Introduction My client is Margie M, a 69-year-old widower with chronic venous stasis ulcers. PMH: Asthma, Cataracts, Arthritis, Tremors, Narcolepsy, Congestive Heart Failure, Diabetes Mellitus, Peripheral Vascular Disease, Hyperlipidemia, Hypothyroidism, Gastric Ulcers, Irritable Bowel Syndrome, Micro colitis, Restless Leg Syndrome, Hypertension, Atrial Fibrillation, Anxiety, Depression, Cardiac Stents, and Cerebral Aneurysm. She lives in a two bed, one bathroom house with her daughter Becky, Becky’s husband Jim, and her twenty-five year old grandson Jason. Margie’s daughter, Terri, Jason’s mother is deceased. Jason has mild developmental delays. Margie has one son, Buster, who is not involved in her care. We met for lunch at McDonald’s…show more content…
Becky is doing her dressing changes, but stated it is hard to get the supplies since she was discharged from home health. Jason also came to live with her after his mother’s death. They now have four adults in a two-bedroom house and this has led to decreased intimacy between Becky and her husband. I believe they are a loving family and that is their strength. Becky tries to do everything on her own and does not ask for help. That is their weakness. I feel they could decrease caregiver role strain by delegating some of the responsibility to other family members. Nursing Theorist Dorothea Orem’s Self Care Model states, “Nursing care is required when an adult is unable to perform self-care sufficiently to sustain life, maintain health, recover from disease or injury, or cope with the effects of disease or injury” (Orem, 1991). Orem also takes into account that caregivers also need nursing care. Six concepts: 1. Self-care requisites. A. Universal- Margie’s daughter prepares her meals and encourages her to eat. She is able to swallow without difficulty and her appetite is fair. She does need prompting and set up, as she forgets simple things like putting a straw in a cup or cutting up a piece of meat. She is continent of bowl and bladder and bathes herself with minimal assistance. She tends to isolate herself, but enjoys cooking with her daughter.
