop which ended just west of Oak Street on Highway 90.
