Program Assessment How many persons are receiving services? As a whole the CHEER program is now actively involved within the Liberty city’s Block by Block boundaries. This extents north until NW 79 street, west to NW 27 avenue, east until I-95, and south until NW 41 street. There are 13, 089 families within Liberty City. Within those families CHEER services approximately 350 families. Are those receiving services the intended targets? Of those receiving services, all are consider to be the intended targets. CHEER is funded through a grant from the Health Choice Network who has placed, as any grant would, strict regulations as to whom CHEERS becomes available to. Continued progress is very much dependent on community cooperation and vision. Barriers we anticipate include: realistic concerns about privacy law (HIPAA and FERPA). National standards and regulations for the exchange of health information are more developed than for educational records. Resistance of clinical, educational and social service professionals to change practice patterns to be able to effectively utilize cross-sector data. Significant change management and partnership development will be needed to implement new protocols to leverage an increasingly rich, yet potentially overwhelming, store of health, education and social service information (The strategic community plan for Miami Childrens Initiative, …show more content…

Because CHEER is such a new project, most of the regulations of collecting data and delivering services are created by staff members who may not necessary have the background in social work or social services. There are a lot of trails and errors which for the most part is embraced by the Director(s). Is the program well organized? Does staff work well with each other? The CHEER program has not been mastered fully and because of its constant growth as it contains school and community involvement; the program probably will never be welled organized. The staff of MCI are open to each others mistakes and is williang and ready to lend a helping hand when needs arrive. Does the program coordinate effectively with the other programs and agencies with which it must interact? The CHEER program coordinates with other programs clients are a part of to effectively develop a cradle to career model in which all parties involved with the families work together to achieve the goals set by community, family and organization(s).
