I was extremely excited about participating in the Criminal Litigation Clinic at the Philadelphia Defenders Association. Not only have I conducted Preliminary Hearings this fall, I will also be developing reports for clients in mitigation. However, my excitement quickly turned into frustration. My first Preliminary Hearing client was a 43-year-old Hispanic male who had been arrested and charged with possession (alleged heroin) and possession with the intent to deliver alleged heroin. Although the field test ruled the drug inconclusive, the seizure analysts revealed that the alleged heroin was actually Fentanyl. In the courtroom, particularly in Philadelphia, Fentanyl is considered worst than heroin because it has an alarming rate of overdose deaths. In some jurisdictions, when a user dies as a result of Fentanyl, authorizes will trace the Fentanyl purchase back to the seller and proceed to charge the seller with manslaughter.

It seems each year that the automobile accidents increase, and there are a high number of death and injuries that follow. Records show that nearly 1.3 million people die in road crashes and 20-50 million are injured. More than half of all traffic death occur among young adults 15-44 years of age( ). Accidents can happen at any time and can be caused by many reasons such as rain, speeding, drug, or alcohol. An automobile accident is what brought the 16-year-old boy to the emergency to be care for by the medical staff there. The patient was seen by the ER staff for several hours, and he was admitted later in the inpatient unit for observation. The next morning when staff tried waking the patient he was died.

Fun fact, in the UK, some hospitals use diamorphine, a generic name of heroin to prescribed as a strong pain medication in patients suffering from myocardial infarction, post-surgical pain, and chronic pain, including end-stage cancer and other terminal illnesses. It is still given over there, instead of using Morphine, because some hospital state it a lot better from pain
Narcotic analgesics, especially morphine are underused for pain control with in the medical field. This underuse is because medical professionals, including doctors, fear patient addiction, side effects and possible lose of their licenses. These fears deny adequate healing and a better quality of life to those who would benefit from a more effective use of these drugs, as done in hospice care.
During my rotation in the operating room at Community medical center, I observed the preoperative, intraoperative, and postoperative care for a patient who underwent a laparoscopic hysterectomy. I believe that an appropriate preoperative plan of care for this patient would have included a full physical exam and an interview for patient history, a pelvic exam to look over and understand the nature of the patient’s complications, blood testing including a CBC and WBC to note any signs of infection or contraindications for the procedure, and a urine test to rule out any urinary tract infections or pregnancy. It would be important to interview the patient and ask questions to determine how the patient is feeling about their procedure and to better assist with any anxiety or pain they may be dealing with preoperatively. It is important to consult with the patient well before the procedure to ensure that she knows to refrain from smoking for at least 8 weeks before the procedure because this reduces the risks of complications such as infections, issues with blood pressure, heart rate, blood flow, and respirations when under anesthesia, and promoting overall health and risks associated with smoking after the procedure. (ASAHQ) It is also important to educate the patient to consume no food or drinks after midnight the night before the scheduled procedure. Opioids drug are a very serious matter that goes around daily without being noticed. Although some precautions have not been a total fail, recent epidemic shows the problems with the Opioid drug addiction is bigger than what we can handle and cannot simply curb the epidemic.

They fulfill these duties providing Americans with adequate and efficient health and human services and monitoring services designed to increase the efficiency of care in the health system (Thacker, 2014). One of the services being monitored by the department of Health and Human Services is the electronic health record system, which carries private and vital information of patient’s health record enabling all eligible participating health workers access to these records (Thacker, 2014). A breach of the protective health information of patients in a health organization creates chaos as these are against the health insurance portability and accountability (HIPAA) law (Thacker, 2014). Hence, measure will have to be put in place to determine what caused the breach and how to rectify it to ensure the breach never happens again (Thacker, 2014).
