On Sunday, 10/04/2015 at 01:59 hours, I, Deputy Stacy Stark #1815 and Sergeant Carson Bunton #2029 were dispatched to 2615 Dutch Ridge Road, Carbondale, IL for a report of a Battery In Progress. The victim reported she was battered and pushed down by her boyfriend. I arrived on scene at approximately 02:11 hours. I approached the front door and the victim, Hollie J. McIntosh (F/W, DOB: 07/26/1988) opened the door and let me inside. I inquired with McIntosh what happened. McIntosh stated she was shoved and choked by her boyfriend, James Dean Carvell (M/W, DOB: 01/25/1988). I observed a red mark on McIntosh’s neck. McIntosh advised Carvell was in the bedroom. I re-entered the residence and spoke to McIntosh. I gave McIntosh a copy of the Illinois Domestic Violence Act Victim Information and the Written Statement and Explanation of Rights. McIntosh signed the Crime Victim Rights and Compensation Acknowledgement Form at 2:12 …show more content…

The photographs show redness to her neck, scraps on her right forearm, and a mark on her right wrist. I requested McIntosh to complete a voluntary written statement and call the Jackson County Sheriff’s Office when it is complete. McIntosh did not want to sign a complaint for Domestic Battery. McIntosh only wanted Carvell to go to jail so he would sober up. McIntosh feels that Carvell needs some type of alcohol rehabilitation. McIntosh stated she is only afraid of Carvell when he drinks alcohol. Deputy Justin Bandy #3105 and Sergeant Bunton removed Carvell from my squad car to Deputy Bandy’s squad car. Deputy Bandy transported Carvell to the Jackson County Jail. Carvell was released to the custody of the Jackson County Jail. Deputy Bandy issued the Domestic Violence Battery Notice to Carvell prior to him exiting the jail. This is the report to
