Initially we had difficulty obtaining his discharge orders as the hospital care manager replied she had forwarded the documentation, but neither I nor Mrs. Bianco had received a copy. Mrs. Bianco made a visit to Mills Creek and met with Mr. Smith at that time she was made aware of an appointment that Mr. Smith attended. Mr. Smith attended a physician appointment on June 28, 2017 with Dr. Leonard, Orthopedic trauma. Mills Creek assisted with the transportation and a mobile x-ray unit was coordinated to obtain x-rays of the femur at the facility as Mr. Smith didn’t bring his slide board to the appointment and couldn’t get onto the radiology table.

As a kind hearted individual who treats everyone with respect and dignity, I feel as though I will be an advantage to this Veteran center nursing program. My last assignment in the military was on a medical surgical unit in which the majority of our census were veterans. During this time, I was able to develop a rapport with our patients that had major health issues. Although these patients were sick, I enjoyed being able to see them and help them whatever they needed. I perceived as though my patients felt better after our encounter and all left with a positive sentiment about me. One example that comes to mind is of a homeless patient I had. Interventions provided during this service: Intensive home based services were provided. WYP praised the client for doing well in school (following directives and no challenging behaviors). WYP assisted the client to engage in a positive activity (connect four) while in the local community. Client was rewarded by watching 5 minutes of the anime clip for participating and 10 minutes for beating the WYP. WYP assisted the client with maintaining a positive relationship with his foster family through positive feedback. WYP encouraged the client to continue to list positive things about his foster family. WYP assisted the client with following directives playing cards. Client was rewarded by watching 5 minutes of his anime for following directives.
a. As current enlisted soldier in the Army I feel that the care that veterans get upon return from war is mediocre at best. They have good health care and pensions , if you don’t suffer from PTSD they don’t care enough for these individuals. Yes they have some help for them but the government looks for ways to not help or to give them as little help as possible. These are men and women are willing to give there life, see people die, kill others, and leave there families all in order to protect our country and they can't get help with the problem they get from going to war.
This evidenced-base proposal paper is to address the lack of communication with the multidisciplinary team with home healthcare cases. This is a problem that is faced everyday by home healthcare nurses and other providers. Home healthcare is independent care provided at home these services can be provided over time and across geographic locations. Home care nurses tackle many responsibilities that include not only the patients but also family concerns and problems. Nurses are at the home front on a daily basis with individuals and need a decision/guidance support system to manage the different levels of care demands. One of the most important factors in maintaining continuity of care is the compliance with documentation. In order for providers to communicate with one another documentation needs to be clear and also requires a standardized nursing language (SNL) (Lee Eun, Lee, & Moorhead , 2009).
Throughout history, veterans have suffered after fighting in wars. Millions of veterans were killed and injured. Veterans do not receive all the care they need. Many veterans are currently homeless or suffering due to the fact that the government is not providing enough. Taking care of veterans costs a lot. Healthcare is needed throughout the world. Veterans need healthcare after serving in wars to treat their problems and injuries. Jobs are also important to everyone. Veterans are jobless after fighting in wars. The government continues to attempt helping veterans but it is not enough.
Imagine being born with a severely weak immune system. A person like this is in constant paranoia because the risk of getting sick with pulmonary disease/infection is extremely high and their life expectancy may be lower than someone with a ‘normal’ or healthy immune system. Primary care and Primary Health Care(PHC) are two terms that appear synonyms and are both used interchangeably, however, both terms contain slightly different meaning. According to Muddon, Hogg, Levitt (2006), Primary Health Care is based on the WHO definition which defines PHC as “an essential part of the health care based on practically, socially acceptable methods and technology that is made universally accessible to anyone in a community, through the full cost of the community and country can afford”. Therefore, if the Health Bus provides services that are culturally appropriate and addresses the needs of clients, it is anticipated that participants would be satisfied with the services and would use the Health Bus services. Evaluation may show that the Health Bus team is addressing other social determinants of health such as reducing social isolation among the clients. As seen in the study by Whelan that indicated that clients felt more socially inclusive as a result of chatting with staff and being treated with respect and dignity at the Health Bus in Toronto (Whelan et al., 2010).
