Mr. Jose Murillo was hired on December 22, 2014 by the Los Angeles County Fire Department as an Information Systems Analyst I. Mr. Murillo is currently assigned to the Business Analysis Section within the Information Management Division. He successfully completed his probationary period on June 21, 2015. This report highlights his performance that resulted in his final appointment.
During this probationary rating period, Mr. Murillo was assigned to support:
• Implementation of Documentum, an Electronic Document Management System (EDMS) for HHMD. o Served as a liaison between the users and software vendor support staff. o Collaborated with lead analyst, software designers and subject matter experts to establish the technical vision and analyzed tradeoffs between usability and the performance needs of the system. o Performed and coordinated user acceptance testing (UAT) with client, managed issues and the subsequent resolution for system related defects and production issues. o Assisted in defining quality assurance testing strategy, identifying risks and functional workarounds to resolve application problems.
• Support analyst for HHMD, Envision software. o Facilitated the Consolidated Uniform Participating Agency (CUPA) Billing for FY2015-16.

Murillo has demonstrated very high quality work. During this rating period, he produced training materials for LNS and MPS that were detailed, clear, and well-constructed. On several occasions, Mr. Murillo was asked to provide training for ODD and the Help Desk staff which resulted in positive feedback. His ability to instruct and teach has proven to be valuable to the Help Desk team. He frequently receives positive feedbacks from both IMD and OD staff on his ability to provide detailed information. He consistently provided detailed analysis when asked to assess a situation or troubleshoot an issue. Mr. Murillo is an active listener and manages his time

Another way of successfully gathering information is by building a prototype or model of the system, so that users can test or get an idea of what the finished product will be like. With this they can determine issues, problems, or inconsistency with the system. Another important part of gathering information is organizing it so that it can be understood and put to proper use. I propose categorizing the requirements into functional requirements, operational requirements, technical requirements, and transitional requirements. The functional requirements define how the user thinks the system is functioning overall, the operational requirements define what background processes need to be executed in order for the system to work optimally over a period of time, the technical requirements define what technical issues that must be addressed in order to successfully implement the system, and the transitional requirements define the processes or steps needed to implement the system smoothly and successfully. ("Mind Tools", 2012).
The implementation portion of the Riordan project could in fact, prove to be one of the most challenging parts. However, this could be the part that is worth all of the effort. There will have to be a team of experienced developers in various different Information Technology fields to see the implementation portion stage through to completion; this cannot be known as a “one-man show.” Several of the departments within the Riordan organization will have to be given priorities when it comes to implementing the new human resource management system. MMCC, a Medicaid managed care organization, is located in the low-income community of Baltimore City. It is positioned across from a major medical center that does not provide its services. MMCC seeks to reduce their medical loss ratio (MLRs). As part of their strategy, they enroll poor people of the community through incentives, utilize a distant medical center to provide their services, and cut back on administrative requirements. To get communities members to enroll, they offer turkeys to enrollees during holiday times. They fail to inform members that they must use the medical center across the city and not the one directly across from their location. Once a member of MMCC, the patients experience poor quality care. Operators are limited and cannot keep up with calls to make appointments. Health care delivery site’s parking lot has limited parking spaces. The waiting room has limited seating. The actions of MMCC are legally and ethically questionable.
Listo Systems is a graphic service agency and is one of the leading companies within the sector. They recently implemented a new black box technology which resulted in a demand for trainings for the employees as well as “train-the trainer” sessions. McCall lists the following attributes as part of a quality software product: correctness, reliability, efficiency, integrity, usability, maintainability, flexibility, testability, portability, reusability, and interoperability. These quality attributes will be used as evaluating factors in determining whether scenario-based usability engineering or software architecture-based design can help significantly increase the chances of meeting a project’s goal with respect to that quality attribute. This analysis should provide an overarching view as to which design scenario is best suited for a project given a set of desired quality goals. Below is an introduction to the two methods being compared.

The coding category includes Certified Coding Associate (CCA); Certified Coding Specialist (CCS); and Certified Coding Specialist-Physician-based (CCS-P). Lastly, observation is performed by using questionnaire form. According to Burch (1992), the questionnaire is analyzed and transform into structured form that easy to understand. After all information has been collected, structuring of system requirement takes place. It focused on development process modeling which perform “graphically representing the process, or actions, that capture, manipulate, store, and distribute data between a system and environment” (Hoffer, George, & Valacich, 2012, p. 182). In this step, Data flow diagram (DFD) is structured by system analyst using special tools and techniques to create a decision table. According to Hoffer, George and Valacich (2012), decision table is a “diagram of process logic where the logic is reasonably complicated” (p. 200). This table is useful to help system analyst to make a decision toward the project. Then all information’s gained from this phase are documented in System Analysis Report (SAR) that acts as a guideline or reference to the future system development project (Burch, 1992).
