Facts of the Case: On March 17, 1980, a 14 year-old student and her classmate in Piscataway Township High School had gotten caught smoking cigarettes in the public school bathroom by a teacher. This violated the school rule. The teacher took them to Theodore Choplick, the assistant principal’s office. The assistant principal then questioned girls about smoking in the bathroom. One girl admitted to smoking. However, one female denied that they had been smoking at all. The assistant principal requested that he look into the student’s purse. SUMMARY OF ALLEGATIONS A report was received on 06/05/2017 alleging that the mother (Hermionne) left Ashante (C-V 17) with a non-relative since 02/2017 without any legal rights. According to the report, the mother refuses to take her child back home and will not engage with Ms. Aarons (caretaker) to provide legal documentation for the child to be enroll in school and taken to a Primary Care Physician. Ashante has not been is school for the past 4 months and are unable to enroll in school without paperwork. According to the report, Ashante self mutilates her arms, and the mother refuses to get counseling. The report indicates Ashante was to follow-up with a Cardiologist for a chest pain and the mother refuses to take her to the doctor.
On 02/11/2017 I was Dispatched to Smith Springs Elementary School for an investigation. A student Cire Kistner, was reported as being visibly distraught upon her arrival at school. She was taken to Lance Forman's office the schools principal and was questioned. The student reportedly told Forman that she and her sisters walked into the bathroom and witnessed their father, Eric Kistner, with a needle in his mouth and blood on his forehead. He was passed out on the floor. There was a spoon with "black watery play dough" on the counter. Principal Forman contacted DCS and informed them of the situation as well as MNPD. Metro P.D held the students at school, while also performing a welfare check on their father at the Kistner residence and calling
On 9-17-17, the unknown male child's grandmother Faye; asked me to look at the child back. The child's left shoulder blade had a scar. The scar appeared to like someone used their two middle fingers and dug into his skin. The child was bleeding and scared. The child's grandfather (unknown) did this allegation. Faye called the child to come to her. The child was afraid to come to the grandmother. Faye stated told the child "I am not going to do anything to you." The child finally came to Faye. The child did not seek medical attention. The child's mother *(Santana) was not at home. It is unknown if Santana knows about this incident. The reporting party (RP) stated referral #1143-3655-1102-7078895 was generated from the following referral #0534-9062-5752-307279 dated 1/28/15 regarding resident Nicole Morris age 14 DOB: 6/1/00. Per the referral Nicole is an alleged victim of sexual abuse, physical abuse and general neglect by an unknown perpetrator. According to the RP on 1/28/15 a staff member named Rebecca took Nicole to Long Beach Memorial Hospital clinic to be examined after she disclosed having suicidal ideation. Nicole revealed to hospital staff that she planned to hang herself. While waiting at the hospital Nicole disclosed being "fingered" by two different boys (names unknown) at the home. Nicole also disclosed that a girl (name not provided) beat her up. Referral 0290-1132-4581-1002176 dated 3/13/15 reported by Tatiana Garcia, Clinician with Bayfront Youth Group home.
Facts: In a New Jersey high school, two freshman girls were caught smoking in a bathroom. When taken to the Assistant Vice Principal, the first student admitted to smoking while the second, designated T.L.O due to her age, denied smoking in a restricted area. The VP proceeded to take student into his private office and searched her purse and discovered a pack of cigarettes. However, as he was searching, he also identified a stack of rolling papers, which led to a more thorough search of the purse which produced a small amount of marijuana, a pipe, a number of empty plastic bags, a significant amount of money, as wells an index card with names of various people who owed T.L.O money. This evidence implicated that the student was dealing, and T.L.O admitted to selling marijuana. The authorities were called and T.L.O was arrested.

On 09/23/2016 at approximately 12:00 AM, this Investigator made contact with Yago Krause (DOB: 01/27/00) at his home located at 4101 E. Baseline Rd. Apt. 1024 Gilbert, AZ. According to Yago, he has been home alone for the past 2 to 3 days since he mother was admitted to the hospital. Yago stated that he was at school when his friend Mason Hicks (DOB: 01-28-97) called him about his mother having an overdose. Yago stated that Mason started living in the home about one day prior to his mother 's overdose. Yago said that he asked hospital works what to do since he is by himself. Yago stated that his step father left just before his mother had an overdose. Yago brought this Investigator to a family friend 's home who also lived in the complex. Yago at first refused to go with DCS but cooperated. Yago stated she feels safe at home and with her mother. Yago denied any substance abuse, criminal history, domestic violence, physical abuse, sexual abuse, or emotional abuse. Since Yago 's mother is hospitalized, he was placed with a family friend.
