EREPORT # 19021 stated the following: The incident happened last night, 2/8/2016. Destiny's mother has "taken off" as evidenced by empty drawers and closets. Destiny stayed home alone all night by herself. The concern is that Destiny is not supposed to be alone with her dad, per DHS. Dad never came home last night, but the concern is 1) dad comes home and she is alone with him or 2) dad doesn't come home and she is alone again by herself all night. I found out abou the situation because the teacher told me. estiny disclosed to her teacher about being home alone, mother "taken off" and that dad may come home and she has to be alone with him. SUMMARY OF ALLEGATIONS A report was received on 06/05/2017 alleging that the mother (Hermionne) left Ashante (C-V 17) with a non-relative since 02/2017 without any legal rights. According to the report, the mother refuses to take her child back home and will not engage with Ms. Aarons (caretaker) to provide legal documentation for the child to be enroll in school and taken to a Primary Care Physician. Ashante has not been is school for the past 4 months and are unable to enroll in school without paperwork. According to the report, Ashante self mutilates her arms, and the mother refuses to get counseling. The report indicates Ashante was to follow-up with a Cardiologist for a chest pain and the mother refuses to take her to the doctor.

Martin Proctor, a father of two, was recently accused of abusing his baby and eight-year- old daughters, placing them in Highland District Hospital. The children then were transferred to Cincinnati Children’s Hospital later being released and put under the care of Child Protective Services and placed in foster homes. During the stay at the hospital, the children had fractures of several bones, placed into a coma after having several seizures, and doctors found evidence of previous abuse. EREPORT # 19130 stated the following: The child is at Diamond Grove Center. Mya reported that 7 years ago her cousin Charron raped her when she was 8 years old. They stayed overnight with their relatives and Charron took off her clothes while they all slept in the same bed. Onautyca, Deja, her, and Charron slept in the big bed together. Mya reports thst she told her mother, but her mother didnt do anything about it. Mya reports that she sees Charron often at church or the weekends, and her mother continues to take her and her sisters around him. UCM: CPSW received a call from Ms. Messerli reporting that she needs the lock number for her storage from foster provider family. She also stated that she needs the boy's cell phone back. CPSW reminded Ms. Messerli that this writer will ask the foster provider about the phone and storage numbers. CPSW reminded Ms. Messerli to focus on her recovery and not to disrupt her children as well as the foster provider and if she needs anyting to let the CPSW aware of it. Also, CPSW reminded Ms. Messerli not to contact or call both children until the theripist approves it. Ms. Messerli stated that she will try. Howerver, Ms Messerli sounded unhappy and really upset about not contacting her children Nick and Alexander.
Ms. Myiesha Leonard reported that her understanding of why the children were removed from their home was because the mother called in to CPS and reported that she could not care for the children anymore. Ms. Leonard did not clearly describe the allegations of abuse and/or neglect and she understands her brother mental needs but does not believe he was violence toward anyone. She reported knowing about the abuse or neglect of the children. Ms. Leonard appears to need additional information about the abuse and/or neglect of the children because of not knowing what took place. RCG Rose Campos reports no health problems. Ms. Campos also reported that she has had no problems with Elijah at home. However, I am concerned that Elija is not safe in his present home with, the maternal grandmother due to Austin’s behavior. Ms. Campos reports no physical harm to Elijah by Austin, but Elijah has witnessed Austin’s violent and abusive behavior to Ms. Campos. I am concerned about Austin’s exposure to this violent and abusive behavior to his grandmother.
The reporting party (RP) stated the foster mother Yolanda Walker recently began caring for foster child Kylie Jane' Davis age 3. According to the RP the child began exhibiting behaviors resident to sexual abuse. The foster mother stated the child would go into a fetal position during diaper changes. Initially the foster parent believes it was due to her new environment, however the behavior continued. Subsequently the foster mother spoke with the foster child's biological mother Teena Pitts who disclosed of finding her daughter sitting on her ex-boyfriends lap wearing no diaper or bottoms. Consequently the mother did not have the child examine.
I have been in contact with Ms Mullen almost every day since last week. Ms Mullen was informed that, after two investigation, there is no evidence that her grandson is being abused by CLCM caseworkers. She said that is not going to stop calling this office and Dr Richardson’s office. She believes that OAAPI investigators violated her rights because they disclosed confidential information to CLCM Investigator, Karen Johnson. She did not provide any proof of it.
Throughout the course of 6/22/15- 6/27/ Igoe REACH has come into the home of Jessica to observe her behavior. The behaviors, which Jessica engages in, place her in harm of herself and her peers. Monday she engaged in elopement, which then led to a sit down strike in the middle of the road. The incident required two staff to physically escort her in the home while trying to deescalate the behavior, which included screaming and kicking staff, was punched in the face another staff was also forced in a ditch that was filled with mud. All these incidents happened while in the presence of her support coordinator for Prince William County. Collin another in home counselor came to observe Jessica. I made contact with him on his second time with Jessica. Collin was unaware of Jessica normal behavior so staff shared with him that he should, “observe” her getting of the bus. Immediately entering the house she started to engage in behaviors that led staff to verbal redirection. This took place for about 45 minutes until the AD for Community Concepts (Regina Goldston) advised him to back away and observe from a distance. It has been noted that Jessica will engage in more behavior with an increase presence of staff. Collin spoke with the home manager about the normal behaviors. The OM asked what exactly could reach provide to Jessica as far as support goes.
