My Client Has Suffered From Multiple Diagnoses During His Time At Sonoma Developmental Center ( Sdc )

I will be using the patient initials (MK), throughout this paper when I reference her. I asked the patient a list of questions about her life and I will summarize the answers for you now. MK is an alert and oriented 84 year-old female who is happily married and lives with her husband of 56 years. MK is retired and worked as a high school secretary for 20 years in the town she lived in. She grew up in Pennsylvania with her parents and was the oldest of five children. Her family medical history includes cardiac disease and diabetes with all of her brothers and sisters still alive. MK has three of her own children who all live with in a 30 minute drive from her home and they speak frequently on the phone with her and her husband. MK and her husband are going on a vacation together this summer to South Carolina with her son and his family. MK still hosts holidays and family gatherings at her home with only a couple holidays hosted by her son at his home. MK and her husband receive social security and her husband’s pension as their income. They have Medicare and also a supplemental insurance through AARP. MK enjoys her exercise class, reading, her card club, traveling and is also very active in her church including being on the bereavement committee. MK still drives her own car around town to visit friends and shop but prefers not to drive at night

When I first decided that I wanted to further my education in nursing, I was inspired by my job as a Home-Health Aide. At the age of sixteen, I started working for a company called Miracles In Motions. I worked with individuals who are mentally and physically disabled; assisting them with all their daily needs. One day on my job, I was introduced to Mr. Peloquin, a man with Cerebral Palsy. I introduced myself to him and he began telling me stories about himself and his disability. I could tell that he enjoyed my company and he expressed that he does not get to tell his stories often. The story that really caught my interest involved his recent hospital visit. He told me how mistreated he had been by the hospital staff due to his disability; there was no interaction from the health professionals, and he was not treated like a human being. A.O. went to foster care for two months, but his father gained custody in July 2012. The father mentioned the foster care had an effect on A.O. A.O. lives with his father and his grandmother. The father works at a thrift store. A.O.’s Mother has Neurofibromatosis Type I too. The father mentioned that A.O. had a speech delay. A.J.’s father mentioned that they received assistance from the government. A.O.’s grandmother and father reported that he eats a good variety of food as he eats fruits and vegetables. This client’s name is Tyler Harker. (He is a fourteen-year-old male student who has type 1 diabetes mellitus. Tyler’s primary caregiver is himself and his mother. He is in the psychosocial stage of identity versus role confusion. The cognitive stage of this client is formal operational. Tyler is the learner. He speaks English and does not have any disabilities related to learning, has no other pre-existing health conditions, and no sociocultural factors that may impede learning. The client identifies his strengths as being able to take care of himself and being independent. His motivation to learn seems to be strong as evidenced by behaviors of listening, nodding, and sitting straight up in a chair with arms open. The client has good reading and comprehension at his age level as indicated by him reading out of his school history book. Indications that suggest the client has the ability to problem solve include him being able to come up with concepts and alternatives. The home environment is very clean, uncluttered and organized and requires no modifications. If the client cannot carry out his care, his mom and dad are both available and in-tune with how to give Tyler’s care. Other support available for Tyler include his peers, church group, and diabetes camp he attends yearly. Tyler is ready to deal with his own self-care and motivated to learn [Hunt, 2009].)

As I was getting ready to enter the patient’s room, I was thinking about the many ways I could start a conversation with her. I had previously entered her room and I knew she was one of the sweetest and happiest people ever. I decided to write my assignment on her because of her inspiring personality, despite her physical condition. I was amazed to see how a person who could do nothing but the bare minimum was so happy and thankful towards life. I spent a lot of time with her through out the day. *Student A engaged in great eye contact with her patient, which I found to be great because I noticed he found comfort in that. He would smile and engage in conversation, when initially he had a very flat effect and didn’t talk much at all. She had a very neat appearance hair pinned up appropriately and clothing and shoes clean and tidy, which in return makes the patient comfortable because an ill person does not want an unprofessional looking individual giving them care. She displayed appropriate gestures, informing the patient prior to any contact what she was going to do prior to doing so, in return the patient thanked her so much for being so patient and understanding with him. Student A displayed great vocal expression during her assessment. She was so thorough while using a reassuring tone, engaging in conversation during her assessment as well as being very tentative to everything the patient was saying, and any questions he had.

Vanessa started to lose weight, have hysterical behavior and intense sadness after her father dies. Who has a full time nurse with him to help with his medication and dietary needs, which means when he wakes up there is a nurse there and when he goes to sleep the nurse leaves. Colton has been homebound most of his life except for a short attempt to attend preschool. His interests are on a wide range from week to week. He mostly enjoys talking about becoming a stop sign man when he becomes an adult. He has a younger sister who attends school on a regular basis. Part A Anna’s Story Female, 32 years old, Parent & Carer Anna is a 32-year-old full-time mother to two children aged 2 and 4 years old and is expecting her 3rd child to arrive soon; Anna’s husband is at work full-time, and sometimes overtime to support the family financially. A few months prior to becoming an informal carer for her husband’s parents Anna lost her mother, and was one of the support networks for her Father, with one child at the time. Anna is the informal carer to her Father-in-Law who suffers from heart conditions and was to her Mother-in-law who had knee re-construction surgery on both of her knees. Ten months after; Anna gave birth to her 2nd child and her mother-in-law had her surgery; she unfortunately did not recover as to plan and had developed dementia, approximately 2 years later Anna’s mother- in-law passed away, while she is 7 months pregnant with her 3rd child.

The client that I chose to write about is my client by the name of JB. He is a low class, white, male, age 7, and is diagnosed with ADHD. His mother was being seen by me originally by me but she decided she wanted to do a different kind of therapy to treat her past trauma. She insisted on having her son come into therapy because she thinks he may have been affected by her actions as he is growing up. JB’s mother is a recovering drug and alcohol addict. She was recently released from rehab in March and has been sober for nine months now. During my first session with JB’s mom we did her family's genogram. I did not get a chance to do a genogram for JB for I do know from our sessions that he as a step brother and two half siblings on his dad’s side but is the only child to his mother. Our first session was Thursday August 24, 2017. I wasn’t able to get him to complete the survey for the PRN because his mother opted out of it her first session because the tablets were not working so we didn’t do it for him either. We have had two sessions together so far and I can definitely see us building a therapeutic relationship.
