Narcotic analgesics, especially morphine are underused for pain control with in the medical field. This underuse is because medical professionals, including doctors, fear patient addiction, side effects and possible lose of their licenses. These fears deny adequate healing and a better quality of life to those who would benefit from a more effective use of these drugs, as done in hospice care.
The physician adjusts the dosage of buprenorphine if withdrawal symptoms or cravings are bothersome or if there are medication-related side effects. Patients visit their physician frequently until progress is on firm ground, usually once weekly. Stabilization lasts about two to four weeks.
This is a reflective essay based on my experience of participating in delivery of anaesthetic and surgical care to a patient undergoing abdominal hysterectomy under general and regional anaesthesia. I will be describing the process involved, my participation and contributions, what I learnt during the experience and how this gained knowledge will improve my professional competency.

Treatment centers, physicians and first responders in the rural areas of the United States need more access to naloxone to prevent opioid-related overdose deaths. The U.S Surgeon General Vivek Murthy stated, “People find themselves in overdose situations don’t have to lose their lives because family members or emergency responders don’t have access to the reversal drug naloxone” (Wolf, 2016). Expanding access to rural areas can save lives and prevent deaths.

Although some precautions have not been a total fail, recent epidemic shows the problems with the Opioid drug addiction is bigger than what we can handle and cannot simply curb the epidemic.
I returned to the recovery ward, my patient was still hypertensive and tachycardic and I felt by assessing her non-verbal signals of communication that she was still in great discomfort. After 15 minutes of no improvement I returned to theatre to see the anaesthetist, I explained that I was not happy with the patient’s level of pain and requested that he come to the recovery ward to assess the patient. He reluctantly came to the recovery ward and after spending a few minutes assessing the patient agreed that she was in an unacceptable level of pain and prescribed a further 5mg of morphine which I duly gave to the patient in 2.5mg increments. After this the patients heart rate and blood pressure decreased to pre operative levels, she seemed to be more relaxed and eventually fell asleep. After a further period of time spent continually reassessing the patient and when I was satisfied she was comfortable and haemodynamically stable I discharged the patient back to the ward.

During my rotation in the operating room at Community medical center, I observed the preoperative, intraoperative, and postoperative care for a patient who underwent a laparoscopic hysterectomy. I believe that an appropriate preoperative plan of care for this patient would have included a full physical exam and an interview for patient history, a pelvic exam to look over and understand the nature of the patient’s complications, blood testing including a CBC and WBC to note any signs of infection or contraindications for the procedure, and a urine test to rule out any urinary tract infections or pregnancy. It would be important to interview the patient and ask questions to determine how the patient is feeling about their procedure and to better assist with any anxiety or pain they may be dealing with preoperatively. It is important to consult with the patient well before the procedure to ensure that she knows to refrain from smoking for at least 8 weeks before the procedure because this reduces the risks of complications such as infections, issues with blood pressure, heart rate, blood flow, and respirations when under anesthesia, and promoting overall health and risks associated with smoking after the procedure.
