I don't even know where to begin, but this is completely unacceptable. My truck TR137 has been down since the end of my shift on Thursday, June 23rd, on my post trip inspection I noted an oil leak and steer axle shock had come unbolted on the bottom, both on the passenger side, both of these items I noted on my VCR. I arrived at the yard on Friday, June 24th at my regular time. On my pre-trip inspection, I noticed that the shock was fixed and that the side of the engine with the leak had been washed, but on the VCR it was only noted that the shock was fixed. I questioned the mechanic on duty, Bill, about the oil leaked, he said they could not find it, but go ahead and run it, we'll see what happens. Well, I only made it two miles
UCM: CPSW met with Mr. Mauer for supervision at Brookdale and the visit went well so far. Mom and dad played with their son. Ms. Mauer changed Kaison's diaper and feed him during the meeting. Kaison seemed little tired today. Mr. Stark joined us today and he mentioned that he was in jail for two days and his case was dropped because it was wrong allegation from a friend. Ms. Mauer, Mr. Stark will be meeting their county nurse today after supervsion visit is done. Kaison is doing well and he had no fever and no coughing during the visit. The foster provider stated that since he got the shot for croup to help him breath better he has been doing well. No fever and no coughing so far according the foster provider. On January 10, 2017, I, Officer LESSANE, with the city of Beaufort Police Department, responded to 1030 Ribaut Road #6, in the city limits of Beaufort, to conduct a follow up on a previously reported domestic (17B01907). Upon arrival, I made contact with the victim SOFIA CHAVEZ and confirmed the offender VICTOR MANUEL CRUZ-ANAYA information. While at the victim residence, I took updated photos of her injuries. She also provided me with a photo of the offender. After clearing from the victim residence, I relocated to the offender job in an attempt to make contact with him. Negative contact was made, no further actions were taken.

We originally did not recognise that the child had Foetal Alcohol Syndrome. The child was a slow developer and one who would have huge tantrums. We didn’t think anything of it but just thought some children develop skills slower than others and tantrum wise we put it down to, some children are just difficult but would eventually grow out of it. We only started suspecting something was different when the child started going to pre-school. If I had not had to go to work; then the child would have not attended pre-school but gone into school, when they would reach school age.
On 04/14/2016 at 2315 hours your Affiant was dispatched to 109 E. Center St. Apartment Upper for an overdose. Upon arriving on scene EMS was tending to the overdose patient. Your Affiant spoke with a female in the apartment who identified herself as Tiara Pleva while Stamets spoke with the male who lived in the apartment Aaron Campbell. Pleva stated Conrad shot up with heroin. Your Affiant asked where the heroin if any left and empty baggies are, and needle Conrad used. Pleva stated she does not know where any of the stuff is.

The entire months I was out of communication, she was looking for me. I was going through all this harassment, abuse, and I could not hear her. I was so stressed that I could not think. Her family was even looking for me, but I put my all into her murder case to help close it. I suffered a great deal of loss dealing with this matter because domestic abuse has forever changed me. I no longer see Arkansas, especially Little Rock the same anymore. I am afraid to come to Little Rock because of the fear of running into him. I’m going to start by saying we have some very good educators at Durant Public School that, make sure we are well prepared for college. Here at Durant Public School have been wonderful, exciteful, and delightful, I completed ninth, tenth, and eleventh grade here at Durant Public School. I’m currently attending the twelfth grade. Now I’m about giving you a brief summary of my experience at Durant Public School.
October 2008, about 12 p. m, we gathered and rehearsed, which is routine and to remember what to do in case of a firefight and IED explosions. Subsequently, it was followed by prayer and mission manifest. Vehicles were assigned, drivers, personnel and gunners and a routine vehicle check to ensuring that everything is working including the radio and the BFTM (Baseline Forecasting and Tracking Matrix) equipment. About 7 p.m. that evening, I was relieved as the lead driver since I couldn’t see well to drive in the night, the second vehicle was asked to lead. Unfortunately, the lead vehicle doesn't have guard light, making it difficult for the gunner to locate disturb earth. Suddenly, the radio and the BFTM on the vehicle were jammed. Intermittently, we were worried because of our past experience. There was nothing we could do to fix the problem without stopping the vehicle, though, we could switch to one emergency channel to call special OP unit for backup. An attempt didn't work as well, and we were technically stuck. In my mind, I envision something worse would eventually happen. However, I didn't have the authority to abort the mission.

My name is Tien. I am Vietnamese. Currently, I am living in Thu Duc district, in Ho Chi Minh city. I study intermediate level at the Thu Duc college of technology. I don’t live in a dormitory of school because my house is relatively near the school. I live with my parents. I study business administration. I like football, volleyball but I was only playing volleyball in the army, when I was demobilized there was no yard to play. I also listen to music a lot.
One major accomplishment was being a member of Phi Theta Kappa. After community college, I applied for and received the Presidential Scholarship at Jacksonville State University. I am a senior at JSU majoring in occupational safety and health administration. This field of study sparked my interest because of the challenges it presents, the earning potential, the ability to be mobile throughout the business day, and because of the complexities of the field. OSHA continues to evolve in best safety practices and processes as the industrial world strives to maintain a safe and stable work environment. For my major, I am required to work as an intern to acquire work experience which will prepare me for a full-time position upon graduation. I plan to complete my internship program this summer with Compliance Specialist Incorporated located in McCalla, Alabama. CSI is a safety consulting firm that analyzes and advises management with environment health and safety issues in industrial facilities. My career goal is to work at Alabama Power Company or Coosa Valley Electric Cooperative to provide safety coordination and instruction to employees and contractors. When I arrived, I spoke to Donald Irvin Ingvalson DOB: 03-10-1954. Donald stated he was Emily’s step father and that he hadn’t heard from her in several hours, but he was aware of the current situation. Donald also stated that Emily’s 11 year-old daughter was inside the house with his wife Sharon Kay Ingvalson DOB: 12-31-1961. I asked if he was aware of any weapons Emily might have with her and he couldn’t say for sure. Registration – At the onset of the month the Program Coordinator reached out to 66 prospective participants for the OST program. The program was only able to reach 23 parents that expressed interest in the program and requested an enrollment packet. The remaining prospective did not answer, contact numbers were wrong, or numbers were out of service. Overall, the OST mailed out the enrollment packet to over 60 prospective participants. The program looks forward to the registration of the prospective participants it was able
Registration – The Program Coordinator has been meeting with parents and contacting student’s homes to get participants register for the program. Currently there are… student registered for the program. The Program Coordinator will continue to meet with more parents and contact prospective participants to get has many students registered for the program before the school year starts, because registering participants during that time is difficult.
