On 04/14/2016 at 2315 hours your Affiant was dispatched to 109 E. Center St. Apartment Upper for an overdose. Upon arriving on scene EMS was tending to the overdose patient. Your Affiant spoke with a female in the apartment who identified herself as Tiara Pleva while Stamets spoke with the male who lived in the apartment Aaron Campbell. Pleva stated Conrad shot up with heroin. Your Affiant asked where the heroin if any left and empty baggies are, and needle Conrad used. Pleva stated she does not know where any of the stuff is.

On 10/30/2017, I Deputy Mignone #95 was assigned to Intake/Release. At approximately 1600 hours, I was called to cell HD-6 to assist with giving an inmate his medication. I met my supervisor Sergeant Johnson #150 along with multiple other officer. I witness inmate Matthew Spradlin standing in the cell with feces on his head and face. Inmate Spradlin stated that he had been eating his own feces and drinking his own urine. I was informed that Dr. Dahner had ordered a shot due to inmate Spradlin’s behavior.

The BSW program here at Wayne State gave me excellent preparation for the MSW program. While in the BSW program, there were several sacrifices that I had to make socially and financially in order to do well in school. For a year and half I maintained two jobs, while also going to school full time and being involved in extracurricular activities. At first everything seemed easy to manage. On 05-21-2016 at 0834 hours I was contacted at the front desk by April Sutton in reference to a stolen motor vehicle and stolen wallet.

At approximately 1400hrs on Wednesday, March 16,2016 I arrived at 7810 Shaver Rd for my internship with the Portage Department of Public Safety. I entered the station and put on my vest for my ride along. I then proceeded to the sergeant's office where I received the name of the officer I would be riding with for the day. I was scheduled to ride with Officer Vanportfleet for the day. Vanportfleet was on a call at the time so I went with Officer Dmoch. The First thing that we did was go to a neighborhood watch meeting for millim meadows. Jason was the owner and he talked about how they would be implementing the Silent Observer Program for the residents to help deter crime in the neighborhood. He also talked about the new security cameras that they would be installing

On Thursday, December 14, 2017 at approximately 9:40am I, Officer Jaren Wren was conducting Supervision Sanction Program (SSP) pill call and observed that Resident Jessica Harris ACC# 714001 hair was braided. I asked Resident Harris who braided her hair she stated she did. I told Resident Harris to step out of line, remove a braid and redo it. Resident Harris began taking the braid down, but couldn't re-braid it. I then gave Resident Harris a direct order to remove the braids, and in a very aggressive tone she said, "No!" I then told Resident Harris to face the wall to be placed in restraints. She then said, "Bitch don't put your hands on me!" and began resisting at that time.

On 09/04/2015 at 20:32 hours, I conducted a motor vehicle stop of a Ford F-150 bearing NJ registration X70DNC for failing to maintain lane on Hurffville going northbound. I activated my overhead lights and sirens and conducted the stop in the A15 lot of the Deptford Mall.

On 8/25/17, I attended Mr. Barrera’s follow-up appointment with Dr. Dietzel to evaluate the status of his left shoulder postoperative. X-rays were taken prior to him being seen.

Morris Brandon Elementary School feeder to Sutton Middle School. My middle school years were not as successful as my elementary years. I struggled academically due to the financial hardship my family endured. My father was a successful business owner and my mother was a stay home mom; however, the family business collapsed due to tax problems with the IRS (internal revenue). Unfortunately, we lost our home and had to move with a family member. My father did not move with us, which the family no longer lived together. This had a major effect on my middle school years. The sudden lack of stability and continuity brought about academic and behavioral problems.

On January 29, 2018 the dispatcher put out a call of shots being fired at College Villa apartments. I responded to the call and made my in that direction from the station. I was in route when the dispatcher said that individuals were getting into a white vehicle and leaving the apartments. Sergeant Holter was in front of me as we were driving south on Ladnier road. He spotted a vehicle leaving College Villa apartments and turn onto West Park drive. It matched the description given and he followed it. He followed it as it stopped at a house on West Park. He advised that someone was running from the vehicle and headed towards College Park school. I passed West Park and drove to the school parking lot parallel to Holter's position. I found no one in the area and proceeded to go to College Villa. I made my wy to the back of the complex and saw a black male standing outside of one of the apartments.
