CPS spoke to social worker Millie Khemiri at the Family Assessment Program (FAP). Ms. Khemiri reported that the source came to FAP requesting services for the child Wellington. Ms. Khemire reported that the source reported that Wellington smokes marijuana and drinks alcohol. CPS inquired if the mother reported that the child frequently visit a family on Utica Ave. Ms. Khemire stated yes and reported that the mother had a GPS tracker locating the child on Utica Ave. CPS asked Ms. Khemiri if the child reported smoking marijuana and drinking alcohol at the family home. Ms. Khemire stated that the child didn’t disclose smoking marijuana or drinking at the family
PER REPORTER: Ariyanna is in DHS custody. Reporter was in a session with the child today. She said a neighbor (Ann Moore) is at the home often with her children. Ann has two children in her home. Ann choked Ariyanna on two different occasions, pushed her head into the washing machine, and screamed at her. This has happened twice. Reporter observed a scar on the child's neck today. She said it was from Ann's long fingernails when Ann scratched her. Ariyanna said she didn't know where the foster parent was at first then said she was in her bedroom. She didn't tell the foster parent. Reporter did speak with Ariyanna's case worker. Ariyanna will change homes within a week. She doesn't need immediate medical
The service user I chose to assess is a 17-year-old Hispanic male whose modified name is Ricardo Sanchez. I provided casework services to Ricardo for two years in my previous role as a Therapeutic Caseworker at my current workplace, the Children’s Village, a foster care agency. Ricardo was placed in foster care after he was removed from his father’s care due to verbal and physical abuse. Ricardo’s father assumed his custody after Ricardo’s mother passed over a drug overdose when he was 15. This was Ricardo’s second time in foster care. Ricardo’s first foster care experience happened when he was 10 years-old as a result of alleged physical abuse by his mother, who at the time had full custody of Ricardo. The report was made by Ricardo’s third grade teacher after noticing bruises on Ricardo’s arms. At the time, Ricardo was only in foster care for two months as
Ms. Henderson notes that during the assessment process Aunika was placed with her for an evening, but was then removed and placed into foster care. She reports being confused by this decision, as her nephew, the son of her fiancé, was able to stay in the placement at her home with her and his biological father, Glenn Stillmen. I discussed with Ms. Henderson the difference between placing a child with a legal parent, which I clarified with her, Mr. Stillman is, and placing a child with a relative, which would be what the placement between she and Aunika, would be. I clarified further, that with a relative placement there are certain certification requirements that must be reached in order for the child to reside there, while they are a ward of the courts and DHS Child Welfare is charged with their safety. A report was received on 06/05/2017 alleging that the mother (Hermionne) left Ashante (C-V 17) with a non-relative since 02/2017 without any legal rights. According to the report, the mother refuses to take her child back home and will not engage with Ms. Aarons (caretaker) to provide legal documentation for the child to be enroll in school and taken to a Primary Care Physician. Ashante has not been is school for the past 4 months and are unable to enroll in school without paperwork. According to the report, Ashante self mutilates her arms, and the mother refuses to get counseling. The report indicates Ashante was to follow-up with a Cardiologist for a chest pain and the mother refuses to take her to the doctor.
EREPORT # 19021 stated the following: The incident happened last night, 2/8/2016. Destiny's mother has "taken off" as evidenced by empty drawers and closets. Destiny stayed home alone all night by herself. The concern is that Destiny is not supposed to be alone with her dad, per DHS. Dad never came home last night, but the concern is 1) dad comes home and she is alone with him or 2) dad doesn't come home and she is alone again by herself all night. The case was brought to DCFS attention when a reporter stated on 7/11/17, OPWI attempted to obtain an emergency order of protection on behalf of Kiara (age 7). Reportedly Ashley (mother) is actively using METH and also methodone she gets from a clinic, and is also involved in prostitution. Ashley told OPWI that Ashley ''can't wait outside for the bus to go to drug court because she's done bad things to people (robbed people, committed home invasion, taken people's money and drugs) and these people are after her.'' Reporter states that one of these people came to OPWI's house looking for Ashley and scared Kiara. Ashley recently got out of jail, stayed with OPWI for a couple of days, left last Friday telling Kiara she would be back in a couple of hours, didn't return
Intake Worker, FPS White received a phone call from the reporter on 11/14/17. The reporter stated that mom went to jail and got out yesterday. The reporter stated that they are not really concern as to mom going to jail due to it was driving with invalid license. The reporter stated that Jessica Fields' oldest son, Raymond Fields called and told the reporter that the reporter needed to come get the children. The reporter stated that Raymond stated that Jessica was on drugs, pills and meth. Jessica drinks all the time. Jessica got beaten up by the boyfriend, Michael Hays in front of the children. He (Issac) told me this the other day and it was about 2 or 3 days ago, “I guess”. I have never reported it. I called today because Ms. Judith with MYPAC came in my home today and told me to make a report. Per intake worker (Gail) was on the phone also. Per Ms. Judith I was his (Issac) therapist with Youth Villages.
Introduction The problems and concerns are a 16-year-old teenager named Tara Jones. Tara has been mandated to the Albert County Court for running away from her fostered home. The situation as of concern is Tara’s behaviors and outburst in the foster home. Tara’s beliefs are, her behaviors are under control and the foster mother needs to be more understanding and get a grasp with dealing with her behaviors. Tara had an argument about dating a 25- year old man.

The current five step program enacted by the Department of Human Services is both inefficient and permissive, calling for minimal action and few opportunities for the reconnection of a family. Often times, case workers of the Department of Human Services (DHS) attempt to persuade young people into removal and treatment with no clear understanding of the challenges that the family faces. Case workers, although inclined to tell you they are there to keep the family together, often lead to tearing a family apart because of the lack of assistance offered by DHS. A study done by Roan Fairbairn and David Murray explored the gaps in care provided by DHS and its workers, with the outcomes of the youth involved; most importantly targeting how case workers interacted with young people, stating that “strategies that attempt to engage young people in a dialogue about their individual circumstances, including drug use, may be more effective than attempts to coerce them into treatment.” They go on to explain that “practioners with the best intentions intervene and inadvertently cause more harm for the clients than if they had not acted at all.” (Fairbairn and Murray 18) Little is available in terms of a nationwide change. In recent years, a call to action has been released with a keen eye on the lack of progress made by DHS to change their system. The client was admitted to the residential program in July of 2016 for her cannabis use. She was referred by Drug Court due to her failure to comply with the program rules. The client has a past of domestic violence and defiant behavior towards her mother and not abiding by curfew.