The case was brought to DCFS attention when a reporter stated on 7/11/17, OPWI attempted to obtain an emergency order of protection on behalf of Kiara (age 7). Reportedly Ashley (mother) is actively using METH and also methodone she gets from a clinic, and is also involved in prostitution. Ashley told OPWI that Ashley ''can't wait outside for the bus to go to drug court because she's done bad things to people (robbed people, committed home invasion, taken people's money and drugs) and these people are after her.'' Reporter states that one of these people came to OPWI's house looking for Ashley and scared Kiara. Ashley recently got out of jail, stayed with OPWI for a couple of days, left last Friday telling Kiara she would be back in a couple of hours, didn't return
This supplement is a record of my actions in this investigation. On May 23, 2016, I was assigned this case to follow up. This case involves Ms. Anjelica Cassidy Grim a 17 year old T.C. Williams High School student. I made multiple attempts to located Anjelica at School and at 27 South French Street (home). On May 27, 2016, I spoke to Sal Gnim (mother) who reported her daughter returned home the next day. She confirmed that her daughter had been out with her boyfriend. Sal told me that they had a conversation and cleared the conflict up. Sal reported that this was the first time her daughter had runaway. I received permission from Sal to speak to her daughter in School.
This supplement is a record of my actions in this investigation. On July 6, 2015, I was assigned this case to follow up. This case involves Mr. Tavon Lanier a fourteen year old male. Mr. Lanier’s mother, Ashauta Lanier reported her son ran away on July 4, 2015. On July 7, 2015, I spoke to Mr. Don Earlington Intake Officer of the Court Service Unit. He confirmed an active detention order for Mr. Lanier. Mr. Earlington confirmed previous runaway events and additional incidents in Maryland where Mr. Lanier had taken a handgun to school. Mr. Lanier’s current Probation Officer is Macalister.
PER REPORTER: Richard was arrested on 6/17/2015. The reporter said Graysin told her that “daddy went to jail and I was with him”. Richard was arrested for possession of a control substance. It is unknown by the reporter what kind of control substance. Per reporter Richard’s girlfriend Amanda was arrested on 1/1/2015 for a felony charge for possession of a control substance. It is unknown where Graysin was during the time of her arrest. It is unknown by the reporter is Garysin has been hurt or harm due to Richard and Amanda having possession to control substances. It is unknown if the substances are been manufactured in the home. It is unknown if Graysin has access to the substances. It is unknown if they are using the substance around Graysin.
EREPORT # 18573 statedthe following: The incident took place on today. DFCS worker accompanied Investigation Terry Smith to 317 West Main St, Sardis, MS due to reports coming to the Sheriffs Department alleging that Dakota, his girlfriend Kynli, and Dalton are staying in a home alone with no adults. The concerns individual also stated to Invest. Smith that the children were not in school. Kynli is pregnant and will be having her baby within the next month. After going to the home Invest. Smith and DFCS worker were able to assess that the allegations were true and the home was filthy and no adults were in the home and the children were not going to school. Introduction The following is a case study of a client by the name of Lydia Sanchez. Lydia has been truant 20 times within the past 3 months. Lydia teacher Ms. Ellis sent a referral to the main office because Lydia came in her classroom smelling like alcohol, and marijuana. Ms. Ellis also noted in the referral that Lydia pushed her when she tried to look inside of her backpack. The principal found cigarettes, and a white water bottle filled with alcohol inside Lydia’s backpack. Ms. Ellis noted in the referral that Lydia has not handed in any assignments this year yet. Ms. Ellis told me that Lydia was a straight a student last year, and that she does not know what has gotten into her. Lydia’s mother Stephanie also called into the school stating that her daughter Lydia needs help, and that she has been dating a 21 year old man for the past 6 months. Stephanie does not know what to do with Lydia anymore, and she is worried that she might drop out of high school.
Per Reporter: Patricia has an open investigation in Pearl River due to drug usage. On 8/18/17, Daniel picked up Julie from Patricia’s home. A drug screen and home visit was requested for Forrest County. Shalacey (previous case worker) was unable to make contact with Daniel. There has not been an update or information in regards to the home visit or drug screen via Kadedra (current case worker). Jennifer spoke with Daniel via phone. Daniel informed Jennifer that he was able to meet Julie’s basic needs, but has not been confirmed. Patricia’s case is approaching its 30 days, which is approaching closure. Patricia is now stating Daniel is on drugs, but did not go into details. It is unknown if the drugs are used around the children. It is unknown
